Genu valgum after proximal tibial fractures in children.

Eight children with fractures of the proximal tibial metaphysis developed a significant genu valgum on the injured side. A study of the radiographs showed that the deformity was primarily due to bowing of the tibia below the site of fracture. The deformity may be the result of plastic bowing of the tibia accentuated by weight bearing during healing.

[Sarmiento functional bracing of tibial fractures (author's transl)].

The functional bracing advocated by Sarmiento was used in the conservative treatment of 74 recent tibial fractures (6) adults, 13 children) and in 12 fractures with delayed union. The leg was immobilized in an Orthoplast below-the-knee functional brace with patellar tendon bearing. A flexible plastic insert fitted on the heel allowed the use of sock and shoe. Knee and ankle motion were possible and early weight-bearing was allowed. The results of this method of treatment in recent fractures was excellent since healing occured without delay and function restoration was rapid. In delayed union o

Tibial fracture associated with use of Arthrex self-tapping screws during tibial plateau leveling osteotomy in dogs and development of transcortical tibial fracture.

OBJECTIVE: Medical records for dogs treated with tibial plateau leveling osteotomy (TPLO) using Arthrex 3.5 mm self-tapping screws (STS) were evaluated to determine the incidence rate of transcortical tibial fractures (TCTF) and development of complications. STUDY DESIGN: Retrospective case report. STUDY POPULATION: A total of 78 dogs. METHODS: Dogs that underwent a TPLO performed from March 2022 to August 2023 were reviewed. Postoperative TPLO radiographs were examined for evidence of a TCTF with Arthrex 3.5 mm STS. Records were reviewed for complications up to 14 weeks after surgery. RES

Single-Stage Fixation of Ipsilateral Tibial Plateau and Tibial Plafond Fractures: A Case Report.

BACKGROUND: Simultaneous ipsilateral fractures of the proximal and distal tibial articular ends are uncommon. Although this pattern has sometimes been described as a 'floating tibia', the term is not standardised. Management is dictated by fracture morphology, soft-tissue status, patient physiology and the ability to obtain stable articular reduction without additional soft-tissue injury. CASE PRESENTATION: A 45-year-old man sustained a closed Schatzker VI tibial plateau fracture, an ipsilateral partial-articular distal tibial plafond fracture documented as AO/OTA 43-B2 and a comminuted fibul

Long-term follow-up of fresh tibial osteochondral allografts for failed tibial plateau fractures.

BACKGROUND: The management of large posttraumatic full-thickness osteochondral defects in the proximal part of the tibia remains a challenge. The goal of treatment is a pain-free range of motion of the knee that provides enduring function and enables a young patient to participate in a wide range of activities. The use of fresh osteochondral allograft transplantation for tibial plateau lesions has been well documented. The purpose of the present study was to assess the survivorship and the long-term functional outcome following fresh osteochondral transplantation for unipolar posttraumatic tib

[Osteosynthesis for tibial plateau fractures in geriatric patients].

The surgical management of tibial plateau fractures in geriatric patients presents relevant challenges compared with younger patient populations. In addition to fracture-specific aspects, these particularly include patient-related factors, such as osteoporosis, multimorbidity and cognitive impairment, all of which can influence the interpretation of indications, operative strategy and postoperative management. This review article provides an overview of the key considerations for osteosynthesis of geriatric tibial plateau fractures and summarizes the relevant influencing factors that should be

Medical and Legal Risks in Tibial Plateau Fractures.

INTRODUCTION: Tibial plateau fractures, which comprise about 1% of all fractures, can be challenging to diagnose in the emergency department setting. Missed and delayed diagnoses can result in poor outcomes for patients and legal risks for clinicians, necessitating a high level of vigilance. CASE SERIES: In this article we review three malpractice cases related to tibial plateau fractures. Key issues included missed or delayed diagnosis, mismanagement of associated complications, inadequate discharge instructions, and lack of documentation. CONCLUSION: Tibial plateau fractures can be challen

Evaluation and Management of Posterior Tibial Plateau Fractures.

Tibial plateau fractures are caused by high-energy or low-energy trauma and result in complex injuries that require careful management of both osseous injuries and associated soft tissues. The posterior aspect of the tibial plateau can be involved in a variety of fracture patterns, requiring systematic evaluation, imaging, and advanced surgical planning to address these complex injuries. Early classification systems failed to classify posterior plateau fractures; however, three-dimensional imaging and newer classification schemes, including the Quadrant System and 3D systems, have incorporated

Osteosynthesis by locking plate for proximal tibial fractures.

INTRODUCTION: Treatment of proximal tibial fractures is known to be difficult. We report our own experience of the treatment of these fractures and evaluate our results. The hypothesis was that the clinical and radiological results were good. MATERIAL AND METHOD: From January 2004 to October 2008, fourteen AO-type 41A2-3 and C1 fractures have been treated with a LCP locking plate (8 women and 6 men, average age 60.42). Plating was performed either with an open approach or a minimal invasive approach. Clinical and radiological follow-up was carried out looking for range of motion of the knee j

Concomitant Ankle Injuries Associated With Tibial Shaft Fractures.

BACKGROUND: Ankle injuries associated with tibial shaft fractures can cause postoperative ankle pain and stiffness even when satisfactory bony union has been achieved. Although several previous studies have described these injuries, they have not been clearly defined or classified in terms of ankle injury type or need for surgical fixation. METHODS: Seventy-one consecutive patients (mean ± SD age, 48.3 ± 16.7 years; 37 men and 34 women) with tibial shaft fractures who underwent computed tomography examination were included. Data were collected including age, sex, body mass index, fracture loc

Tibial tubercle avulsion fractures in adolescent basketball players.

Tibial tubercle avulsion fractures most commonly occur in adolescent boys and usually result from pushing off or landing while jumping. These fractures are relatively uncommon but can have a significant functional effect. The purpose of this study was to determine the long-term outcome with return to play in 8 adolescent basketball players with at least 4 years of postoperative reconditioning. Results with return to play at the preinjury level are favorable after treatment of acute tibial avulsion fractures in adolescent basketball players. Long-term outcome was excellent in all patients regar

Distal tibial fractures treated with hybrid external fixation.

Management of distal tibia fractures remains challenging. The purpose of this study was to evaluate whether hybrid external fixation, as definite treatment, was associated with satisfactory results and low rate of complications. Between November 1999 and October 2006, 48 patients (33 men and 15 women) with a mean age of 45.6 years and a median ISS of 14.3 were admitted to our department with a distal tibia fracture and treated with the use of a hybrid external fixator. Eight patients had an open fracture. Mean follow-up was 14 months (range, 9-36 months). In 40 patients, radiographic evidence

Acute compartment syndrome in tibial plateau fractures--beware!

Acute compartment syndrome can complicate fractures of the tibial plateau and must be recognized early so that timely fasciotomy can be performed. The presence of the open fasciotomy wound complicates management of the underlying proximal tibial fracture, which ideally needs early anatomic reconstruction of the articular surface and stable metaphyseal fixation. The purpose of this review is to highlight the importance of this problem, understand treatment strategies, and review outcomes.

Three-column fixation for complex tibial plateau fractures.

OBJECTIVES: 1) To introduce a computed tomography-based "three-column fixation" concept; and 2) to evaluate clinical outcomes (by using a column-specific fixation technique) for complex tibial plateau fractures (Schatzker classification Types V and VI). DESIGN: Prospective cohort study. SETTING: Level 1 trauma center. PATIENTS: Twenty-nine cases of complex tibial plateau fractures were included. Based on routine x-ray and computed tomography images, all the fractures were classified as a "three-column fracture," which means at least one separate fragment was found in lateral, medial, and po

[Tibial diaphysis fractures below a total knee prosthesis].

The fractures of the tibial diaphysis below a total knee prosthesis are a surgical challenge and intra medullary nailing seems impossible. For the SOFCOT 2005 symposium on peri prosthetic fractures we reviewed our files: six cases were found and analysed. These fractures are rare: 5 cases from the symposium (out of 96 periprosthetic fractures at the knee) are reported plus a more recent one. They were observed in elderly (over 70) women except two cases of rheumatoid arthritis. All the patients were osteoporotic either due to aging or to a long duration corticosteroid treatment. All happened a

Foot progression angle after distal tibial physeal fractures.

Twenty-three patients with Salter 1 or 2 fractures of the distal tibia had follow-up with footprints to assess rotational deformities. Fourteen had external rotation deformity >+2 SD of the normal mean of 5 degrees to 6 degrees. Eight of the 14 had the finding of physeal widening of 2 mm or more. The widening was most commonly wedge-shaped and based anteriorly. The mean foot progression angle of the fractured sides was externally rotated 14 degrees, and the mean foot progression angle of the uninjured sides was externally rotated 10 degrees; both exceeded the normal mean. The fractured side me
