Emergency tourniquet effectiveness in four positions on the proximal thigh.

OBJECTIVE: The purpose of the present study is to determine the performance of tourniquet use by the placement of the tourniquet's windlass on the extremity in four positions--medial, lateral, anterior, and posterior--to inform tourniquet instructors and develop best tourniquet practices. METHODS: A HapMed™ Leg Tourniquet Trainer was used as a manikin to test the effectiveness of an emergency tourniquet, the Special Operations Forces Tactical Tourniquet. Two users made 10 tests, each in four positions. RESULTS: Effectiveness rates of tourniquet use were 100% in all four positions. The two to

Tourniquet use for peripheral vascular injuries in the civilian setting.

BACKGROUND: Haemorrhage in peripheral vascular injuries may cause life-threatening exsanguination. Tourniquets are used extensively by the military, with increased interest in the civilian setting to prevent deaths. This is a retrospective study of trauma patients at two large Canadian trauma centres with arterial injury after isolated extremity trauma. We hypothesized that tourniquet use may decrease mortality rate and transfusion requirements if applied early. METHODS: The study group was all adult patients at two Level 1 Trauma Centres in two Canadian cities in Canada, who had arterial inj

[Use of the tourniquet test in diagnosing dengue-like syndrome].

A 26-year-old nurse presented with a three-day history of sudden onset of pyrexia of 39 °C, malaise, and a patchy maculopapular rash, after returning from a monthlong work in Senegal. A tourniquet was applied to help locate the vein for diagnostic blood sampling. After the procedure, a petechial rash erupted with a demarcation line below the level of the tourniquet. A diagnostic test for dengue fever was performed and confirmed by the polymerase chain reaction (PCR) to be positive for serotype DEN-1. The tourniquet test is performed by inflating a blood pressure cuff tied on the upper arm to a

Stretch and wrap style tourniquet effectiveness with minimal training.

UNLABELLED: The objective was to determine if proper application of the Stretch, Wrap, and Tuck Tourniquet (SWAT-T) would stop arterial flow and would occur with minimal training. METHODS: Fifteen undergraduates watched a 19 second video three times, practiced twice, and applied the tourniquet to volunteers at 10 locations: 3 above the elbow or knee and 2 below. RESULTS: Successful occlusion (60 second Doppler signal elimination) was more frequent than proper stretch (96 versus 75), more frequent on arms than legs (59 versus 37), and achieved before completed application (16 +/- 8 versus 33

A case of suicidal ligature strangulation by using a tourniquet method.

Ligature strangulations are usually homicidal. Accidental and suicidal ligature strangulations are quite rare. In this study, an unusual case of suicidal ligature strangulation with a tourniquet method using a walking stick is presented.A 70-year-old-man was found dead in his room, a piece of cloth wrapped around the neck, knotted, and tightened by a walking stick. His hand was still on the walking stick which was seemingly used by him for a tourniquet effect. Autopsy showed bilateral sternocleidomastoid muscle hemorrhage. A thorough death scene investigation, detailed corpse examination, and

Limb complications following pre-hospital tourniquet use.

It has been stated that the application of a pre-hospital tourniquet could prevent 7% of combat deaths, however their widespread use has been questioned due to the potential risk from prolonged ischaemia. We reviewed members of the UK Armed Forces who sustained severe limb-threatening injuries in Iraq and Afghanistan, and performed a matched cohort study based on the presence or absence of pre-hospital tourniquet application. When a pre-hospital tourniquet had been applied, 19/22 patients had a least one complication compared to 15/22 where no tourniquet had been applied [p = 0.13]. There were

Repeated tourniquet testing as a diagnostic tool in dengue infection.

Prospective evaluation of repeated standard tourniquet testing as a diagnostic indicator of dengue infection was done. Included were patients admitted to a children's hospital in Kuala Lumpur on a clinical suspicion of dengue infection based on the World Health Organization (WHO) criteria. A standard method of tourniquet was performed on 79 patients on a daily basis following admission. subjects and negative in the remaining 14 subjects. Fifty-eight subjects were serologically confirmed cases, 4 indeterminate and the remaining 17 subjects had negative serology. For diagnostic classification, 1

Survey of tourniquet use in orthopaedic foot and ankle surgery.

BACKGROUND: Tourniquet technique varies among foot and ankle surgeons, and to establish a standard practice guideline the current standard of care should be examined. METHODS: One hundred and forty responses were received after 253 surveys were mailed to American Orthopaedic Foot and Ankle Society (AOFAS) members, concerning type of tourniquets, location, and pressures used. RESULTS: Cuff pressures most commonly used were 301 to 350 mmHg for thigh cuffs (49% of thigh cuff users) and 201 to 250 mmHG for calf and ankle cuffs (52% of calf cuff users, 66% of ankle cuff users). A substantial numb

[A case of pulmonary embolism associated with pneumatic tourniquet deflation].

Intraoperative pulmonary embolism is an unusual complication of lower extremity surgeries. We report a case of cardiac arrest due to pulmonary embolism associated with pneumatic tourniquet deflation. A 48-yr-old woman underwent achillorrhaphy under spinal anesthesia. About 4 min after deflation of the tourniquet, cyanosis appeared, and her consciousness was lost. Endotracheal intubation was performed, and ephedrine and epinephrine were injected intravenously. Right ventricular dilation was noted in echocardiography, although left ventricular contractility was intact. Multiple emboli were obser

A prospective randomised trial of tourniquet in varicose vein surgery.

A prospective randomised trial of 50 patients was carried out to assess the autoclavable Lofquist cuff (Boazal, Sweden) as a tourniquet in varicose vein surgery and determine the effect on bleeding, bruising, cosmesis and patient pain and activity. Patients undergoing unilateral long saphenous vein ligation, stripping and avulsions were randomised to tourniquet or no tourniquet. Lofquist cuffs were applied after inflation to 120 mmHg to the upper thigh for the duration of the surgery. Varicose vein grade, duration of surgery, blood loss, extent of bruising at 7 days, pain and activity scores o

Use of an adjustable tourniquet to reverse cyanosis in the newborn pig.

BACKGROUND: Previous surgical models of cyanosis have been permanent. Because normal oxygenation was not restored in these models, it is unclear whether the metabolic changes produced by prolonged exposure to hypoxemia are irreversible. We therefore designed an experimental model of cyanosis that is reversible. METHODS: The left atrial appendage was anastomosed directly to the main pulmonary artery in 8 piglets, aged 2 to 4 weeks. RESULTS: The oxygen saturation fell from 95.3% +/- 0.8% to 72.4% +/- 3.9% (p < 0.001). A tourniquet was placed around the anastomosis to produce incremental change

Upper extremity tourniquet effects in carpal tunnel release.

We studied the presence and extent of upper extremity denervation in limb muscles after surgical release of the carpal tunnel. Sixty consecutive patients were randomly assigned to have the procedure performed with (31 patients) or without (29 patients) tourniquet application. Cervical paraspinal and upper extremity muscles were examined electromyographically 3 weeks before and 3 weeks after carpal tunnel release. Three weeks after the operation 77% of the patients in the tourniquet group had denervation in other than thenar muscles. Only one patient in the control (no tourniquet) group had sim

The effects of tourniquet use in elbow surgery: a retrospective analysis.

BACKGROUND: Pneumatic tourniquets are frequently employed in extremity surgeries, aiming to enhance intraoperative visibility and minimize blood loss. Although their benefits and drawbacks have been extensively studied in lower limb operations, their impact on upper limb procedures, particularly elbow surgery, remains poorly understood. This study investigates the advantages and risks associated with pneumatic tourniquet used in elbow surgery. METHODS: A retrospective analysis was conducted on 183 patients who underwent elbow surgery for fractures between January 2019 and September 2023. Pati
