Pediatric Bilateral Facial Paralysis: An Unusual Presentation of Lyme Disease.

Pediatric bilateral facial nerve paralysis (FNP) is a rare condition, representing less than 2% of all cases of FNP. The differential diagnosis of FNP is extensive (ranging from infectious, traumatic, neurologic, to idiopathic) and often can present as a diagnostic challenge. In contrast to unilateral presentation, bilateral FNP presents as a manifestation of serious systemic conditions, including meningitis (infectious and neoplastic), brain stem encephalitis, Guillain-Barre syndrome, sarcoidosis, Lyme disease, human immunodeficiency virus infection, leukemia, and vasculitis. In the evaluatio

Diaphragmatic paralysis due to Lyme disease.

Lyme disease is a tick-borne spirochaete infection which, in a proportion of patients, can lead to neuropathy. This article describes a case of diaphragmatic paralysis due to Lyme disease. A 39-yr-old male presented to the hospital because of an acute left facial palsy. Six weeks prior to admission he had developed a circular rash on his left flank during a camping holiday. He also complained of shortness of breath and arthralgia for 1 week. His chest radiograph demonstrated a raised right hemi-diaphragm. Diaphragmatic paralysis was confirmed by fluoroscopy (a positive sniff test). Serology re

Ixodes (Ixodes) scapularis (Acari:Ixodidae): redescription of all active stages, distribution, hosts, geographical variation, and medical and veterinary importance.

The blacklegged tick, Ixodes (Ixodes) scapularis Say, 1821, is redescribed, based on laboratory reared specimens originating in Bulloch County, Georgia. Information on distribution, host associations, morphological variation, and medical/veterinary importance is also presented. A great deal of recent work has focused on this species because it is the principal vector of the agent of Lyme disease (Borrelia burgdorferi Johnson, Schmidt, Hyde, Steigerwaldt & Brenner) in eastern North America. Its distribution appears to be expanding, and includes the state of Florida in the southeastern United St

[Tick-borne meningopolyneuritis (Garin-Bujadoux, Bannwarth)].

The meningopolyneuritis first described in 1922 by Garin and Bujadoux develops after a tick bite. At the site of the bite in some of the cases, erythema chronicum migrans develops first. Severe pains follow in this area which may spread to other parts of the body. Then peripheral paralysis appears with asymmetrical distribution. The motor cerebral nerves, especially the facial nerve, are also frequently affected. Mild sensitivity disorders occur with no systematic arrangement. There is a lymphocyte pleocytosis of 100-2600/3 mm3 and an increase in protein of varying intensity, meningeal symptom

The avermectins: A new family of antiparasitic agents.

The avermectins are macrocyclic lactones produced by fermentation of the soil micro-organism Streptomyces avermitilis. They show activity against a broad range of nematodes and arthropod parasites of domestic animals at dose rates of 300 microgram/kg or less. Unlike the macrolide or polyene antibiotics, they lack significant antibacterial or antifungal activity. By oral or parenteral administration, avermectins are active against gastrointestinal nematodes and lungworms, and important ectoparasites such as lice, mange mites, ticks and larval stages of flies. They show excellent activity agains

Paralytic strabismus as a manifestation of lyme borreliosis.

Lyme disease is a multi-system organ disorder caused by Borrelia burgdorferi. Although ocular manifestations have been reported, these remain a rare feature of the disease. This report shows a 49-years old patient that has been bitten by a tick and as consequence of which developed symptoms of the Lyme disease. In 1998 the patient was hospitalized in our Eye Clinic due to operating treatment of the paralytic strabismus (abductal nerve paralysis), as a rare feature of the Lyme disease. Postoperative squint angle was significantly reduced, but without any temporal movement. Diplopia was still pr

Dysphagia due to Lyme Disease: A Case Report.

Lyme disease is a multisystem infection that can affect the joints, heart, and nervous system when untreated. While it can present with cranial nerve palsy, dysphagia is rarely reported. This case highlights a rare instance of dysphagia in Lyme disease, typically known for neurological symptoms like facial nerve palsy. Despite the absence of erythema migrans or a documented tick bite, the patient's facial palsy, hearing loss, vocal cord paralysis, and dysphagia were attributed to Lyme disease. With the rising prevalence of Lyme disease, similar cases may increase, particularly in endemic regio

Powassan Virus in the United States: An AI-Integrative One Health Scoping Review of Ecological Drivers, Zoonotic Interactions, Symptom Profiles, and Public Health Implications.

BACKGROUND: Powassan virus (POWV) is the only tick-borne encephalitis-group flavivirus endemic to the United States (U.S.) and is an increasingly serious public health threat. Reported U.S. cases remain relatively few but have grown steadily-from fewer than two annually before 2005 to a record 60 in 2024-and neuroinvasive disease carries a 10-15% case fatality with lasting neurological sequelae in roughly half of the survivors. Examining POWV through a One Health lens, we conducted a scoping review of its ecological and zoonotic drivers, its interactions with co-circulating tick-borne pathogen

A Sensitive Nano Luciferase Immune Complex Assay System for Highly Sensitive and Specific Detection of Antibodies Against Tick-Borne Encephalitis Virus.

Tick-borne encephalitis virus (TBEV) can cause fever, headache, neurological disorders, and/or peripheral flaccid paralysis; therefore, it is a major threat to public health. A rapid, sensitive, and simple method for detecting anti-TBEV antibodies is needed urgently to determine infection and for vaccine evaluation. Here, a luciferase-based immunocomplex assay system (Luc-IC) was developed to detect TBEV antibodies. The system is based on a reporter Nano luciferase (NLuc) that is co-expressed as a fusion protein with viral envelope domain III (ED3) in COS7 cells. The cell supernatant was used

Severe Fatal ARDS Due to Untreated Human Granulocytic Anaplasmosis in a 67-Year-Old Man: A Case Report.

BACKGROUND Anaplasmosis, or human granulocytic anaplasmosis (HGA), is a tick-borne diseased caused by a gram-negative, intracellular bacterium, Anaplasma phagocytophilum. HGA usually presents with mild symptoms but can be more severe. This report describes a 67-year-old male resident of rural Pennsylvania, admitted to the hospital after a fall, who developed fatal acute respiratory distress syndrome (ARDS) associated with human granulocytic anaplasmosis (HGA) following transmission of Anaplasma phagocytophilum by a tick bite (Ixodes scapularis). Case Report: Bilateral diaphragmatic dysfunction due to Borrelia Burgdorferi.

SUMMARY: In this case report we describe a rare case of bilateral diaphragmatic dysfunction due to Lyme disease. CASE REPORT: A 62-years-old male presented to the hospital because of flu-like symptoms. During initial evaluation a bilateral diaphragmatic weakness with orthopnea and nocturnal hypoventilation was observed, without a known aetiology. Bilateral diaphragmatic paralysis was confirmed by fluoroscopy with a positive sniff test. The patient was referred to our centre for chronic non-invasive nocturnal ventilation (cNPPV). Subsequent investigations revealed evidence of anti- Borrelia se

Recurrent Facial Paralysis in a 14-Year-Old Due to Brazilian Lyme Disease: A Case Report.

Lyme disease is a prevalent infection in the northern hemisphere, affecting approximately 450,000 new cases annually in the United States and 65,000 in Europe. This illness is spread by the bite of ticks harboring Borrelia spirochetes and develops in three progressive phases. In the second phase, neurological complications are common, including cranial nerve involvement. Notably, facial nerve neuropathy occurs in about 10% of adults with neurological symptoms, with an even higher incidence among pediatric cases. In this context, a 14-year-old female patient presented with acute facial paralysi
