The complexity of trauma to the cranio-cervical junction: correlation of clinical presentation with Doppler flow velocities in the V3-segment of the vertebral arteries.

BACKGROUND: Trauma to the craniocervical junction (CCJ) encompasses a wide and complex spectrum of injury with an even broader range of clinical presentation, and since trauma to this area borderlines neurosurgical main areas of interest, these relatively rare injuries can be overlooked. In fact, there has been an abundance of documentation in the literature since Sir Charles Bell first reported on a case of occipital condyle fracture in 1817 in which a patient suddenly dropped dead as he turned his head to thank physicians and nurses while leaving the hospital. The death was attributed to com

Correlates of Skin Conductance Reactivity to Stroke-Related Trauma Reminders During Hospitalization for Stroke.

OBJECTIVE: Although several risk factors for stroke-induced posttraumatic stress disorder (PTSD) have been identified, objective risk measures that can be detected in the acute aftermath of these events are needed. This study is the first to collect an objective measure of psychophysiological arousal-skin conductance (SC) reactivity to a trauma interview-in patients after stroke or transient ischemic attack (TIA) and investigate correlates of SC reactivity. METHODS: Mobile SC measurement during a resting baseline and standardized trauma interview was performed in-hospital in 98 individuals fo

Neat1 lncRNA organizes the inflammatory gene expressions in the dorsal root ganglion in neuropathic pain caused by nerve injury.

Primary sensory neurons regulate inflammatory processes in innervated regions through neuro-immune communication. However, how their immune-modulating functions are regulated in concert remains largely unknown. Here, we show that Neat1 long non-coding RNA (lncRNA) organizes the proinflammatory gene expressions in the dorsal root ganglion (DRG) in chronic intractable neuropathic pain in rats. Neat1 was abundantly expressed in the DRG and was upregulated after peripheral nerve injury. Neat1 overexpression in primary sensory neurons caused mechanical and thermal hypersensitivity, whereas its knoc

Vasospasm following low-velocity penetrating pediatric intracranial trauma.

BACKGROUND: BB guns or non-powder guns created in the modern era are able to reach exceedingly fast velocities as a result of advances in compressed-gas technology. While missile penetrating trauma has been well documented in neurosurgical literature, penetrating intracranial injury secondary to non-powder guns, along with their associated complications and treatments, is not well described, and even less so in the pediatric population. CASE PRESENTATION: Here, we describe an unusual case of a 6-year-old Indigenous child who was accidentally shot with a BB gun to the head. He subsequently dev

Sjögren's syndrome with nervous system injury combined with pulmonary and osseous cryptococcosis: a case report.

BACKGROUND: Sjögren's syndrome is a common autoimmune disease that can involve the nervous system, but rarely both the central and peripheral. Long-term use of high-dose corticosteroids and immunosuppressants are the main risk factors for Cryptococcus infection in patients with Sjögren's syndrome, of which pulmonary infection is the most common, while multiple bone infections are rare. CASE PRESENTATION: A 46-year-old Chinese woman with a 2-year history of Sjögren's syndrome presented to our hospital with numbness of limbs, shortness of breath, and weakness. Blood immunochemistry showed that

[Nerve Injuries in Hip and Knee Arthroplasty - Risk Factors, Diagnostic and Therapeutic Approaches].

Iatrogenic nerve injuries are rare complications of total hip and knee arthroplasty, which may cause chronic pain and loss of function, severely affecting the patient's daily activities and quality of life. Nerves "at risk" include the sciatic nerve, the femoral nerve, the lateral femoral cutaneous nerve and the superior gluteal nerve during total hip arthroplasty, and the infrapatellar branch of the saphenous nerve as well as the peroneal nerve during total knee arthroplasty. Multiple procedure-related and patient-related factors have been identified to modify the risk of nerve injury in the

Selective activation of microglia in spinal cord but not higher cortical regions following nerve injury in adult mouse.

Neuronal plasticity along the pathway for sensory transmission including the spinal cord and cortex plays an important role in chronic pain, including inflammatory and neuropathic pain. While recent studies indicate that microglia in the spinal cord are involved in neuropathic pain, a systematic study has not been performed in other regions of the central nervous system (CNS). In the present study, we used heterozygous Cx3cr1GFP/+mice to characterize the morphological phenotypes of microglia following common peroneal nerve (CPN) ligation. We found that microglia showed a uniform distribution t

Management of birth injuries.

Birth injuries are a significant cause of neonatal morbidity and mortality. Although often associated with traumatic delivery, birth injuries often occur in normal spontaneous deliveries in the absence of any risk factors. This article discusses the diagnosis and management of the most common birth injuries that are encountered by health care providers who care for newborns.

[Epidemiological aspects in occupational neurology. Investigational methodology]].

INTRODUCTION: In the working population, unsatisfactory working conditions (in the broadest sense) may cause effects whose gravity depends mainly on the characteristics of exposure and the general condition of the individual. DEVELOPMENT: These effects may, in practice, be classified as occupational accidents and professional illness (both legal concepts), illnesses related to work, diminished quality of life and effects on offspring. Agents as varied as head injuries and metals are known to cause from loss of memory or behavior disorders to chronic toxic encephalopathy or Parkinsonism. There

Central nervous system superficial siderosis, headache, and epilepsy.

Almost 95 cases of superficial siderosis of the central nervous system have been reported in the literature. These patients showed a clinical syndrome characterized by ataxia, deafness, pyramidal system involvement, and mental deterioration with xanthochromic cerebrospinal fluid and neuroradiological findings of hemosiderin deposits. About 30% of the patients had headache as an accompanying symptom. In the present case report, we describe a 33-year-old man with the typical clinical features of superficial siderosis, who complained, since aged 8, of a severe recurrent frontal headache often ass

Acute electrical burns.

Electric current can damage an individual by thermal heating of the tissues; by disregulating autonomously functioning organ systems, such as the circulatory and respiratory systems; or by once-only or continuing stimulation of the nerves and striated muscles. In the first case, the result is coagulation necrosis, and in the second case life-threatening events can occur, such as arrhythmia or apnea. In the third case, vigorous nerve stimulation can lead to paralysis and vasospasm. Massive muscle contractions due to nerve stimulation or the direct triggering of striated muscles can cause ruptur

[Traumatic injury of ciliary ganglion].

The authors report a case in which slight cranial trauma was followed by manifestations of damage to the right ciliary ganglion. After 3 weeks the signs of damage began to regress and disappeared completely. Laboratory investigations failed to demonstrate any abnormalities. Apart from this trauma there were no other detectable causes, and especially there were no history data suggesting past infection, botulism or syphilis. The mechanism of ciliary ganglion injury is not known (contusion, haematoma, oedema etc.).

Serious head injury in young children: birth trauma versus non-accidental head injury.

Birth injury of the skull and central nervous system can be a complication of a difficult delivery, especially following forceps or vacuum-assisted delivery. Birth trauma of the head can also mimic the appearance of a non-accidental head injury and is therefore an important differential diagnosis. We report on two young infants with serious head injuries. The difficult differential diagnosis birth trauma versus non-accidental head injury is discussed and the necessity for cooperation between clinicians, forensic doctors and specialized neuroradiologists is emphasized.

Computed tomography of acute cerebral trauma.

Hounsfield's development of computed tomography (CT) in 1972 revolutionized the care of patients with acute craniocerebral trauma. CT evaluation facilitates early surgical and medical intervention and has significantly improved patient outcome. This review describes the role of CT in assessing acute head trauma. Despite the growing role of magnetic resonance imaging in the acute, subacute, and chronic phases of brain injury as well as in many other central nervous system disorders, CT retains its unique capacity to image acutely ill patients rapidly and accurately.

Effects of pudendal nerve injury in the female rat.

To test a neurogenic hypothesis for external urethral sphincter (EUS) dysfunction associated with urinary incontinence, the proximal pudendal nerve was crushed in anesthetized retired breeder female rats (n = 5) and compared with a sham lesion group (n = 4). Outcome measures included concentric needle electromyograms (EMGs) from the target EUS, voiding patterns during a 2-hour dark period, and micturition data over a 24-hour period. Fast Blue (FB) was introduced to the crush site at the time of injury and Diamidino Yellow (DY) to the EUS at the time the rats were killed (3 months post-operativ

Effect of perioperative autonomic nervous system imbalance on surgical outcomes: a systematic review.

The autonomic nervous system (ANS) is essential for maintaining physiological homeostasis. Autonomic nervous system imbalance, characterised by sympathetic hyperactivation and low parasympathetic tone, can occur during the perioperative period. These changes drive systemic stress responses, cardiovascular instability, impaired tissue repair, and immunosuppression, which in turn increase infection risk, neurocognitive decline, and multiorgan dysfunction. Surgical trauma, anaesthesia, pain, hypothermia, and psychological stressors all contribute to this dysregulation, and consequently low parasy
