Sustaining progress in HIV care in Africa with a transition to enabled self-care: a modelling study.

BACKGROUND: The funding crisis for HIV services in east, central, southern, and west Africa means that innovative strategies for continuing prevention and care for HIV are needed. One such strategy is enabled self-care: the provision of free HIV self-tests, pre-exposure prophylaxis (PrEP), tenofovir-lamivudine-dolutegravir for post-exposure prophylaxis (PEP), and antiretroviral therapy (ART) provided in local pharmacies and by community-based health workers. We modelled the introduction of a policy of transition to enabled self-care compared with continuation of current service provision. MET

Social and Behavioral Barriers to Effective Care During the Transition to End-Stage Kidney Care.

Individuals living with CKD are disproportionately burdened by a multitude of adverse clinical and person-centered outcomes. When patients transition from advanced kidney disease to kidney failure, the psychosocial effects as well as social determinants of health challenges are magnified, making this a particularly difficult time for patients beginning kidney replacement therapy. The key social determinants of health challenges often include food and housing insecurity, poverty, unreliable transportation, low level education and/or health literacy, lack of language interpreters and culturally

Evaluation of a Health Care Transition Improvement Process in Seven Large Health Care Systems.

PURPOSE: Youth and young adults require systematic planning, transfer and integration into adult healthcare. A national health care transition (HCT) learning network (LN) shared strategies during monthly calls to improve HCTs using Got Transition™'s Six Core Elements. Among LN participants, we conducted a pre-post mixed-methods evaluation of this evidence-informed process improvement framework. DESIGN AND METHODS: Leaders from seven health systems in the LN recruited 55 participating practice sites (12 primary care, 43 specialty care, 47 pediatric care, and 8 adult care). Dyadic capability in post-stroke transitional care: a study protocol for instrument development and longitudinal evaluation.

INTRODUCTION: Stroke represents a major burden for both survivors and caregivers. Although caregiver preparedness has been widely studied, it is typically assessed through self-reported measures that capture subjective perceptions rather than real-life adaptive functioning. Conventional training approaches, focused on knowledge and technical skills, often fail to support sustainable long-term care. The concept of capability may offer a more comprehensive framework for evaluating caregiving; however, it remains insufficiently operationalized and empirically validated in stroke populations. AIM

Impact of a Comprehensive Transitional Care Management Model on Use of Community-Based Rehabilitation After Stroke.

BACKGROUND: Community-based physical and occupational therapy (PT/OT) are critical for stroke recovery but are underused. We conducted a secondary analysis of the COMPASS (Comprehensive Post-Acute Stroke Services) study, a pragmatic trial of comprehensive postacute transitional care (TC) to investigate whether TC programs increase PT/OT use. METHODS: Forty hospitals were randomized to implement COMPASS-TC or maintain usual care for patients with stroke/transient ischemic attack. In a crossover phase, usual care hospitals implemented COMPASS-TC. We linked participants to administrative claims

Implementation of Artificial Intelligence-Powered Patient Simulation for Communication Training in a Telehealth Transitional Care Program.

INTRODUCTION: Effective communication during care transitions is essential for patient safety and quality outcomes. Telehealth-based transitional care programs decrease 30-day readmissions and improve patient satisfaction, yet staff frequently report the need for structured communication training. Artificial intelligence (AI)-supported simulation may offer a scalable training method for health care settings with limited simulation resources. This study evaluates whether AI-powered simulation training improves communication performance during real telehealth patient encounters. METHODS: This q

Telehealth-Enabled Transitional Care Across Inpatient-to-Outpatient Settings in Patients with Diabetes: A Scoping Review.

BACKGROUND: The inpatient-to-outpatient transition is a critical and vulnerable period for patients with diabetes, frequently marked by fragmented communication, incomplete follow-up, and preventable readmissions. Telehealth has emerged as a promising approach to bridge this gap; however, the design and implementation of telehealth-enabled transitional care models have not been systematically characterized. OBJECTIVE: This scoping review aimed to map existing evidence on telehealth-enabled transitional care for patients with diabetes; characterize intervention components, modalities, and targ

Transitional care services are cost-effective for older patients with chronic obstructive pulmonary disease in Korea.

BACKGROUND: Transitional care services (TCS) may reduce post-discharge readmissions and improve disease management in patients with chronic obstructive pulmonary disease (COPD). This study evaluated the cost-effectiveness of TCS using state-specific relative risks (RRs) derived from a randomised controlled trial (RCT). METHODS: Between September 2022 and September 2024, 417 patients with COPD discharged from three university hospitals in South Korea were assigned to TCS (n = 210) or usual care (n = 207). State-specific RRs were derived from 3-month RCT outcomes and incorporated into a Markov

[Questionnaire survey regarding pediatric to adult transitional medical care targeting members of Japanese Society of Neurology].

The Special Committee on Measures for Transition from Pediatric to Adult Health Care surveyed approximately 9,000 members of the Japanese Society of Neurology regarding transitional care. Only 744 responses were returned, less than 10% of the total number of members contacted. More than half answered that they generally provide treatment for adult patients with a childhood-onset disease, with many noting as reasons that the related diseases and ages in such cases are targeted by neurology specialists, and that other adult medical departments are not equipped to treat them. As for reasons given

Cross-sector collaboration in transitional care of people experiencing homelessness: insights from an exploratory network analysis.

Interprofessional care coordinators informally collaborate across sectors to facilitate care of patients with multimorbidity and social complexity, such as in the case of ill-housed persons at hospital discharge. The complexity of homeless patients demands research on cross-sector collaborative interventions like medical respite, a cross-sector transitional care model. Relational Coordination (RC) and exploratory social network analysis were used to assess the cross-sector collaboration of a medical respite network (N = 15 organizations), by comparing administrative (n = 20) and frontline care

Transitional care after hospitalization for sepsis in Germany- results from the population-based AVENIR cohort study.

PURPOSE: Sepsis survivorship is associated with significant long-term morbidity, mortality and health care utilization. Transitional care between inpatient and follow-up care is crucial, but insufficiently understood. We investigated health care utilization in sepsis survivors 90 days post-discharge, comparing translational care during 2016-2019 vs. 2020 in the first year of the pandemic. METHODS: This retrospective cohort study used nationwide health claims data of the "AOK- die Gesundheitskasse". Sepsis patients with inpatient treatment in 2016-2019 were identified using explicit ICD-10 cod

Transitional care in clinical networks for young people with juvenile idiopathic arthritis: current situation and challenges.

Clinical networks for paediatric and adolescent rheumatology are evolving, and their effect and role in the transition process between paediatric and adult services are unknown. We therefore explored the experiences of those involved to try and understand this further. Health professionals, young people with juvenile idiopathic arthritis and their families were recruited via five national health service paediatric and adolescent rheumatology specialist centres and networks across the UK. Seventy participants took part in focus groups and one-to-one interviews. Data was analysed using coding, m

Under-recognition of 22q11.2 deletion in adult Chinese patients with conotruncal anomalies: implications in transitional care.

22q11.2 deletion syndrome (22q11.2DS) is a multi-systemic disorder with high phenotypic variability. Under-diagnosis in adults is common and recognition of facial dysmorphic features can be affected by age and ethnicity. This study aims to determine the prevalence of undiagnosed 22q11.2DS in adult Chinese patients with conotruncal anomalies and to delineate their facial dysmorphisms and extra-cardiac manifestations. We recruited consecutively 156 patients with conotruncal anomalies in an adult congenital heart disease (CHD) clinic in Hong Kong and screened for 22q11.2DS using fluorescence-PCR

Impact of Demographic Factors and Other Characteristics on Housing Outcomes at Discharge from Transitional Care Program.

Homelessness is a major public health concern, and successful rehousing is an important outcome for people experiencing homelessness (PEH). However, limited evidence exists on which individual factors are associated with rehousing after transitional care; this study examined characteristics associated with being rehoused at discharge from Durham Homeless Care Transitions (DHCT). We analyzed data from DHCT, a transitional care program serving PEH. Independent variables included demographic characteristics, self-efficacy, mental healthcare status, and unmet identity and communication access (ICA

Embedding Clinical Pharmacy in Transitional Care: A Telehealth Comprehensive Medication Management Intervention for Postdischarge Glycemic Optimization.

BACKGROUND: Hospital discharge is a high-risk period for patients with diabetes. Pharmacist-led comprehensive medication management (CMM) reduces hemoglobin A1c (HbA1c) in ambulatory settings, but structured telehealth CMM embedded within hospital-based transitional care platforms is rarely described. This article reports the implementation, fidelity, and observed outcomes of a telehealth CMM program for patients discharged with poorly controlled diabetes from a nonacademic hospital. METHODS: Using the Institute for Healthcare Improvement Model for Improvement, eight Plan-Do-Study-Act (PDSA)
