Saving the despair: conservative management of an endo-perio lesion using hemisection.

Hemisection, a surgical procedure involving the removal of one root and its associated coronal portion from a multi-rooted tooth, is a complex treatment option that aims to salvage a severely compromised tooth while maintaining its functionality and overall oral health. A middle-aged female patient presented with pain and tooth mobility in the lower jaw for the past 2 months. Initially, root canal treatment was done to treat symptoms of symptomatic irreversible pulpitis and apical periodontitis. However, considering the poor prognosis and restorability of the tooth, surgical flap reflection wa

Neuropathic ankle joint in Charcot-Marie-Tooth disease after triple arthrodesis of the foot.

Charcot-Marie-Tooth disease results in pes cavus or equinocavovarus deformity. An accepted surgical correction is triple arthrodesis of the foot. In a retrospective study of 45 Charcot-Marie-Tooth patients, two cases developed neuropathic ankle joint after triple arthrodesis. This procedure prevents mobility of the tarsal joint and consequently increases stress on the ankle joint. If coupled with decreased joint sensation, it may predispose the ankle to neuroarthropathy. This paper emphasizes joint position sense as an important consideration before triple arthrodesis or any other surgical pro

Periodontal and Periapical Outcomes of Surgical Extrusion: A Prospective Clinical Volumetric Study.

INTRODUCTION: Outcome predictors for the restoration of endodontically treated teeth mainly include cavity wall integrity and the presence of a 1.5-2 mm ferrule. However, in some clinical situations, the aforementioned predictors cannot be achieved. Three different techniques have been proposed to obtain an adequate ferrule respecting the supracrestal tissue attachment: surgical crown lengthening, orthodontic forced eruption, and surgical extrusion. There are little published prospective data evaluating periodontal changes after surgical extrusion. Therefore, the aims of this study were to rep

Local delivery of recombinant osteoprotegerin enhances postorthodontic tooth stability.

Relapse after orthodontic tooth movement is a significant problem in orthodontics. The purpose of this study was to examine the efficacy of the osteoclast inhibitor osteoprotegerin-Fc (OPG-Fc) for inhibiting postorthodontic relapse. Rat maxillary molars were moved mesially and allowed to relapse for 24 days. Low-dose (1 mg/kg) or high-dose (5 mg/kg) OPG-Fc or saline was injected adjacent to the molars during relapse. Tooth movement, micro-CT, histologic bone quality, and serum OPG and TRAP-5b were measured. OPG-Fc injections significantly diminished postorthodontic relapse from 63% (0.78/1.20

Odontogenesis-Empowered Extracellular Vesicles Safeguard Donor-Recipient Stem Cell Interplay to Support Tooth Regeneration.

Harnessing the developmental events of mesenchymal condensation to direct postnatal dental stem cell aggregation represents a cutting-edge and promising approach to tooth regeneration. Tooth avulsion is among the most prevalent and serious dental injuries, and odontogenic aggregates assembled by stem cells from human exfoliated deciduous teeth (SHED) have proven effective in revitalizing avulsed teeth after replantation in the clinical trial. However, whether and how SHED aggregates (SA) communicate with recipient components and promote synergistic tissue regeneration to support replanted teet

Tooth root resorption: A review.

Tooth root resorption is multifactorial, leading to progressive destruction and eventual loss of tooth root dentin and cement. There are internal and external types of root resorption, each having its variety. The etiology and pathogenesis of tooth root resorption are poorly understood, and the most significant etiological factors are trauma, pulpal infection, tooth bleaching, and orthodontic treatment. Tooth root resorption is primarily asymptomatic; thus, it is revealed accidentally by radiographic examination. Progressive clinical manifestations are pain, tooth discoloration, tooth mobility

Localized prepubertal periodontitis: literature review and report of case.

This case describes a young, healthy, white female who demonstrated anterior alveolar bone loss along with premature loss of her primary incisors. The alveolar bone loss remains unexplained. The root surfaces of most of the primary anterior teeth exhibited one or more eroded areas devoid of cementum with some evidence in two teeth of cellular resorptive activity. These findings suggest that premature root resorption was occurring concurrently with unexplained extensive alveolar bone loss. The child will be examined periodically to determine whether this process of bone loss with subsequent too

Dental aspects of patients with cystic fibrosis: a preliminary clinical study.

The oral conditions of 42 patients being treated for cystic fibrosis were evaluated. The patients were grouped by age and, in some aspects, were compared with a small control group of their siblings. The patient group had a lowered incidence of plaque and less gingival disease than did the control group in which every person had some amount of plaque or gingival disease. Calculus formation was minimal. The reasons for the finding of minimal plaque in the patient group could be related to several factors, including the life-long use of various antibiotic agents, the chewing of digestive enzyme

Influence of a parafunctional oral habit on root fracture development after trauma to an immature tooth.

Root fractures in immature teeth are rare because the resilience of the alveolar bone is more favorable to the occurrence of luxation. This article reports a case of traumatic injury in an immature permanent tooth that progressed to root fracture, having a parafunctional oral habit as the possible modifying factor of case evolution. A 12-year-old boy presented for treatment complaining of a defective restoration and mild pain on the maxillary right central incisor. The patient had a history of crown fracture in this tooth due to trauma 2 years before. The clinical examination showed healthy gi

Intentional replantation with apical periodontal ligament preservation: A case report.

BACKGROUND: Intentional replantation (IR) is a conservative treatment option for compromised teeth when the viability of the periodontal ligament (PDL) can be preserved. Maintaining viable PDL cells and using bioactive agents such as ethylenediaminetetraacetic acid (EDTA), platelet-rich fibrin (PRF), and injectable platelet-rich fibrin (i-PRF) may enhance the regenerative potential of replanted teeth. METHODS: A 59-year-old female patient presented with Grade III mobility and extrusion of maxillary tooth #9 following trauma. Clinical examination revealed a probing depth of 10 mm with bleeding

Neoplasia masquerading as periapical infection.

Seven examples of neoplasia which presented as periapical radiolucencies are described. These were all initially treated for presumed periapical infection. The atypical features that should alert dentists to the possibility of a tumour presenting in this manner are: a vital tooth with minimal caries, root resorption and an irregular radiolucent outline, tooth mobility in the absence of generalised periodontal disease, regional nerve anaesthesia, and failure to respond to good endodontic therapy. All material removed at the time of apical surgery must be examined histologically to prevent neopl

From hopeless to good prognosis: journey of a failing tooth.

Chronic periodontitis, along with associated clinical findings such as pathologic tooth migration, diastema, functional and aesthetic aberrations, poses an immense challenge to a dental professional. These findings convert clinical decision making into a daunting task and adversely affect the prognosis and the treatment plan for the presenting clinical problem. An interdisciplinary approach aimed at restoring functional and aesthetic needs of the affected individual within the limitations of such a compromised clinical scenario may be a viable alternative to any radical treatment causing loss

The fate of a mid-root fracture: a case report.

The present report describes a case of a mid-root fracture in a maxillary central incisor of a 19-year-old patient. The fractured tooth was splinted with composite that was removed only 3 years later, as the patient did not appear for follow-up examinations. At this time, the radiographs revealed a normal periodontal ligament, rounding of the borders of the fragments and pulp obliteration of both fragments. Eight years later, the tooth was clinically normal and blurred calcification of the root canal was disclosed radiographically. After 13.5 years the patient complained of tooth mobility and

Sleep-related faciomandibular myoclonus: A sleep-related movement disorder different from bruxism.

We describe a 33-year-old man who presented with lip and tongue nibbling and bleeding during sleep. Videopolysomnography revealed myoclonic jerks involving the masticatory and facial muscles recurring mainly during NREM sleep. There was no tonic EMG masticatory activity typical of bruxism. EMG analysis demonstrated the recruitment of V- to VII innervated muscles and, in half of the episodes, also the sternocleidomastoideus. Our patient had sleep-related faciomandibular myoclonus (SFMM) with spontaneous jerks of oromasticatory and cervical muscles, occurring only during sleep. Tooth grinding, t

Regional odontodysplasia: report of four cases.

Four cases of regional odontodysplasia are described. Clinically erupted teeth appear hypoplastic and are often mobile. The radiographic appearance has given rise to the term 'ghost teeth' and the cases presented demonstrate this with short roots which are less well calcified than normal teeth. Clinical and radiographic findings showed that this condition affects only one side of the jaw, but can be seen in any part of the upper or lower jaws. This article represents cases in different parts of the mouth with varying degrees of involvement.

[A bacteriological study of supragingival bacterial plaque in subjects undergoing orthodontic therapy].

Direct contact between tooth enamel and bacteria is currently considered a sine qua non of tooth decay. Any condition which encourages the deposit and accumulation of bacterial plaque represents a risk factor for tooth decay. Dental therapy often entails the use of bands or brackets which house metal apparatus used to apply force or support mobile devices (plates, etc.). It is therefore possible to hypothesise that, by preventing its mechanical removal, dental treatment can lead to quantitative and qualitative changes in bacterial plaque.
