Assessment of Intercostal Nerve Block Analgesia for Thoracic Surgery: A Systematic Review and Meta-analysis.

IMPORTANCE: The use of intercostal nerve block (ICNB) analgesia with local anesthesia is common in thoracic surgery. However, the benefits and safety of ICNB among adult patients undergoing surgery is unknown. OBJECTIVE: To evaluate the analgesic benefits and safety of ICNB among adults undergoing thoracic surgery. DATA SOURCES: A systematic search was performed in Ovid MEDLINE, Ovid Embase, Scopus, and the Cochrane Library databases using terms for ICNB and thoracic surgery (including thoracic surgery, thoracoscopy, thoracotomy, nerve block, intercostal nerves). The search and results were

Computed Tomography-Guided Percutaneous Coblation of the Thoracic Nerve Root for Treatment of Postherpetic Neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is one of the most intractable pain disorders and often does not respond to medication, physical, and interventional procedures. Coblation technology has been demonstrated to have potential for neuralgia, but there are rare reports of the efficacy and security of coblation for PHN. The thoracic segment is the most common predilection part of PHN, so we conducted this long-term study to investigate the results of coblation for the treatment of thoracic PHN. OBJECTIVES: The aim of this study was to determine the efficacy and security of computed tomograp

Shrug radiographs for the diagnosis of long thoracic nerve palsy in traumatic brachial plexus injury.

BACKGROUND: Preoperative diagnosis of long thoracic nerve (LTN) palsy is important for shoulder reconstruction after a traumatic brachial plexus injury (BPI). In the present study, we developed an objective diagnostic method for LTN palsy for patients with traumatic BPI. METHODS: This is a retrospective review of 56 patients with traumatic BPI who had been receiving treatment at a single institution for over 8 years. The patients were divided into 2 groups: an LTN palsy group (n = 30) and a no palsy control group (n = 26). The LTN palsy group had 21 different palsy types with 4 and 5 C5-7 and

Bilateral Thoracic Paravertebral Nerve Blocks for Open Gastrostomy in Patients with Amyotrophic Lateral Sclerosis.

Most amyotrophic lateral sclerosis (ALS) patients require enteral feeding, thus necessitating placement of a gastrostomy tube (G-tube) for worsening dysphagia. This case report discusses the use of bilateral thoracic paravertebral nerve blocks (TPVB) in two patients with ALS to facilitate an open G-tube placement to avoid ventilatory assistance.

Peripheral Nerve Injury in Thoracic Surgery Detected by Automated Somatosensory Evoked Potential Monitoring.

Study Objective. Our objectives were to estimate the incidence of symptoms of peripheral nerve injury (sPNI) in thoracic surgical patients undergoing video-assisted thoracic surgery or open thoracotomy and to determine whether intraoperative somatosensory evoked potentials (SSEPs) waveform changes correlate with postoperative peripheral neuropathic symptoms. Methods. We conducted a prospective observational study in the operating room of a tertiary hospital. We measured SSEPs intraoperatively and assessed patients for sPNI postoperatively. Results. Forty-four patients consented. Six were exclu

Spontaneous cerebrospinal fluid leak from an anomalous thoracic nerve root: case report.

The authors report the case of a 28-year-old woman with a spontaneous cerebrospinal fluid leak from the sleeve of a redundant thoracic nerve root. She presented with postural headaches and orthostatic symptoms indicative of intracranial hypotension. CT myelography revealed that the lesion was located at the T-11 nerve root. After failure of conservative management, including blood patches and thrombin glue injections, the patient was successfully treated with surgical decompression and ligation of the duplicate nerve, resulting in full resolution of her orthostatic symptoms.

Dermal Sinus Tract of the Thoracic Spine Presenting with Intramedullary Abscess and Cranial Nerve Deficits.

Congenital dermal sinus tract of the spine is an unusual developmental defect which represents a failure of the surface ectoderm and dermal elements to separate from the neuroectoderm. A 15-month-old female presented with high fever, severe right hemiparesis, difficulty breathing and cranial nerve deficits. Magnetic resonance imaging (MRI) of the spine revealed a congenital dermal sinus tract at the Th6 level, an intramedullary collection extending up to the brainstem and a probable intramedullary cystic lesion. The child was operated acutely with ligation of the sinus tract, drainage of the a

Anatomical basis of the risk of injury to the right laryngeal recurrent nerve during thoracic surgery.

PURPOSE: Despite the intrathoracic part being short, the right laryngeal recurrent nerve is often injured during thoracic surgery. The aim of this cadaver study was to understand the mechanisms of right laryngeal recurrent nerve injuries during thoracic surgery and to describe anatomical landmarks for its preservation. METHODS: Dissections were performed on 10 fresh human cadavers. A right anterolateral thoracic wall segment was removed, preserving the first rib. Dissections were carried out to identify the following structures: first rib, esophagus, trachea, right main bronchus, right brachi

Robotic intercostal nerve graft for reversal of thoracic sympathectomy: a large animal feasibility model.

BACKGROUND: A subset of patients who undergo video-assisted thoracoscopic sympathectomy for hyperhydrosis develop post-procedure compensatory sweating that is perceived as more debilitating than their initial complaints. We propose a novel treatment to reverse sympathectomy by implantation of an intercostal nerve graft using the da Vinci robot. METHODS: A robotic swine model was established using single-lung ventilation and four ports. The pleura was incised and a representative segment of sympathetic chain was transected. A nearby intercostal nerve was harvested and sutured to the sympatheti

Structural transformations in sympathetic ganglia and the thoracic part of the vagus nerve in conditions of gravitational overloading.

The aim of the present work was to undertake a complex of studies of structural transformations in the anterior thoracic ganglia of the sympathetic trunk and the thoracic part of the vagus nerve after acute and chronic gravitational overloading (GOL). Studies were performed on 28 white mongrel male rats aged 8-21 weeks. Animals of series I (acute GOL) were rotated in a centrifuge on one day (three rotation sessions with two 20-min breaks, giving a total rotation time of 31 min). Animals of series II (chronic GOL) were rotated in an alternating two-week regime for 13 weeks (total rotation time

Dermatomal and mixed nerve somatosensory evoked potentials in the diagnosis of neurogenic thoracic outlet syndrome.

To evaluate the diagnostic utility of dermatomal and mixed nerve somatosensory evoked potentials (SEPs) in patients with thoracic outlet syndrome (TOS) and to compare their value with routine electrodiagnostic methods, we studied a group of 44 patients with neurogenic TOS and 30 healthy controls. In addition to bilateral median and ulnar SEPs, evoked potentials were recorded after stimulation of C6 and C8 dermatomes from the first and fifth digits, respectively. The patients were classified into 3 groups according to the nature of their clinical condition. The abnormality rate for both ulnar a

Galactorrhoea and amenorrhoea due to an intradural neurinoma originating from a thoracic intercostal nerve radicle.

A 42-year-old woman had galactorrhoea and amenorrhoea. Four years previously she complained of sensory disturbance of her legs, with galt disturbance. Serum PRL level was 1408 mU/l. Magnetic resonance imaging showed a spindle-shaped mass in the spinal canal at the level of the ninth thoracic vertebra. The tumour was removed and histologically was found to be a neurinoma originating from the right ninth intercostal nerve radicle. Soon after the operation, galactorrhoea, amenorrhoea and neurological symptoms disappeared. Prolactin levels normalized at 346 mU/l. These findings indicate that hyper

A Comparison of Intercostal Nerve Block and Thoracic Epidural Anesthesia in Patients Undergoing Video-Assisted Thoracic Surgery: A Propensity Score-Matched Retrospective Study.

BACKGROUND: Intercostal nerve block (ICNB) plus intravenous (IV) patient-controlled analgesia (PCA) could be an alternative method of perioperative pain management in patients undergoing video-assisted thoracic surgery (VATS). However, the efficacy of this strategy has not been established. METHODS: A retrospective observational study was conducted at an acute care hospital in Japan. Among patients who underwent VATS under general anesthesia from January 1, 2012, to December 31, 2022, we included those who received ICNB or thoracic epidural anesthesia (TEA). The ICNB group had postoperative I

The long thoracic nerve as a donor for elbow extension reconstruction in traumatic adult brachial plexus injuries.

BACKGROUND AND AIMS: Elbow extension reconstruction in traumatic adult complete or extended partial brachial plexus injuries (BPIs) is challenging owing to the limited availability of reliable uninjured donor nerves. The intraplexal long thoracic nerve (LTn), offers an underused donor option is selected circumstances. Its segmental innervation of serratus anterior allows selective distal harvesting with minimal functional loss. However, its use has been described only in small, heterogeneous series with limited outcome data. We aimed to evaluate the clinical utility and outcomes of LTn transfe

Nerve entrapment complicating neurogenic thoracic outlet syndrome surgery: A 10-year retrospective study.

Neurogenic thoracic outlet syndrome results from compression of the brachial plexus. The symptoms are mainly pain, upper-limb weakness and paresthesia. Management always starts with a rehabilitation program, but failure of rehabilitation may necessitate surgery. In practice, we observed that several patients developed secondary distal nerve entrapment in the months following surgery, with no preoperative compression. We aimed to assess the occurrence of distal nerve entrapment after surgery for neurogenic thoracic outlet syndrome in a retrospective cohort study. Seventy-four patients were incl
