We report a 66-year-old Japanese woman in whom tremor of lips appeared during speech. Her past and family histories were unremarkable. On neurological examination, there was no abnormal finding except the lip tremor. Results of laboratory findings were all within normal levels. Her MRI and EEG were normal. Surface EMG studies revealed that regular grouped discharges at a frequency of about 4-5 Hz appeared in the orbicularis oris muscle only during voluntary speaking. The tremor was not observed under conditions of a purposeless phonation or a vocalization of a simple word, suggesting that the

Neuroimaging has been used extensively to study tremors, providing invaluable information about the underlying mechanisms and applications to clinical practice. Although isolated tremor syndromes, such as essential tremor, are generally not associated with specific neuroimaging findings, structural (MRI, CT) and molecular (PET, SPECT) brain imaging can be useful in the differential diagnostics and identifying the etiology of tremor, especially in combined tremor syndromes such as parkinsonism-associated tremors and Holmes' tremor. In addition, brain MRI, including tractography and other connec

PURPOSE OF REVIEW: This review describes the most recent advances in epidemiology, classification, genetics, pathology and treatment of essential tremor. In addition, recent advances in more rare forms of tremor are summarized.
RECENT FINDINGS: Clinical, biochemical, pathological and imaging studies suggest an abnormal functioning of the cerebellum in essential tremor. Minor changes of cognition and personality may be due to secondary effects. Dementia and possible shortened life span seem to be limited to late-onset essential tremor. Many of these issues are not yet finally settled and need

Tremor is defined as rhythmic oscillatory activity of body parts. Four physiological basic mechanisms for such oscillatory activity have been described: mechanical oscillations; oscillations based on reflexes; oscillations due to central neuronal pacemakers; and oscillations because of disturbed feedforward or feedback loops. New methodological approaches with animal models, positron emission tomography, and mathematical analysis of electromyographic and electroencephalographic signals have provided new insights into the mechanisms underlying specific forms of tremor. Physiological tremor is d

BACKGROUND AND PURPOSE: Essential tremor (ET) is likely the most common movement disorder. The aim of the study was to carry out spectral analysis of the essential tremor recorded by an accelerometer and assess the symmetry of tremor parameters between the two hands.
MATERIAL AND METHODS: We examined 39 patients with ET diagnosed clinically using the criteria of the Movement Disorder Society. The control group consisted of 52 healthy persons. A biaxial accelerometer mounted at the dorsal side of the hand was used. Spectral analysis was performed. Tremor intensity, frequency of spectral peaks,

Essential tremor (ET) is a common condition that is present in as many as 23% of elderly individuals. Our objective was to determine the risk of ET and to study the impairment resulting from ET among relatives of ET cases compared to relatives of controls. ET cases and matched controls from the Washington Heights-Inwood community, New York, and their first- and second-degree relatives underwent a standardized tremor examination. The risk of having ET in relatives of cases vs relatives of controls was compared using Cox proportional hazards models. Five hundred ninety-one subjects were examined

Dystonia and tremor are common movement disorders with heterogenous presentations. Their overlap, termed "tremor in dystonia" for this review, is a common phenomenon that is classified inconsistently in practice and the literature. While the two-axis structure of the dystonia and tremor classifications clarified some aspects of tremor in dystonia, confusion and contention remain amongst clinicians. This is due not only to the varied definitions proposed over time and clinical heterogeneity, but the current lack of consensus on the underlying pathophysiology. This paper outlines the existing an

Tremor is a hyperkinetic movement disorder that consists of rhythmic, involuntary oscillatory movement of a body part. It may be present at birth or develop later during childhood. Pediatric tremor may be an isolated disorder or a sign of an underlying genetic and/or neurologic disease. This review provides a summary of the features and classification of pediatric tremors, recommended clinical evaluation, and currently available treatments.

Tremor and myoclonus are two common hyperkinetic movement disorders. Tremor is characterized by rhythmic oscillatory movements while myoclonic jerks are usually arrhythmic. Tremor can be classified into subtypes including the most common types: essential, enhanced physiological, and parkinsonian tremor. Myoclonus classification is based on its anatomic origin: cortical, subcortical, spinal, and peripheral myoclonus. The clinical presentations are unfortunately not always classic and electrophysiologic investigations can be helpful in making a phenotypic diagnosis. Video-polymyography is the ma

Tremor is clinically defined as a rhythmic, oscillating movement of parts of the body, which functionally leads to impairment of the coordination and execution of targeted movements. It can be a symptom of a primary disease, such as resting tremor in Parkinson's disease or occur as an independent disease, such as essential or orthostatic tremor. For the development of tremor, cerebral components as well as mechanisms at the spinal and muscular level play an important role. This review presents the results of new imaging and electrophysiological studies that have led to important advances in ou

BACKGROUND: Tremor is a symptom of many diseases and can constitute a disease of its own: essential tremor.
OBJECTIVE: The genetics of essential tremor and differential diagnosis of monogenic diseases with the symptom tremor.
MATERIAL AND METHODS: Literature search and search of clinical genetics databases, e.g. OMIM, GeneReviews, MDSGene and the German Neurological Society (DGN) guidelines.
RESULTS: The genetics of essential tremor remain unresolved in spite of large, adequately powered studies. Tremor is a symptom of differential diagnostic value in many movement disorders. A slight tremo

Tremor has been recognized as an important clinical feature in dystonia. Tremor in dystonia may occur in the body part affected by dystonia known as dystonic tremor or unaffected body regions known as tremor associated with dystonia. The most common type of tremor seen in dystonia patients is postural and kinetic which may be mistaken for familial essential tremor. Similarly familial essential tremor patients may have associated dystonia leading to diagnostic uncertainties. The pathogenesis of tremor in dystonia remains speculative, but its neurophysiological features are similar to dystonia w

Tremor is a hyperkinetic movement disorder characterized by rhythmic oscillations of one or more body parts. It can be disabling and may impair quality of life. Various etiological subtypes of tremor are recognized, with essential tremor (ET) and Parkinsonian tremor being the most common. Here we review the current literature on tremor treatment regarding ET and head and voice tremor, as well as dystonic tremor, orthostatic tremor, tremor due to multiple sclerosis (MS) or lesions in the brainstem or thalamus, neuropathic tremor, and functional (psychogenic) tremor, and summarize main findings.

The various causes of tremor are considered by order of prevalence in the elderly. 1) Essential tremor occurs in 5% of the population beyond 65 years of age. It is characterized by a tremor accompanying muscle activity, and interesting, when complete, the two upper limbs, the head and the voice, with variable degree of severity. Its progression is very slow. The diagnostic criteria are only clinical, but require ruling out iatrogenic and metabolic causes. 2) Tremor in Parkinson's disease is opposed point by point to essential tremor. However, confusion may occur due to some intermediate clinic

Tremor in childhood, beginning in the neonatal period, is more common than generally appreciated. Although some tremor disorders in children (eg, essential tremor) also affect adults, others (eg, shuddering, jitteriness, spasmus nutans, and vitamin B12-deficiency tremor) are seen exclusively in children. This review covers the etiology, clinical features, and treatment of the major tremor syndromes in children, and when appropriate, makes comparisons with similar disorders in adults.

Tremors other than those associated with Parkinson's disease are commonly encountered in clinical practice. The differentiation of tremor subtypes depends primarily on the presence of distinct clinical characteristics and is facilitated by the use of consistent nomenclature. Such differentiation can be helpful in determining the etiology of the tremor and assist in its management. In this review, the authors outline recently proposed changes to classification and review the clinical features, differential diagnosis, and current therapy for nonparkinsonian tremors.
