[Thumb replacement operation using metacarpal distraction osteotomy as a secondary intervention after thumb amputation].

After thumb amputation distal to the MCP-1 joint, good results can be achieved with lengthening of the first metacarpal bone by an external fixateur if a correct technique is applied. In all our patients the lengthened thumb was mechanically stable and sensitive, thus improving the patient's ability to use the hand to grasp. Another advantage of this procedure is the relative simplicity of the method. Furthermore, the original nerve and vascular supply can be preserved.

Radiological evaluation of ulnar thumb metacarpophalangeal ligament reconstruction for chronic thumb instability in cadavers.

Various techniques have been described to restore metacarpal stability in the thumb. Ligament reconstruction techniques that position the bone tunnels in a triangular configuration, with the apex proximally, are optimal for stabilizing the metacarpophalangeal (MCP) joint, while preserving the thumb's range of motion. Using an interference screw ensures this reconstruction is as strong as the native ligament. Our purpose was to evaluate a new technique designed to treat cases of chronic thumb instability. We created metacarpal instability in 10 fresh cadaver forearms by sectioning the ulnar col

The spastic thumb. Current concepts.

The spastic thumb is frequently positioned in the palm (thumb-in-palm deformity), impeding hand function and in severe cases affecting hygiene. It usually results from intrinsic muscle spasticity and weakness of thumb retropulsion and extension although other possible causes, as a spastic flexor pollicis longus, may be associated. Correction of the thumb deformity provides substantial functional and aesthetic improvement in affected individuals, including the functional and nonfunctional hand. The surgical strategy depends on a thorough physical examination and the goal must be set specificall

Pollicization and Pediatric Thumb Reconstruction.

Functional impairment, absence, or traumatic loss of the thumb is associated with considerable morbidity. A fully functioning thumb is estimated to account for 40% of hand function. An array of options exists for thumb reconstruction, and the intervention selected must be tailored to each individual patient. Pollicization is a powerful and elegant operation that can dramatically improve function for many patients. However, the surgeon and patient must be keenly aware that pollicization does not construct a "normal" thumb. Herein, we present a stepwise approach to treatment, including surgical

Congenital thumb differences- current concepts.

Anomalies of the thumb development are not uncommon and may be associated with a number of syndromes also. These anomalies range from total absence to duplication. Reconstructive surgery for the creation of an opposable thumb is the most rewarding aspect of Hand Surgery and also the most challenging. Classification systems have been modified for better description. A number of procedures have been introduced to improve the functionality of the hand in anomalies and age old concepts are undergoing a metamorphosis to further this. A brief description of the common conditions and their treatment

Extreme Thumb Losses: Reconstructive Strategies.

BACKGROUND: Historically, complex amputations of the thumb have been managed in two stages and often require additional soft-tissue and tendon transfer for successful outcomes. This article provides several novel strategies to address these problems in a single stage using existing muscles, immediate free tissue transfer, and toe transfer. METHODS: From a personal experience of 482 toe transfers, 24 cases were performed to reconstruct extreme thumb losses. All thumbs were reconstructed in one stage. In only one case, the thumb was reconstructed with a second toe transfer; the remainder had a

Thumb amputations from team roping.

BACKGROUND: Thumb injuries during team roping have elements of both avulsion and crush, resulting in a poor prognosis for replantation success. PURPOSE: To review 19 cases of thumb amputation from team roping at our institution since 1983. STUDY DESIGN: Retrospective cohort study. METHODS: Cases were included in the study only if a microvascular repair of artery and vein was needed for the thumb to survive. Vein grafts were used to span the damaged vessel segment. Of the 19 thumb amputation cases, 15 attempts were made to replant the thumb. In the remaining four cases, patients had bone sho

Thumb replantation after cardiac transplantation.

Generally, severe medical illnesses, including chronic immunosuppression and heavy steroid use, have been deemed relative contraindications for replantation surgery. We report the successful replantation of an amputated thumb in a patient who had previously undergone heart transplantation and was on therapeutic immunosuppression. Despite extensive soft-tissue damage and joint involvement, thumb function and sensibility returned to acceptable levels. This case demonstrates that digital bony union, tendon repair healing, and nerve regeneration are not significantly altered by the standard immuno

Conservative treatment of mallet thumb.

Four cases of mallet thumb were treated conservatively by splinting the interphalangeal joint of the thumb in extension using the Stack splint. Two patients had sharply cut the extensor tendon on the dorsum of the proximal phalanx and two had avulsion of the extensor tendon from the base of the distal phalanx. Eight weeks of continuous splinting was followed by 2 weeks of night splinting. Six months of follow-up revealed excellent range of motion in all four cases.

Thumb reconstruction by digital transposition.

In traumatic loss or congenital aplasia of the thumb, transposition of a digit on its neurovascular pedicle is the preferable method for thumb reconstruction. The index finger is the digit of choice, but any other finger or a portion of a finger can be used, especially if it is damaged. The operative techniques differ considerably, depending on the level of thumb loss. In total amputation or total aplasia all the parts of the new thumb have to be constructed with the structures of the transposed index finger. The skeletal readjustment consists of shortening in the metacarpal region, rotation o

Analysis of Causes for Congenital Ulnar Deviated Thumbs at the Distal Phalanx Level in 157 Thumbs.

PURPOSE: To investigate anatomic abnormalities causing a congenital ulnarly deviated thumb at the distal phalanx. METHODS: A total of 122 children with 157 congenital ulnarly deviated thumbs at the distal phalanges were reviewed, including those with isolated deformity or polydactyly. We analyzed the incidence and characteristics of the underlying anatomic abnormalities as well as the differential diagnoses. RESULTS: Three main causes of an ulnarly deviated thumb were observed. Abnormal hypertrophic epiphyses were found in 96 thumbs. An extra phalanx lying between the normal proximal and dis

The Development of Trigger Thumb in the Contralateral Thumb in Pediatric Patients Presenting Initially With Unilateral Involvement.

Background: Pediatric trigger thumb is a common condition that can occur bilaterally. There have been reports of a metachronous relationship between trigger thumbs developing in both extremities. Surgeons might consider delaying operative treatment of unilateral trigger thumb due to the concern that contralateral symptoms may develop later in childhood, requiring a second procedure and anesthetic event. Methods: We retrospectively reviewed patients diagnosed with pediatric trigger thumb from 2008 to 2016 at a large pediatric hospital. Data collected included age at presentation and onset, late

Conservative treatment for a ruptured extensor tendon on the dorsum of the proximal phalanges of the thumb (mallet thumb).

Of 35 patients with mallet thumb, 25 received conservative coil-splint immobilization treatment. Those who received treatment were able to extend the interphalangeal joints to at least 0 degrees with the exception of four patients. The number of patients who did not respond to the treatment was greater in those 30 years of age or older compared with those 29 years old or younger, but there was no statistically significant difference. Satisfactory extension was observed in 12 patients who received treatment within 2 weeks after sustaining injuries. Four of 10 patients who received treatment 2 w
