Aortic dilatation after endovascular repair of blunt traumatic thoracic aortic injuries.

OBJECTIVE: Endovascular repair of blunt traumatic thoracic aortic injuries (BTAI) has become routine at many trauma centers despite concerns regarding durability and aortic dilatation in these predominantly young patients. These concerns prompted this examination of thoracic aortic expansion after endovascular repair of a BTAI. METHODS: The immediate postoperative and most recent computed tomography (CT) scans of patients who had undergone urgent endovascular repair of a BTAI and had at least 1 year of follow-up were reviewed. Diameter measurements were made at four predetermined sites: immed

Injuries significantly associated with thoracic spine fractures: a case-control study.

OBJECTIVE: To determine injuries significantly associated with traumatic thoracic spine (T-spine) fractures METHODS: This was a case-control study undertaken in an adult trauma centre. Cases were patients admitted with a traumatic T-spine fracture between January 1999 and August 2007. Each case had two controls matched for sex, age (+/-5 years) and injury severity classification (major/minor). Data were collected from patient medical records and the trauma service database. Multivariate logistic regression was used to determine injuries significantly associated with T-spine fracture. RESULTS

[The role of VATS in the treatment of blunt thoracic injuries].

AIMS: Video assisted thoracoscopy (VATS) is a more and more frequently used method in the diagnosis and treatment of blunt thoracic trauma. In some cases it has diagnostic and in others therapeutic effect. The authors analyze the role of VATS in the diagnosis and the treatment of 83 patients treated with haemothorax. PATIENTS AND METHODS: There were 83 patients treated (60 male, 23 female, mean age of 54.4 years) with haemothorax caused by blunt thoracic trauma. Chest tube drainage was used in 31 (37.3%) cases. Urgent thoracotomy was performed in only two (2.4%) cases. Elective, planned VATS

The midterm results of stent graft treatment of thoracic aortic injuries.

INTRODUCTION AND OBJECTIVES: Several publications document the technical feasibility of stent graft repair of aortic transection. We report our mid-term results of endovascular repair of thoracic aortic transections using covered stent grafts and compare this to a cohort undergoing open repair during the same time period to demonstrate the shift in practice pattern at our institution. MATERIALS AND METHODS: A retrospective review of patients who sustained blunt thoracic transection was undertaken. Medical records were examined to identify the clinical outcome of the procedure, and follow-up C

[Primary management of fresh injuries of the thoracic and lumbar vertebrae].

Results of conservative treatment of vertebral body fractures and of repositioning and internal fixation, are compared with those obtained by dorsoventral repositioning and fusion. The results achieved with patients treated by dorsoventral fusion were statistically significantly better than those obtained with the other two treatment methods. Consequently, we modified the treatment concept of thoracic and lumbar vertebral body fractures. Every vertebral disk showing irreversible damage is now resected and the mobility segment involved is fused. The fracture is treated conservatively only in ca

Thoracic and abdominal injuries in skiers: the role of air evacuation.

The increasing popularity of skiing as a recreational sport has resulted in a greater number of major thoracic and abdominal injuries. These injuries, unlike the more common orthopedic injuries, are often life threatening. This 8-year study reviews 44 thoracic and abdominal ski injuries managed at an urban trauma center since the inception of a helicopter air evacuation program with in-flight resuscitation capabilities. Twenty-five per cent of the injured had signs of hemorrhagic shock, and nearly 60% required a major operation. The injuries were caused by high-speed collisions with stationary

Pediatric blunt thoracic aortic injuries: Understanding the role of patient size and utilization of thoracic endovascular aortic repair.

BACKGROUND: Thoracic aortic trauma management includes nonoperative observation, open repair, or thoracic endovascular aortic repair (TEVAR). While TEVAR is frequently used in adults, there is no standardized treatment for pediatric thoracic aortic trauma despite many pediatric patients with adult-equivalent height and weight. We aim to better understand the management of blunt pediatric thoracic aortic trauma by evaluating patient size and interventions as well as the utilization of TEVAR in this population. METHODS: This is a retrospective review of the American College of Surgeons Trauma Q

Thoracic hyperextension injury with complete "bony disruption" of the thoracic cage: Case report of a potentially life-threatening injury.

BACKGROUND: Severe chest wall injuries are potentially life-threatening injuries which require a standardized multidisciplinary management strategy for prevention of posttraumatic complications and adverse outcome. CASE PRESENTATION: We report the successful management of a 55-year old man who sustained a complete "bony disruption" of the thoracic cage secondary to an "all-terrain vehicle" roll-over accident. The injury pattern consisted of a bilateral "flail chest" with serial segmental rib fractures, bilateral hemo-pneumothoraces and pulmonary contusions, bilateral midshaft clavicle fractur

Nerve injury: injury to the recurrent laryngeal, phrenic, vagus, long thoracic, and sympathetic nerves during thoracic surgery.

Nerve injury occurs in about 1% of patients undergoing thoracic surgery. Most commonly, it is the result of surgical trauma specific to the area of operation. Alternatively, it may be due to retraction on adjacent structures during the exposure of complex surgical procedures. This article reviews the most common types of nerve injury and discusses the pathophysiology, etiology, diagnosis, and treatment of each.

A meta-analysis of the incidence of intra-abdominal injuries associated with thoracic or lumbar flexion-distraction injuries.

BACKGROUND: Intra-abdominal injuries (IAIs) are often associated with thoracic or lumbar flexion distraction injuries (TLFDIs) or Chance fractures. The incidence ranges from 10 to 50 % in previous literature. AIM: To synthesize data about the incidence of IAIs associated with TLFDIs. METHODS: We searched PubMed, WOS, and Cochrane databases for all studies reporting the incidence of IAIs associated with TL FDIs. The primary outcome was the overall pooled incidence of IAIs, surgical intervention, and specific organ injuries. A subgroup analysis was done for studies that included adults, pediat

The anatomic feasibility of thoracic branched endoprosthesis in the treatment of blunt thoracic aortic injury.

INTRODUCTION: Blunt thoracic aortic injury (BTAI) is one of the leading causes of death among trauma patients who sustain high impact thoracic trauma with rapid deceleration. Thoracic endovascular aortic repair (TEVAR) is indicated in high grade injuries and requires a management strategy for the left subclavian artery (LSA). Gore TAG thoracic branch endoprosthesis (TBE) is a newly approved TEVAR device for to maintain LSA patency utilizing a side branch with reported use in acute indications. The anatomic suitability of this device for a population of BTAI patients and optimal inventory for o

Single retrograde thoracic branch endoprosthesis versus standard endovascular repair with subclavian coverage for treatment of blunt thoracic aortic injuries.

OBJECTIVE: The Gore TAG thoracic branch endoprosthesis (TBE) allows zone 2 thoracic endovascular aortic repair (TEVAR), maintaining left subclavian artery (LSA) patency via side branch. We compared TBE for blunt thoracic aortic injuries (BTAIs) with TEVAR with subclavian artery coverage when seal into zone 2 was required. METHODS: We retrospectively identified patients with repair for BTAI between 2011 and 2024. Patient characteristics, procedural details, and outcomes are reported and compared between cohorts. RESULTS: Between 2011 and 2024, 174 patients had endovascular repair of a BTAI at

Minimally invasive management of penetrating thoracic and abdominal injuries: a case series.

BACKGROUND: Penetrating thoracic and abdominal trauma presents a diagnostic and therapeutic challenge in the emergency department (ED). Traditionally managed with open laparotomy or thoracotomy, advances in minimally invasive surgery (MIS) have enabled safe, effective, and rapid diagnosis and intervention. We present a case series of four patients with penetrating gastric, hepatic, and pulmonary injuries successfully managed using MIS, highlighting its role in emergency care. CASE PRESENTATIONS: Four hemodynamically stable patients presented to the ED with penetrating abdominal or thoracic tr

Management of Moderate Blunt Thoracic Aortic Injuries in Patients with Intracranial Hemorrhage.

BACKGROUND: Blunt thoracic aortic injuries (BTAIs) are the second most common cause of death due to blunt-force trauma in the United States. Patients with minimal injuries do not typically require surgical repair, whereas patients with severe injuries are treated emergently. Moderate aortic injuries are repaired in a semielective fashion, but the optimal management of patients with moderate BTAI with associated intracranial hemorrhage (ICH) is unknown. We sought to analyze the management and outcomes of patients presenting with concomitant moderate BTAI and ICH. METHODS: Consecutive patients

Surgical timing for cervical and upper thoracic injuries in patients with polytrauma.

OBJECTIVE Few studies have investigated the advantages of early spinal stabilization in the patient with polytrauma in terms of reduction of morbidity and mortality. Previous analyses have shown that early stabilization may reduce ICU stay, with no effect on complication rates. METHODS The authors prospectively observed 340 polytrauma patients with an Injury Severity Score (ISS) of greater than 16 at a single Level 1 trauma center who were treated in accordance with a protocol termed "early appropriate care," which emphasizes operative treatment of various fractures within 36 hours of injury.
