BACKGROUND: Trachelectomy is a conservative but locally radical procedure associated with a high risk of preterm delivery.
CASE: A 28-year-old patient with cervical cancer FIGO stage IB1 was treated with laparoscopic pelvic lymphadenectomy followed by trachelectomy. Three years later, she conceived spontaneously. In consideration of the high risk of preterm delivery, the cervical status was evaluated by transvaginal ultrasonography. At 16 weeks' gestation, we observed the cerclage suture correctly placed at the level of the internal cervical os and a "neo-cervical" segment length of 1.5 cm. T

OBJECTIVE: Among malignant neoplasms of the female reproductive system, cervical cancer traditionally occupies a leading position in terms of both morbidity and mortality. Traditional surgical treatment of early-stage cervical cancer leads to satisfactory oncological results, but irreversibly reduces the fertility in this category of patients. The aim of this work was to evaluate the applicability of a metal-ceramic implant with shape memory made of titanium nickelide for the purpose of forming uterine closure and strengthening the anastomosis zone.
METHODS: A total of 168 untreated patients

PURPOSE: Radical trachelectomy (RT) with pelvic lymphadenectomy has become a new treatment option for young patients with uterine cervical cancer stages 1A2-1B1 who desire the preservation of their fertility. However, the application of RT for pregnant patients is still controversial. We comparatively studied both obstetrical and oncological outcomes of pregnant patients who underwent vaginal RT during pregnancy and those who underwent vaginal RT before pregnancy.
METHODS: Both obstetrical and oncological results of eight patients who underwent vaginal RT with pelvic lymphadenectomy during pr

OBJECTIVE: Radical vaginal trachelectomy (RVT) offers a possibility for future childbearing for young women with early-stage cervical cancer. However, the literature on quality of life and self-reported morbidity in patients undergoing RVT is scarce. The aim of this study was to prospectively assess quality of life after RVT with focus on urogynecological morbidity and lymphedema. Furthermore, the aim of this study was to compare results with those in women treated with radical abdominal hysterectomy (RAH) and with age-matched control women from the general population.

OBJECTIVE: To present and evaluate an unselected national single center strategy with fertility preserving trachelectomy in cervical cancer. In 2003 nationwide single-center referral of women for trachelectomies was agreed upon between all Danish departments performing cervical cancer surgery with the purpose of increasing volume, to increase surgical safety and facilitate follow-up.
METHODS: Prospective data were recorded in the Danish Gynecological Cancer Database of all Vaginal Radical Trachelectomies (VRT) performed in Denmark between 2002 and 2013. Oncologic, fertility and obstetrical ou

PURPOSE: The purpose of this study was to assess uterine enhancement rate after abdominal radical trachelectomy (ART) using dynamic contrast-enhanced (DCE) magnetic resonance (MR) imaging.
METHODS: Ten patients with early uterine cervical cancer, who were treated by ART, were included in this study. Each patient underwent DCE MR imaging using a 3 T unit to assess uterine enhancement rate at three times (before surgery and 1 and 3 months after surgery). The radiologist calculated mean signal intensities of the anterior and posterior myometrium and also measured the signal intensities of the ur

PURPOSE OF REVIEW: Fertility preservation in early-stage cervical cancer by total laparoscopic radical trachelectomy (TLRT) is gaining acceptance as more cases are published in the literature. The objective is to review all the literatures regarding TLRT especially over the last 12 months and to describe the technique, the operative outcomes, the oncologic outcomes and the obstetric outcomes of this procedure.
RECENT FINDINGS: As the number of cases reported in the literature increases, the effectiveness of TLRT for treating early-stage cervical cancer continues to gain support. Under the enh

Rhabdomyosarcoma arising in the female genital tract carries 5-year survival in excess of 80%, but lifelong infertility may be a consequence of local control strategies. We present the technique and outcome for a fertility-sparing, radical abdominal trachelectomy in a 12-year-old girl with anaplastic, embryonal rhabdomyosarcoma involving the uterine cervix. The patient had presented to our center after the piecemeal resection of a uterine cervical mass; because of concern about microscopic residual disease, we classified her as group II-A according to the Intergroup Rhabdomyosarcoma Study syst

This study examined the risk of intrauterine infection associated with radical trachelectomy (RT) in early-stage cervical cancer patients. This procedure preserves fertility but is linked to increased risk of intrauterine infection due to cervical defects during pregnancy. DNA was extracted from the formalin-fixed paraffin-embedded (FFPE) placental specimens of 23 pregnant post-RT patients and 16S rRNA gene sequencing was used for bacterial identification. The prevalence of Lactobacillus crispatus and Burkholderia stabilis was significantly higher in the non-chorioamnionitis group. In contrast

BACKGROUND: Fertility-sparing management for bulky cervical cancer remains under investigation. We investigated the safety of neoadjuvant transuterine arterial chemotherapy (TUAC) followed by a simple vaginal trachelectomy for patients with tumors larger than 3 cm in diameter who desired uterine preservation.
METHODS: First, to assess the safety and efficacy of TUAC, survival and pathological complete response data from 39 patients with a diagnosis of stage IB2 to stage IIB cervical cancer who participated in 2 consecutive trials during 1997-2006 were analyzed. The neoadjuvant chemotherapy re

We report on a patient with a cut-through endometrial malignancy after supracervical hysterectomy treated by radical trachelectomy and staging via a robotic approach. A 58-year-old patient with incidental finding of a stage IIB G1 endometrial adenocarcinoma after a supracervical hysterectomy underwent robotic radical trachelectomy, bilateral pelvic lymphadenectomy, and ileal resection with anastomosis. The operative time was 360 minutes, and blood loss was 100mL. The pathology report revealed no evidence of residual disease. The patient remains disease-free 18 months after her robotic procedur

OBJECTIVE: The importance of minimally invasive fertility-sparing surgery for cervical cancer is gaining increasing interest, both to achieve a cure and for future fertility. Procedures for robotic radical trachelectomy involving uterine reconstruction are not fully established.
METHODS: This study prospectively verified the feasibility and safety of robotic radical trachelectomy between February 2018 and May 2022. The criteria were almost identical to those for our standard abdominal radical trachelectomy. Larger tumors (> 2 cm in diameter) were acceptable for surgery, provided a secure ≥ 1
