We retrospectively reviewed all pediatric patients (< 18 years old) who presented to a Level I trauma center from 1984 to 1994 with noniatrogenic vascular trauma. There were 48 patients (42 male and 6 female) ages 2 to 17 years. Mechanism of injury included gunshot wounds (34) stab wounds (10), and blunt trauma (4). The lower extremities were most commonly injured (31), followed by upper extremity (17), trunk (8), and neck (4). Twenty-one (44%) patients had associated nonvascular injuries (primarily orthopedic or peripheral nerve). Eighteen (37%) patients underwent preoperative angiography for

BACKGROUND: During the past two decades, there have been many changes in automotive and medical technologies, road infrastructure, trauma systems, and demographic changes which may have influenced injury outcomes. The aim of this study was to examine injury trends among traffic casualties, specifically private car occupants, hospitalized in Level I Trauma Centers (TC).
METHODS: A retrospective cohort study was performed based on data from the Israel National Trauma Registry. The data included occupants of private cars hospitalized in all six Level I TC due to a traffic collision related injur

PURPOSE: Thoracic injury is a major contributor to morbidity in trauma patients. There is limited data regarding practice patterns of video-assisted thoracoscopic surgery (VATS) across trauma-accredited hospitals in the United States. We hypothesized that early VATS remains underutilized affecting patient outcomes.
METHOD: We evaluated a cohort of patients who underwent non-urgent thoracic surgical intervention for trauma from the ACS-TQIP database in 2017 excluding patients who were discharged within 48-h or died within 72-h. We selected patients who underwent partial lung resection and deco

BACKGROUND: Infants with abusive head trauma (AHT) have diffuse brain damage with potentially fatal brain swelling. The pathogenesis of the brain damage remains unclear. We hypothesize that brain damage in AHT is due to hypoxic-ischemic injury with hypoxic-ischemic encephalopathy (HIE) rather than primary traumatic brain injury (TBI) with traumatic diffuse axonal injury (tDAI).
METHODS: We studied brain tissue of AHT victims. Primary outcome measure was the presence of primary traumatic versus hypoxic-ischemic brain injury. The diagnosis of tDAI followed a standardized semiquantitative diagno

Background: Adolescent trauma patients (ATPs) with traumatic brain injury (TBI) are a population with challenges to standardizing treatment practices and optimizing outcomes. Adult trauma centers (ATCs) and pediatric trauma centers (PTCs) may have different care practices and outcomes for ATPs with severe TBI. Methods: A retrospective analysis was performed querying the 2020-2022 Trauma Quality Improvement Program database, observing treatments and outcomes for ATPs aged 10-19 years old with head Abbreviated Injury Scale (AIS) 3-5 and TBI diagnosis. Multivariable logistic regression (MLR) was

Trauma centers around the United States were surveyed to determine the extent and availability of early, aggressive, and appropriate speech rehabilitation services offered to trauma victims. Although significant increases were noted in specialty programs, augmentative systems usage, and follow-up services, there remains need for continued development in many aspects of early rehabilitation programs. As sophisticated medical management continues to salvage lives of America's prime resource, its young people, an added commitment is needed from all rehabilitation professionals to provide the righ

Abdominal trauma accounts for nearly 20% of all traumatic injuries. It often involves young patients sustaining multiple injuries, with a high associated mortality rate. Management should begin at the scene of injury and relies on a structured chain of care in order to transport the trauma patient to the appropriate hospital center. Management is multi-disciplinary, involving intensive care specialists, surgeons and radiologists. Imaging to precisely define injury is best performed with whole body dual phase computed tomography, which can also identify the source of bleeding. Non-operative man

BACKGROUND: Our aim was to express the importance of emergency diagnosis and surgical approach in penetrating cardiac trauma patients.
METHODS: Koşuyolu Heart and Research Hospital moved to its new location in Cevizli Kartal in June 2005. Due to its close proximity to the main roads of the city, the number of trauma cases has increased tremendously. We have retrospectively examined our penetrating cardiac trauma cases treated between June 2005 and September 2008.
RESULTS: Twenty-six trauma cases were admitted to our clinic with penetrating cardiac trauma. Twenty of them were operated on an e

PURPOSE: Mandible fractures are among the most frequently seen injuries in the trauma center setting. Recent shifts in the mechanism and age distribution of patients sustaining these injuries are well documented. This study attempts to define current, predictable patterns of fracture based on patient characteristics and mechanism of injury.
MATERIAL AND METHODS: The charts of 134 patients with 225 mandible fractures treated over a 7-year period by the Otolaryngology-Head and Neck Surgery, Plastic and Reconstructive Surgery and Oral-Maxillofacial Surgery services, our institution, were retrosp

OBJECTIVE: This study assessed the lifetime prevalence of traumatic events among consumers of a community mental health center by using a brief trauma screening instrument. This study also examined the relationship between trauma exposure and physical and mental health sequelae and determined whether the routine administration of a trauma screening measure at intake would result in increased diagnoses of posttraumatic stress disorder (PTSD) and in changes in treatment planning in a practice setting.
METHODS: A 13-item self-report trauma screening instrument, a shortened version of the Trauma

Trauma center registries are the foundation for many surveillance systems that attempt to define the frequency and spectrum of various types of injuries. To assess the representativeness of trauma center-based farm injury surveillance, we evaluated data for 1986-1991 from the Marshfield Clinic/St Joseph's Hospital, a major trauma center located in Central Wisconsin. We compared the pattern of farm injuries seen in residents of the Marshfield Epidemiologic Surveillance Area (MESA), a geographically defined, population-based surveillance area, with those from outside MESA, a nonpopulation-based

This is a report about six cases of replacement of the proximal femur by a cementless special prosthesis in modular system done in the Unfallkrankenhaus Wien-Meidling. One of the patients suffered by a lymphoma, three others had fractures caused by bone metastasis. One was a pertrochanteric fracture complicated by osteoporosis, and finally one fracture of the femur preceded by an insufficient implantation of a semitotal hip replacement. The hipsocket was supplied four times by a cementfree Polyäthylensocket, in one case a CLW-ring was implanted. Investigation showed that after a little more th

INTRODUCTION: Most patients with isolated, closed blunt intracranial hemorrhage (ICH) do not require neurosurgical intervention. However, outcome data for those managed entirely at Level III trauma centers remain incompletely characterized.
METHODS: Using the 2017-2020 Trauma Quality Improvement Program (TQIP), we compared clinical outcomes for non-operative ICH patients treated at Level I/II versus Level III trauma centers. The primary endpoint was composite in-hospital adverse event counts per patient, modeled using negative binomial regression and reported as adjusted incidence rate ratios

BACKGROUND: Patients with mild traumatic brain injury and traumatic intracranial hemorrhage are frequently transferred to higher-level trauma centers despite low rates of neurologic deterioration and operative intervention. Standardized brain injury algorithms prioritize safety but often escalate care for patients with small, nonoperative hemorrhages. In trauma systems with level III and IV centers lacking on-site neurosurgical coverage, identifying patients who can be safely managed locally remains challenging.
METHODS: We performed a retrospective cohort study of adult patients treated betw

PURPOSE: Pediatric pancreatic trauma is uncommon, but it is associated with significant morbidity and an ongoing debate regarding its management. Hemodynamic stability and pancreatic duct involvement are key factors in treatment decisions.
METHODS: A retrospective study analyzed 25 pediatric patients with pancreatic trauma treated between 2010 and 2020 at two high-complexity hospitals in Medellin. Clinical variables, trauma mechanism, imaging findings, management, and complications were analyzed.
RESULTS: Blunt trauma was the predominant mechanism (64%). Preoperative computed tomography was

INTRODUCTION: The factors explaining variability in trauma center performance across quality measures are not well understood. We aimed to characterize the influence of social, cultural, and organizational factors on trauma center performance.
METHODS: We used an explanatory sequential design to study ACS TQIP-participating level I and II trauma centers that were ranked as high- and low-performing (first and last decile) across multiple representative quality indicators. BACKGROUND: Regaining a higher degree of function after spinal cord injury (SCI) requires timely diagnosis and intervention. The American College of Surgeons (ACS) guidelines recommend that SCI should be treated in a designated Level I trauma center. Patients may initially go to lower-level trauma centers, delaying care. This study assesses whether the ACS recommendation is achieved by analyzing interhospital transfer (IHT) of SCI patients.
MATERIALS AND METHODS: Retrospective study using Pennsylvania Trauma Systems Foundation registry (2010 and 2020) to determine the trend of IHT of SCI pati
