Antidepressants for patients with tinnitus.

BACKGROUND: This is an update of a Cochrane review first published in The Cochrane Library in Issue 4, 2006 and previously updated in 2009.Tinnitus is described as the perception of sound or noise in the absence of real acoustic stimulation. It has been compared with chronic pain, and may be associated with depression or depressive symptoms which can affect quality of life and the ability to work. Antidepressant drugs have been used to treat tinnitus in patients with and without depressive symptoms. OBJECTIVES: To assess the effectiveness of antidepressants in the treatment of tinnitus and to

Imaging in pulsatile tinnitus.

Tinnitus may be continuous or pulsatile. Vascular lesions are the most frequent radiologically demonstrable cause of pulsatile tinnitus. These include congenital vascular anomalies (which may be arterial or venous), vascular tumours, and a variety of acquired vasculopathies. The choice of imaging depends on the clinical findings. If a mass is present at otoscopy, thin-section computed tomography (CT) is indicated. In the otoscopically normal patient, there is a range of possible imaging approaches. However, combined CT angiography and venography is particularly useful.

[Therapy of tinnitus in children].

Tinnitus in childhood is quite common when children are directly asked about this syndrom. Children rarely spontaneously complain of tinnitus. Material consists of 67 children (6-18 year old) with tinnitus, treated in the Pediatric Otolaryngology Clinic and Outpatient. Between children there were 2 groups: I tinnitus connected with hearing loss--35 children, II tinnitus without hearing loss--32 children. Methods. Anamnesis, otolaryngologic, neurologic, psychologic examinations, panel of audiologic tests before and after treatment of betahistine. In 12 patients there were recognized conductive

Tinnitus: presence and future.

Tinnitus has many forms; it can be caused by sounds generated in the body (objective tinnitus) that reaches the ear through conduction in body tissue, but much more common is the tinnitus that occurs without any physical sound reaching the ear. Such tinnitus (subjective tinnitus) is a phantom sensation, where abnormal neural activity is generated in the ear, the auditory nerve, or the central nervous system. There are many forms of subjective tinnitus and it can occur with different severity. Subjective tinnitus often occurs in connection with hearing loss such as may occur after exposure to l

Treatment of tinnitus with acamprosate.

Acamprosate, a drug used to treat alcohol dependence, was first reported as a potential treatment for tinnitus in 2005. The drug may improve tinnitus by a dual mechanism of action, acting both as a glutamate antagonist and as a GABA agonist. It is suggested that its action may be both on the ear and the nervous system.

Diagnostic approach to tinnitus.

Tinnitus is a common disorder with many possible causes. Most cases of tinnitus are subjective, but occasionally the tinnitus can be heard by an examiner. Otologic problems, especially hearing loss, are the most common causes of subjective tinnitus. Common causes of conductive hearing loss include external ear infection, cerumen impaction, and middle ear effusion. Sensorineural hearing loss may be caused by exposure to excessive loud noise, presbycusis, ototoxic medications, or Meniere's disease. Unilateral hearing loss plus tinnitus should increase suspicion for acoustic neuroma. Subjective t

Otosclerosis and chronic tinnitus.

Chronic subjective tinnitus is a common feature of clinical otosclerosis. Analysis of the records of 1,014 consecutive cases of clinical otosclerosis, all confirmed by stapes surgery in South Australia between 1960 and 1972, gives a preoperative prevalence of this symptom of 65%. The association of tinnitus with various predictors is considered, and a statistical analysis is presented. Tinnitus has an association with gender (p < .0001), mean preoperative bone conduction (BC) level (p = .0012), mean air conduction (AC) level (p = .0192), and mean air-bone gap (p = .0075). The associations betw

[Tinnitus in systemic diseases].

Permanent or temporary degenerative changes in the internal ear causing tinnitus may occur with particular intensity in patients suffering from systemic disease (diabetes, hypertension, rheumatic diseases, kidney and thyroid gland diseases). Pathomechanisms of hearing impairment and the risk of tinnitus and its character in particular cases are discussed in the paper. The research was carried out on 1200 patients treated in the Laryngological Rehabilitation Centre in Poznań between 1.1.1998 and 04.2001 due to tinnitus. The analysis included the diagnosis of general health condition, general la

Objective tinnitus in children.

OBJECTIVE: To discuss the various symptoms and causes of objective tinnitus in children. STUDY DESIGN: Retrospective case review. PATIENTS: Five children who had audible signals emanating from their ears caused by audible spontaneous otoacoustic emissions, palatal myoclonus, arteriovenous malformation, and acoustic trauma. MAIN OUTCOME MEASURE: The tinnitus reported by the patients was linked to acoustic signals that could be measured objectively or heard by the examiner. RESULTS: Four of the five children had essentially normal hearing. The one child who demonstrated a hearing loss audiom

Neurophysiologic mechanisms of tinnitus.

Research over the past decade has provided new insights into the neural mechanisms likely to produce the false percepts of sound associated with tinnitus. These insights have emerged mainly as a result of electrophysiologic studies, examining changes in brain activity, and behavioral studies, examining changes in perception, in animals that have been treated with well-known tinnitus inducers such as salicylates, quinine, and intense sound. The available evidence, based on electrophysiologic studies, suggests that tinnitus is associated with disturbances in spontaneous neural activity in the au

Imaging of tinnitus: a review.

Tinnitus, a buzzing or ringing in the ear, may be pulsatile or continuous (nonpulsatile). The distinction, with a detailed clinical evaluation, determines the most appropriate imaging study. Pulsatile tinnitus suggests a vascular neoplasm, vascular anomaly, or vascular malformation. Most of the neoplasms are glomus tympanicum and glomus jugulare tumors. Vascular anomalies may cause pulsatile tinnitus, but the mechanism is unknown, and another (treatable) cause should be sought. Most neoplasms and anomalies are best seen on bone algorithm computed tomographic (CT) studies. Dural vascular malfor

Pediatric and Geriatric Tinnitus.

The subject of tinnitus in the population extremes-children and the elderly-is ignored by the literature, probably because children do not complain of tinnitus spontaneously, whereas it is only one challenge among other major health problems in the elderly. A short review of the literature on this subject is presented. Presbytinnitus, defined as tinnitus that accompanies the progressive hearing loss of presbycusis is classified as: type 1 (normal aging affecting the cochlea), and type II (preexistent sensorineural hearing loss accompanied by multiple systemic complaints, especially of sensory

[Treatment of tinnitus with gentamicin].

Tinnitus is a very serious problem in otolaryngology. The authors used for torpid cases the intratympanic administration of Gentamicin sulphate, administered twice a day. Nine patients with Ménière's disease were treated. The results were very favourable, deterioration of hearing did not occur and the tinnitus disappeared or improved markedly.

The dissociation of SOAEs and tinnitus.

Spontaneous oto-acoustic emissions (SOAEs) were detected in 10 of 29 subjects with tinnitus. Because measures of the pitch of tinnitus are highly variable, the relation between the frequency composition of the tinnitus and the frequencies of SOAEs can not be known precisely. This raises the question of whether an observed SOAE could be proven to be the physical basis of an audible tinnitus of unspecified composition. To examine this question, subjects with both SOAEs and tinnitus participated in two demonstrations. First, while the tinnitus was reportedly masked by a high-frequency tone, the S

Animal models of tinnitus.

There are few physiological data available on the origin and nature of tinnitus. It is not even known whether tinnitus associated with cochlear pathology is a manifestation of increased or decreased activity in the cochlear nerve. In previous investigations of cochlear pathology, the spontaneous neural activity has generally been found to be depressed. In the present experiments, an animal model has been established by the administration of sodium salicylate in doses producing blood concentrations that evoke tinnitus in humans. Under these conditions, changes occur in cochlear nerve-fibre thre
