INTRODUCTION: Issuing an organ donation card has become a goal for procurement units to solve the organ shortage. The number of issued cards is an index of the attitudes or even actions of communities toward brain-dead donations. In the present study, we have reported the characteristics of issuing organ donation cards in a single organ procurement unit in Iran.
MATERIALS AND METHODS: This retrospective study used an organ donation willingness database for 3 years after launching the unit. We used the registration data of the first 3 years of its activity from August 1, 2005, to July 31, 2008

BACKGROUND: Solid organ transplantation has evolved globally as an established and life-saving treatment for patients with end-stage organ failure.
METHODS: Since 2007, the Global Observatory on Donation and Transplantation (GODT), an initiative of the World Health Organization in collaboration with the Organización Nacional de Trasplantes (Spain), collects data and reports on the activity of World Health Organization member states in regard to solid organ donation, transplantation, and waitlisting. This ongoing effort provides insights into transplant activities in countries with different h

PURPOSE OF REVIEW: Normothermic regional perfusion (NRP) for the donation after circulatory death (DCD) increases the supply and function of organs. However, the procedures used have led to significant ethical controversies. Despite the numerous studies on the subject, moral issues are seldom analyzed comparatively. This article describes and identifies three ethical problems related to NRP, critiquing them with philosophical and ethical suggestions.
RECENT FINDINGS: Current literature suggests that using oxygenated blood instead of cold perfusion enhances the function of transplanted organs,

IMPORTANCE: Despite the unmet need for donor organs, organ use from donation after circulatory determination of death (DCD) donors has been limited by inferior transplant outcomes. Normothermic regional perfusion (NRP) improves recipient outcomes and organ utilization from DCD donors. There is variability in NRP policies and experience among US organ procurement organizations (OPOs).
OBJECTIVES: To determine OPO experience, identify operational inconsistencies, and explore needs related to NRP.
DESIGN, SETTING, AND PARTICIPANTS: This survey study included 55 OPOs in the US that had recovered

Figuring out what pushes individuals to become organ donors has become the holy grail of social scientists interested in transplantations. In this paper I concentrate on solidarity as a determinant of organ donation and examine it through the history of organ donation in Israel. By following the history of transplantation policies since 1968 and examining them in relation to different types of solidarities, this paper leads to a nuanced understanding of the ties between solidarity and health policy. Attempts to foster an all-encompassing consensus on the definition of brain death yielded the T

Hormonal replacement therapy to brain-dead potential organ donors remains controversial. A retrospective study was carried out of hormonal therapy on procurement of organs in 63 593 donors in whom information on thyroid hormone therapy (triiodothyronine or levothyroxine [T3 /T4 ]) was available. In 40 124 donors, T3 /T4 and all other hormonal therapy were recorded. The percentage of all organs procured, except livers, was greater when T3 /T4 had been administered. An independent beneficial effect of antidiuretic hormone (ADH) was also clear. Corticosteroids were less consistently beneficial (m

The number of organ donations in Germany still does not meet the actual demand of patients on waiting lists, although there has been an increase in the first quarter of 2026 compared to the previous year. An effective and streamlined design of the overall process facilitating to organ donation after brain death is therefore mandatory. In addition to medical aspects, the establishment of adequate structural units and teams as well as mechanisms for quality assurance play a crucial role in achieving a coherent process organization with the best possible outcome, i.e. a high number of transplanta

Donation after circulatory determination of death transplantation offers an effective and safe method of increasing the number of heart transplants performed. Donation after circulatory determination of death heart is ischemic at the time of recovery, and therefore, requires reperfusion. This may take place in the donor, known as thoracoabdominal normothermic regional perfusion (taNRP) or outside of the donor, known as direct procurement and preservation. There is a growing body of evidence that taNRP is associated with better utilization of thoracic and abdominal organs from the donor and wit

BACKGROUND: Outcomes after pediatric liver transplantation are generally excellent, but the limited avavailability of suitable, size-matched liver allografts remains a significant barrier. Machine perfusion technology has emerged as a promising approach to expand the donor pool, enabling the use of less ideal whole liver grafts, such as livers donated after circulatory death, and enhancing the execution of split liver transplantation.
METHODS: This review examines the application of machine perfusion in pediatric liver transplantation, focusing on two primary techniques: hypothermic oxygentae

The transplantation of organs from postmortem organ donors has been a lifesaving and quality-of-life-improving therapy for patients with irreversible organ failure for many years. In Germany, however, there has been an imbalance between the number of organs donated postmortem and the number of patients on the waiting list for years. The anesthesiological management of multiple organ harvesting (MOE) in postmortem organ donors is not an everyday challenge for various reasons: A lack of practical expertise due to the small number of MOE, even at university hospitals (usually < 20 per year), comp

Organ donation after brain death has been practiced in China since 2003 in the absence of brain death legislation. Similar to international standards, China's brain death diagnostic criteria include coma, absence of brainstem reflexes, and the lack of spontaneous respiration. The Chinese criteria require that the lack of spontaneous respiration must be verified with an apnea test by disconnecting the ventilator for 8 min to provoke spontaneous respiration. However, we have found publications in Chinese medical journals, in which the donors were declared to be brain dead, yet without an apnea t

In the Netherlands, the average quality of donor organs continues to decrease as a result of the increasing life expectancy, as well as higher incidence of obesity, diabetes mellitus, and other comorbidities in the general population. Storing donor organs on ice, the current standard practice, appears to be inadequate for preserving these high-risk donor livers. Consequently, an increasing number of donor organs is discarded for transplantation. Over the past years, a novel dynamic preservation technique using a machine perfusion device has been developed. Preservation by machine perfusion ena

Organ transplantation is a treatment of modern medicine and technology that saves the lives of hundreds and thousands of medically suitable end-stage organ failure patients. The first successful kidney transplantation from living-related donors in Bangladesh was successfully performed in 1982 and regularly from 1988. This was then followed by deceased cornea in 1984, and liver and bone marrow from living-related donors in 2010 and 2014 respectively. The Human Organ Transplantation Act was first passed by the parliament of Bangladesh in1999, allowing both brain death donation and living-related

Even with encouraging recipient outcomes, transplantation using donation after circulatory death (DCD) is still limited. A major barrier to this type of transplantation is the consequences of warm ischemia on graft survival; however, preservation techniques may reduce the consequences of cardiac arrest and provide better organ conservation. Furthermore, DCD in trauma patients could further expand organ donation. We present five cases in which organs were retrieved and transplanted successfully using normothermic regional perfusion (NRP) in trauma patients. Prompt critical care support and surg

BACKGROUND: More people who have personally consented to organ donation via first person authorization (FPA) registration before death become organ donors than those not personally consenting. The majority of registrations occur at state-specific department of motor vehicle (DMV) and licensing offices, where people register their vehicles and obtain driver's licenses.
METHODS: One organ procurement organization (OPO) ran 3 DMV offices and implemented an intervention: a donor-centric approach, including employee education, office decoration with donation materials, and customer experience impr

There is international variability in the determination of death. Death in donation after circulatory death (DCD) can be defined by the permanent cessation of brain circulation. Post-mortem interventions that restore brain perfusion should be prohibited as they invalidate the diagnosis of death. Retrieval teams should develop protocols that ensure the continued absence of brain perfusion during DCD organ recovery. In situ normothermic regional perfusion (NRP) or restarting the heart in the donor's body may interrupt the permanent cessation of brain perfusion because, theoretically, collateral

BACKGROUND: The press is an important medium and plays a significant role as an information source for people. Moreover, the daily press transmits opinion-forming contents. During the German "transplantation scandal" various articles were published in the German press focusing on organ donation, transplantation, allocation of organs and brain death determination. Selected important newspaper articles were analyzed using a scientific text analysis as it was assumed that the publications might have had an important influence on attitudes or mistrust of transplantation medicine.

In the recent past there has been considerable discussion about the legal and legislative aspects of transplantation surgery. The present legal codes with German reunification now require a uniform law for the country as a whole. In addition to the different legal models already presented, any "expanded" agreement must not only respect the right of self-determination but also the responsibility of the individual for his societal obligations. The German legislator is now required to enact a uniform law which is reasonable and also considers the necessities of modern transplantation medicine in

Inadequate supply of donor hearts is the greatest limitation to wider utilization of heart transplantation. We performed donor cardiectomy for other transplantation centers in three cases. The donor heart functioned well in all cases, and the recipients were all discharged. Donor cardiectomy for other transplantation centers allows utilization of hearts that might otherwise be lost for transplantation.
