BACKGROUND: A survey amongst the European Society of Thoracic Surgeons (ESTS) members has been performed to investigate the currents trends, rates of adoption as well as potential for future expansion of non-intubated thoracic surgery (NITS) performed under spontaneous ventilation.
METHODS: A 14-question-based questionnaire has been e-mailed to ESTS members. To facilitate the completion of the questionnaire, questions entailed either quantitative or multiple-choice answers. Investigated issues included previous experience with NITS and number of procedures performed, preferred types of anesth

AIM: This meta-analysis aimed to evaluate and compare the outcomes of video-assisted thoracic surgery (VATS) and open thoracic surgery (OT) in the management of pulmonary echinococcosis.
METHODS: We conducted a comprehensive search of PubMed, Embase, and Cochrane databases for studies comparing VATS and OT. Odds ratios (ORs) for binary outcomes and mean differences (MDs) for continuous variables were calculated with 95% confidence intervals (CIs) using the DerSimonian and Laird random-effects model. Heterogeneity was assessed using I2 statistics.
RESULTS: Seven studies involving 2292 patient

BACKGROUND: Pain management is an essential part of enhanced recovery after surgery, facilitating early return to mobility and improving respiratory mechanics. Acute pain management guidelines for patients undergoing video-assisted thoracoscopic surgery (VATS) procedures have been published recently. We assessed compliance of Canadian surgeons with those guidelines.
METHODS: A survey was distributed to all currently practicing Canadian thoracic surgeons through the Canadian Association of Thoracic Surgeons (CATS). The survey included questions about demographics, nerve block techniques, and o

The progressive advances in Minimally Invasive Surgical Procedures, are applied presently to the surgical treatment of intra-thoracic injuries in children by way of Video-Assisted Thoracic Surgery (VATS) techniques. We present the complete findings of two intra-thoracic masses, in an infant and in a school-aged child, with the use of the VATS technique. The operative technique is detailed in each case. We conclude that the lack of invasiveness and cost-efficiency of VATS the technique, constitute it as being the first choice in the treatment of intra-thoracic masses in children.

In the last 20 years, major changes have occurred in the diagnosis and management of thoracic oncologic pathology. Furthermore, thoracic surgery has undergone considerable changes since the inception of the Journal of Thoracic Oncology in 2006. Regarding staging of lung cancer, pleural mesothelioma, and thymic epithelial tumors, several revisions were implemented in the TNM classification. The recently published ninth edition represents a tremendous effort of the Staging and Prognostic Factors Group of the International Association for the Study of Lung Cancer to analyze worldwide survival dat

BACKGROUND: In April 2024, we aimed to reform the physicians' working style in our hospital, which is a core regional hospital where only two full-time thoracic surgeons work. We report three efforts to enhance productivity despite of limited number of staff in the department of thoracic surgery. Efforts:1)A new on-call system for thoracic surgery was introduced in April, 2024. To reduce the burden of on-call, we designated certain days when the on-call service is unavailable, where as the minimum level of medical care is ensured by transporting patients to other hospitals on those days. 2)The

PURPOSE OF REVIEW: Robotic-assisted thoracic surgery (RATS) has emerged as a transformative approach in thoracic surgery, enabling enhanced precision and minimally invasive treatment for lung, esophageal, and mediastinal resections. These advances introduce unique anesthetic challenges requiring tailored strategies to ensure patient safety and surgical success.
RECENT FINDINGS: Key challenges include intrathoracic carbon dioxide insufflation, prolonged one-lung ventilation, and complex positioning. Special considerations apply to high-risk groups such as patients with myasthenia gravis, obesi

Patients undergoing thoracic surgery experience particular challenges for acute pain management. Availability of standardized diagnostic criteria for identification of acute pain after thoracotomy and video assisted thoracic surgery (VATS) would provide a foundation for evidence-based management and facilitate future research. The Analgesic, Anesthetic, and Addiction Clinical Trial Translations, Innovations, Opportunities, and Networks (ACTTION) public-private partnership with the United States Food and Drug Administration, the American Pain Society (APS), and the American Academy of Pain Medi

Tuberculosis and inflammatory conditions are endemic in India and South-East Asia. They cause intense inflammatory reactions and adhesions, thus making surgical resection difficult. In 2009, we installed an intuitive da Vinci HDSi robot to perform our surgery as a part of a robotic thoracic surgery unit. We reviewed our practice to report the trials and tribulations of starting a robotic thoracic surgery program in an inflammatory and infective disease endemic third-world country. With the success of the multispecialty robotic surgery program, we were able to purchase a second robot with an op

Classical video-assisted thoracic surgery (VATS) approach to esophageal cancer uses four incisions. The rationale is to facilitate movement of the instruments and the esophagus and also suturing during placement of a purse-string suture for an intrathoracic anastomosis. Uniportal VATS (U-VATS) is challenge for surgeons, as you have to do an esophageal mobilization and anastomosis from a single 3-5 cm incision. The incision is placed either at the 5th or 6th intercostal space close to the posterior axillary line. Esophagus is mobilized en bloc with the subcarinal and periesophageal lymph nodes.

Pyoderma gangrenosum (PG) is a neutrophilic, ulcerative dermatosis that can develop at sites of cutaneous trauma, including surgical incisions, a phenomenon known as pathergy. The characteristic lesion is a painful, rapidly expanding ulceration with a violaceous undermined border.1 A biopsy taken from the expanding violaceous border shows predominantly neutrophilic dermal inflammation with neutrophilic abscess formation.

The etiology of PG appears to be variable among patients, as about a half of the reported cases are associated with systemic disease such as inflammatory bowel disea

Early pain and persistent parietal disorders remains a major unresolved problem in thoracic surgery. Thoracotomy and the use of multiple ports in most Video Assisted Thoracic Surgery (VATS) procedures are the major cause of this persistent pain. For the last decade, a few publications describing the use of either single incision VATS and cervical thoracic approaches have been reported without significant results in comparison with current used techniques. Intercostals compression during surgery and early after by intercostals chest tube placement, are probably the major cause of postoperative
