Clinical anatomy of the m. flexor carpi radialis tendon sheath.

At the transitional zone from the forearm to the hand the insertion tendon of the m.flexor carpi radialis (FCR) glides on a fibrous and fatty cushion, which is connected dorsally with the joint capsule of the radiocarpal articulation. The tendon distally crosses the palmar side of the scaphoid tubercle and enters the dorsally curved rim of the trapezoid tubercle. At the level of the wrist joint the narrow tendon sheath begins, which extends to the insertion at the metacarpus. Immediately after entering the gliding tunnel the tendon branches off radially as a rule with an accessory fibre strand

No correlation between severity of preoperative degenerative changes in the trapeziometacarpal joint and short-term clinical outcome after total joint arthroplasty.

In this study, we wanted to investigate whether the severity of preoperative degenerative changes in the trapeziometacarpal (TM) joint seen on CT scans would influence the clinical outcomes after total TM joint arthroplasty, and particularly whether discrete degenerative changes in the scaphotrapezial (ST) joint would negatively affect the clinical outcome. Patients classified as Eaton-Glickel stage 2 or 3, as well as patients with Eaton-Glickel stage 4 disease who had discrete degenerative changes in the ST joint (i.e., narrowing and sclerotic changes but no osteophytes) were included in the

[Anterior traumatic axial disruption of the middle carpal joint. Case report with literature review].

We report a case of traumatic axial disruption of the radial mid carpal joint. This unusual type of scaphotrapezo-trapezoïdal dislocation occurred after falling from a height. We performed open reduction and wire fixation. At 12months follow-up, functional outcome was satisfactory with good muscle strength (clamping force, grip strength and pinch grip: 90% compared to the opposite side) and good range of motion was preserved. Work was resumed at the tenth week. Radial midcarpal disruption is uncommon. It includes the separation of the trapezium, trapezoïdal, first and second and third metacarp

Minimally invasive flap compared to a trapezoidal flap in lateral approach maxillary sinus elevation procedures: Four-month post-loading results from a split-mouth randomised controlled trial.

PURPOSE: To test a minimally invasive flap in the lateral approach for maxillary sinus floor elevation when compared to a trapezoidal flap. MATERIALS AND METHODS: Each patient received a bilateral sinus elevation procedure based on two different randomly allocated surgical approaches according to a split-mouth design: in the test side, a horizontal incision at mucogingival line was realised; in the control side, a trapezoidal flap was elevated to prepare the lateral window. Each sinus was filled using deproteinised bovine bone and the window covered with a collagen membrane. Implants were ins

Corrective osteotomy for symptomatic scaphoid malunion.

A prospective study aimed at assessment of the outcome of management of symptomatic scaphoid malunion (hump-back deformity). The work included 13 scaphoid malunions in 13 patients. All patients complained of weak painful hand grip and limitation of wrist function. All cases were subjected to a corrective opening wedge scaphoid osteotomy with insertion of a trapezoid-shaped tricortical iliac bone graft. At the final assessment, after a mean follow-up period of 42 months, the achieved results were rated excellent in seven cases, good in four and fair in two according to the scoring system used.

Designing an Optimized Novel Femoral Stem.

BACKGROUND: After total hip arthroplasty, there would be some problems for the patients. Implant loosening is one of the significant problems which results in thigh pain and even revision surgery. Difference between Young's modulus of bone-metal is the cause of stress shielding, atrophy, and subsequent implant loosening. MATERIALS AND METHODS: In this paper, femoral stem stiffness is reduced by novel biomechanical and biomaterial design which includes using proper design parameters, coating it with porous surface, and modeling the sketch by the software. Parametric design of femoral stem is d

Complications and results of scapho-trapezio-trapezoid arthrodesis.

Forty cases of scapho-trapezio-trapezoid (STT) arthrodesis were reviewed. Indications for the procedure included rotatory subluxation of the scaphoid (30 patients) and STT arthrosis (ten patients). Follow-up time averaged 41 months. Ten patients (25%) required 13 additional procedures: operations for nonunion or a fracture of the distal radius after bone harvesting, partial radial styloidectomy, first carpometacarpal joint arthroplasty, and wrist arthrodesis. Complications without surgeries were seen in several cases: pin-tract infection, sympathetic dystrophy, delayed union treated with brace

Influence of the scaphotrapeziotrapezoid joint on the results of total trapeziometacarpal prostheses.

INTRODUCTION: Scaphotrapeziotrapezoid (STT) joint osteoarthritis may influence the outcome after trapeziometacarpal prosthesis (TMP) implantation. The literature regarding its clinical and radiological assessment is unclear. The aim of our study was to determine by means of a pre- and post-intervention CT study whether the degree of STT involvement influences the clinical-functional or radiological results after TMP implantation, and to establish whether or not STT osteoarthritis could be a contraindication for the use of these implants. METHODS: Prospective study of 60patients with trapeziom

[Effect of dia- and percondylar humeral fractures on the growth of the carpal bones in children].

Using surface measurements obtained from anterior-posterior X-rays, the carpal bones on the side of the previous supracondylar fracture were compared with those on the uninjured side. A disparity in growth was found between the two sides. Most often, an increased growth of the scaphoid was seen on the side of the fracture. Analysis of the data obtained from the measurements of all carpal bones did not lead to any definite conclusions. Of the seven carpal bones studied, a greater length (1 to 3 mm.) was found 86 times and a greater width (1 to 4 mm.) 53 times. Decreased length (1 to 4 mm.) was

Treatment of first metatarsal longitudinal epiphyseal bracket by excision before closure.

The longitudinal metatarsal epiphyseal bracket is a rare ossification abnormality of tubular bones in which an abnormal epiphysis brackets the diaphysis of a phalanx, metacarpal, or metatarsal. This leads to progressive shortening and angular deformity of the involved bone. When it affects the first metatarsal, the longitudinal metatarsal epiphyseal bracket leads to a short, trapezoidal metatarsal. We present the case of a 6-year-old patient with an ossified longitudinal epiphyseal bracket along the medial side of the first metatarsal. The abnormality was surgically treated by bracket excision

Trapezoidal titanium cage in anterior cervical interbody fusion: a clinical experience.

BACKGROUND: Use of autogenous bone grafting taken from anterior iliac crest for anterior cervical interbody fusion has been the "gold-standard" for decades. Substitutes for autogenous bone graft continue to be sought to avoid potential donor site morbidity. A titanium-alloy square-shaped hollow cage was applied to facilitate the process of fusion as a bone graft substitute, then assessed in this retrospective clinical study. METHODS: From July 2000 to July 2001, sixty-three discs from fifty-four patients were selected to receive this cage for one or two segments of the cervical spine for ante

HarDNet-based deep learning model for osteoporosis screening and bone mineral density inference from hand radiographs.

PURPOSE: Osteoporosis, affecting over 200 million individuals, often remains unrecognized and untreated, increasing the risk of fractures in older adults. Osteoporosis is typically diagnosed with bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA). This study aims to develop DeepDXA-Hand, a deep learning model using the efficient CNN-based deep learning architecture, for opportunistic osteoporosis screening from hand radiographs. METHODS: DeepDXA-Hand utilizes a CNN-based, HarDNet, approach to predict BMD non-invasively. A total of 10,351 hand radiographs and DXA pai

A retrospective review of long anterior fusions to the sacrum.

BACKGROUND CONTEXT: In the setting of tumor, infection, or trauma, a corpectomy of the L5 vertebral body may be necessary. However, the space has an irregular trapezoidal shape, and the failure to account for this may lead to improper fitting of the titanium cages or the allograft struts when performing a reconstruction. PURPOSE: The purpose of this study was to evaluate the failure rate of implants used to reconstruct the anterior lumbar spine when an L5 corpectomy has been performed. METHODS: A retrospective review of the medical records and radiographs of 19 consecutive patients undergoin

Treatment of thumb carpometacarpal arthritis with arthroscopic hemitrapeziectomy and interposition arthroplasty.

The carpometacarpal joint of the thumb is a common site of degenerative arthritis. Several surgical treatments exist, but arthroscopic management offers the potential benefit of earlier recovery. The current study evaluated the early clinical outcomes of a procedure involving arthroscopic hemitrapeziectomy with Artelon spacer (Artimplant, Västra Frölunda, Sweden) interposition arthroplasty into the newly created carpometacarpal space.A chart review of 9 patients treated with thumb carpometacarpal arthroscopic hemitrapeziectomy and Artelon spacer interposition arthroplasty between September 200

Comparison of Radiographic and Intraoperative Visual Assessment of Scaphotrapezoid Joint Arthritis in Patients With End-Stage Carpometacarpal Arthritis of the Thumb Base.

Background: The purpose of this investigation is to compare the radiographic and intraoperative assessment of scaphotrapezoid (ST) joint arthritis in patients with end-stage carpometacarpal (CMC) arthritis of the thumb base. We aim to define the incidence of ST arthritis in this population and determine whether radiographic features such as lunate morphology, dorsal intercalated segment instability (DISI), and scapholunate (SL) diastasis are associated with the incidence of ST arthritis. Methods: We retrospectively reviewed consecutive patients with end-stage CMC arthritis of the thumb treated

Simultaneous Dual Prosthetic Replacement of Trapeziometacarpal and Scaphotrapezial-Trapezoid Joints in Pantrapezial Osteoarthritis: Midterm Results of a Combined Implant Strategy.

PURPOSE: Pantrapezial osteoarthritis affects the trapeziometacarpal (TMC) and scaphotrapezial-trapezoid (STT) joints, causing pain, functional loss, and sometimes necessitating surgery. Among the various reconstructive options available for pantrapezial osteoarthritis, trapeziectomy and ligament reconstruction with tendon interposition have consistently yielded favorable long-term outcomes in many series. However, implant arthroplasty remains a valuable alternative for selected patients. This study evaluated a dual-prosthesis approach designed to preserve carpal stability and optimize thumb fu

[Acute traumatic dislocation of the trapeziometacarpal joint treated by percutaneous pinning. Review of six cases].

The authors report a series of six cases of acute traumatic dislocation of the trapeziometacarpal joint of the thumb. Displacement was always dorso-radial. There were associated osseous lesions in one patient: avulsion of the palmar tubercle of the base of the first metacarpal with a fracture of the trapezoid. All the patients were treated immediately by percutaneous pinning according to the technique of Wiggins. The late results, evaluated at an average of 19 months postinjury (range: 8-29 months), were considered to be satisfactory in the majority of the patients. Treatment by percutaneous p

Long-term results of total hip arthroplasty in congenital dislocation and dysplasia of the hip. A follow-up note.

Twenty-three of twenty-nine hips that were previously reported on were studied at an average of fourteen years (range, eight to 16.5 years) after total hip arthroplasty with cement. The original arthroplasties had been performed between November 1971 and January 1976. In all hips, there was dislocation or severe dysplasia. In six hips, superolateral bone grafts were used to increase acetabular coverage. At the latest follow-up examination, seven hips were rated excellent; nine, good; and one, fair. There were six failures (26 per cent) that were revised: four hips (17 per cent) had a fractured
