BACKGROUND: Resection of the thoracic duct is part of the formal en bloc mediastinal esophagolymphadenectomy for cancer, although with the adaptation of minimally invasive techniques, some centers started to leave the thoracic duct compartment in situ. However, previous studies reported thoracic duct lymph nodes in this compartment that may contain metastasis. The aim of this study was to assess the presence and number of lymph nodes in the fatty tissue surrounding the thoracic duct.
METHODS: A right-sided thoracoscopic esophagectomy was performed on seven fresh-frozen human cadavers (male, n

BACKGROUND: Chylothorax after transthoracic esophagectomy for cancer is an uncommon but potentially life-threatening postoperative complication. It has been reported that preventive thoracic duct ligation can reduce the incidence of postoperative chylothorax after esophagectomy for cancer. In this prospective series, we evaluated the results of preventive intraoperative thoracic duct mass ligation in patients who underwent transthoracic esophagectomy for cancer.
METHODS: From 2001 to 2006, 323 patients underwent transthoracic esophagectomy for cancer and duct ligation during the operation was

Iatrogenic chylothorax is an uncommon thoracic duct injury, but well-known and serious complication after thoracic surgeries. Recently, video-assisted thoracoscopic surgery has been applied in the management of postoperative chylothorax and could be indicated earlier. We report herein a case of postoperative chylothorax managed successfully by early thoracoscopic direct suture repair of the site of chylous leak that developed after the thoracoscopic resection of mediastinal mass and surrounding fat with ectopic thymus in a patient with persistent myasthenia gravis with non-Hodgkin lymphoma.

BACKGROUND & OBJECTIVE: Prophylactic ligation of thoracic duct during the radical resection of esophageal carcinoma is usually used to prevent and treat chylothorax, but there is dispute about the effect. Its correlation to other complications and prognosis of esophageal carcinoma patients after operation has seldom been reported. The study was to evaluate its preventive effect on chylothorax and its influences on other complications and prognosis of esophageal carcinoma patients.
METHODS: Clinical data from 389 patients who underwent radical transthoracic esophagectomy from Jun. 1991 to Jun.

STUDY OBJECTIVE: The dilatation of the thoracic duct was previously demonstrated in liver cirrhosis by lymphangiography, endoscopic ultrasound, and at autopsy. The evaluation of the morphologic change of the thoracic duct may be important in assessing the altered lymphodynamics in liver cirrhosis. The objectives of this study were to determine which combination of posture and breathing phase during noncontrast-enhanced magnetic resonance lymphography (MRL) provided the clearest images, and to evaluate the morphologic changes in the thoracic duct in healthy volunteers and patients with liver di

OBJECTIVE: Postoperative chylothorax remains an uncommon but potentially life-threatening complication of esophagectomy for cancer, and the ideal management is still controversial. The aim of the study was to compare the outcomes of patients treated nonoperatively with those of patients promptly undergoing reoperation.
METHODS: From 1980 to 1998, 1787 esophagectomies for esophageal or cardia cancer were performed, and 19 (1.1%) patients had postoperative chylothorax. We analyzed type of operation, surgical approach, delay of diagnosis of chylothorax, daily chest tube output, type of managemen

In 25 dogs, a short segment of the left subclavian vein, adjacent to the connection of the thoracic duct, was dissected free with its jugular branch and ligated. An end-to-side anastomosis, using interrupted sutures was performed in two layers, between the side of the cervical esophagus and the proximal part of the jugular vein. This vein, acting as a reservoir, remains connected to the thoracic duct by the isolated segment of the subclavian vein. The original aspect, of our technique is the use of a catheter placed into a branch of the vein near the site of the anastomosis and through which a

Postoperative chylothorax is a rare complication after thoracic surgery, characterized by the pathological accumulation of chyle within the pleural space. The management of chylothorax remains controversial, particularly in cases that do not respond to conservative treatment strategies. This case series aims to evaluate the application of magnetic resonance thoracic ductography (MRTD) in guiding precise surgical treatment for refractory postoperative chylothorax. We retrospectively analyzed four patients with postoperative chylothorax who did not respond to conservative therapy. All patients u

INTRODUCTION: Given the high variability and fragility of the thoracic duct, good knowledge of its anatomy is essential for its repair or to prevent iatrogenic postoperative chylothorax. The objective of this study was to define a site where the thoracic duct is consistently found for its ligation. The second objective was to define an anatomically safe surgical pathway to prevent iatrogenic chylothorax in surgery for aortic arch anomalies with vascular ring, through better knowledge of the anatomical relationships of the thoracic duct.
METHODS: Seventy adult formalin-fixed cadavers were diss

Chylous peritonitis, which is caused by the accumulation of chyle in the abdominal cavity, is a rare cause of acute surgical abdomen. It has a milky appearance due to its high triglyceride content. It can originate from different mechanisms, including congenital malformations and injury or obstruction of the lymphatic ducts. Due to its low incidence, there is no clear diagnostic or therapeutic approach, adding further complexity to the condition. We present a clinical case of a 37-year-old female patient with a history of blunt cervical trauma that progressed to abdominal distension and perito

Background: The thoracic duct (TD) and the cisterna chyli (CC) exhibit a high degree of variability in their topographical and morphometric properties. Materials and Methods: PubMed, Scopus, Embase, Web of Science, Cochrane Library, and Google Scholar were searched to identify all studies that included information regarding the morphometric and topographical characteristics of the TD and CC. Results: The most frequent location of the TD termination was the left venous angle, with a pooled prevalence of 45.29% (95% CI: 25.51-65.81%). Moreover, the TD terminated most commonly as a single vessel

This study aims to describe a case series of patients who underwent thoracic duct embolization (TDE) to treat traumatic iatrogenic chylothorax (TIC). Three patients were included: Case #1, a 49-year-old woman with follicular lymphoma developed a TIC following video-assisted thoracoscopic surgery to resect a solid right paravertebral mass and was treated with TDE using microcoils and N-butyl cyanoacrylate (NBCA) glue. Case #2, a 68-year-old man with cardiac amyloidosis developed a TIC following heart transplantation and was treated with TDE using microcoils and ethylene vinyl alcohol copolymer.

PURPOSE: The contribution of thoracic duct obstruction to lymphatic flow disorders has not been well-characterized. We describe imaging findings, interventions, and outcomes in patients with suspected duct obstruction by imaging or a lympho-venous pressure gradient (LVPG).
MATERIALS AND METHODS: Clinical, imaging, and interventional data, including the LVPG, of patients with flow disorders and imaging features of duct obstruction who underwent lymphatic intervention were retrospectively reviewed, collated, and analyzed with descriptive statistics.
RESULTS: Eleven patients were found to have

Thoracic duct cysts (TDC) within the retropharyngeal space and mediastinum are exceedingly rare lesions, with the majority related to trauma or neoplasm. We describe a case of an otherwise healthy 8-month-old boy who presented with severe respiratory distress, which was found to be caused by a large, spontaneous TDC occupying most the retropharyngeal and mediastinal space. To our knowledge, this is the youngest patient to date presenting with TDC. Ultimately, his TDC was completely resolved with sclerotherapy, however the patient's age and size presented unique challenges to his medical manage

BACKGROUND: We report a 16-year-old patient with a massive left-sided chylothorax after chemotherapy due to mixed germinal tumor of the testis with massive metastases located in the retroperitoneal space and posterior mediastinum. Chemotherapy resolved the metastases in the mediastinum but evoked a huge pleural effusion in the left pleural cavity, requiring surgical intervention.Left-sided access was used. The 5-mm camera and 3 5-mm working ports were inserted. The parietal pleura was incised and the esophagus located and protected. Behind the esophagus, the thoracic duct and concomitant tissu

A 72-year-old woman visited a clinic for anorexia and general fatigue but no particular abnormality was detected by routine examination at that time. Thereafter, she experienced gradually increasing dyspnea and chest X ray showed right pleural effusion. Idiopathic chylothorax was diagnosed due to the milky effusion with a high concentration of triglyceride (2618 mg/dl) and no apparent causative disease. Irrespective of treatments including dietary restriction, drainage of the pleural space, and somatostatin injections, her effusion did not reduce. The leakage of lymph fluid from the right post

A case of bilateral thoracic ducts with coexistent persistent left superior vena cava (SVC) was identified in a 77-year-old Japanese female cadaver during dissection in a gross anatomy course. The persistent left SVC began at the lower surface of the left brachiocephalic vein, descended in front of the aortic arch, and drained into the right atrium through the coronary sinus. The right SVC was normal both in size and in position. The azygos vein, receiving the hemiazygos vein, opened into the right SVC. The accessory hemiazygos vein and the left superior intercostal vein united to form a commo

BACKGROUND: Compared to other hollow viscus organs, the oesophagus has a unique anatomy in which lymphatic channels are abundantly present in the mucosa and submucosa. It has been hypothesized that tumour cells can directly disseminate from these superficial layers into the thoracic duct without passing juxta-tumoral lymph nodes. We investigated whether tumour cells of an oesophageal carcinoma could be detected in the thoracic duct during operative manipulation.
METHODS: In patients with an adenocarcinoma of the oesophagus and/or gastro-oesophageal junction, undergoing a transthoracic resecti

The degree of lymphogemy in persons who died of different causes (including drowning) was studied by histologic analysis of the thoracic lymphatic ducts. It was found that quantitative estimation of the presented feature makes it possible to estimate the specific type of drowning which depends on the level of venous hypertension in the system of superior vena cava. This phenomena is strongly marked in spastic and mixed types of drowning and is absent in aspiration and reflex types.
