Weight-bearing test of traumatic triangular fibrocartilage complex lesion with unstable radioulnar joint.

BACKGROUND: Ulnar-sided wrist pain might be caused by a lesion of the triangular fibrocartilage complex (TFCC). Patients with TFCC lesion may show an instability of the distal radioulnar joint (DRUJ). Before arthroscopic assessment, conservative therapy using a brace or splint may result in alleviation of symptoms. The results of our previous study showed that patients with a traumatic TFCC lesion and instability of the DRUJ had the smallest weight-bearing capacity and had the largest increase in application of the wrist brace (WristWidget). PURPOSE: In this prospective study, we wanted to te

Clinical evaluation of a wrist sensorimotor rehabilitation program for triangular fibrocartilage complex injuries.

INTRODUCTION: Triangular fibrocartilage complex (TFCC) injuries are associated with distal radioulnar joint (DRUJ) instability and impaired wrist proprioception. Sensorimotor training of extensor carpi ulnaris (ECU) and pronator quadratus (PQ) can enhance DRUJ stability. With limited evidence on effectiveness of TFCC sensorimotor rehabilitation, this study aimed to evaluate the effects and feasibility of a novel wrist sensorimotor rehabilitation program (WSRP) for TFCC injuries. METHODS: Patients diagnosed with TFCC injuries were recruited from May 2018 to January 2020 at an outpatient hand c

Arthroscopic Treatment Results of Triangular Fibrocartilage Complex Tears in Adolescents: A Systematic Review.

INTRODUCTION: Injury to the triangular fibrocartilage complex (TFCC) may cause chronic wrist pain and instability if left untreated. The current literature of adult cases suggests that arthroscopic treatment offers favorable outcomes and is associated with a low complication rate. This systematic review evaluated the outcomes of arthroscopic TFCC surgery in adolescents. MATERIALS AND METHODS: A PRISMA-guided literature search of PubMed, Medline, Embase, Scopus, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, and Cochrane Clinical Answers was conducted

Outcomes of surgically treated distal radius fractures associated with triangular fibrocartilage complex injury.

STUDY DESIGN: Prospective cohort. INTRODUCTION: Clinical studies that evaluate the correlation between associated lesions of the triangular fibrocartilage complex (TFCC) and outcome of distal radius fractures expressed with the patient-rated disability are missing. The purpose of this study was to evaluate the outcomes of distal radius fractures associated with or without an injury of the TFCC. METHODS: Patients undergoing operative treatment for distal radius fracture were prospectively enrolled (n = 70). TFCC was examined by wrist arthroscopy and injuries classified according to Palmer. Arthroscopic one-tunnel transosseous foveal repair for triangular fibrocartilage complex (TFCC) peripheral tear.

PURPOSE: Arthroscopic repair of a peripheral triangular fibrocartilage complex (TFCC) tear is a promising, minimally invasive surgical technique, especially in patients with symptomatic distal radioulnar joint (DRUJ) instability. The purpose of this study was to evaluate the clinical result of arthroscopic one-tunnel transosseous foveal repair for peripheral TFCC tears. METHODS: Sixteen patients who underwent TFCC foveal repair were retrospectively evaluated. The mean follow-up period was 31.1 months. The torn TFCC of all patients was repaired with the arthroscopic one-tunnel transosseous fov

How does ulnar shortening osteotomy influence morphologic changes in the triangular fibrocartilage complex?

BACKGROUND: Ulnar shortening osteotomy often is indicated for treatment of injuries to the triangular fibrocartilage complex (TFCC). However, the effect of ulnar shortening osteotomy on the changes in shape of the TFCC is unclear. In our study, quantitative evaluations were performed using MRI to clarify the effect of ulnar shortening on triangular fibrocartilage (TFC) thickness attributable to disc regeneration of the TFC and TFC angle attributable to the suspension effect of ulnar shortening on the TFC. QUESTIONS/PURPOSES: The purposes of this study were (1) to compare preoperative and post

Arthroscopic treatment of peripheral triangular fibrocartilage complex tears with the deep fibers intact.

PURPOSE: We describe a variant of triangular fibrocartilage complex (TFCC) tears in which the superficial fibers attaching to the ulnar capsule are torn, with preservation of deep fibers inserting on the fovea. We present the clinical and magnetic resonance imaging findings and the results of arthroscopic repair in patients with this injury. METHODS: Twenty-nine wrists were treated arthroscopically for peripheral TFCC tears with outside-in suture repair of the TFCC to the ulnar capsule. A retrospective review of all cases was performed to assess the physical examination, magnetic resonance im

Sonographic Anatomy and Imaging of the Triangular Fibrocartilage Complex and the Distal Radio-Ulnar Joint.

INTRODUCTION: Ulnar-sided wrist pain is a common clinical problem, which can be difficult to diagnose, manage and treat. Ultrasound imaging can be used to identify structural changes which may contribute to the pain. Knowledge of the associated relative anatomy and intertwining structures which form the triangular fibrocartilage complex (TFCC) and distal radio-ulnar joint (DRUJ), sonographic techniques to image these structures and their normal and abnormal appearances can be underappreciated and are required. METHODS: A literature search was conducted to review the current knowledge related

Is the ulnar fovea sign positive only in foveal tears of the triangular fibrocartilage complex?

A positive ulnar fovea sign is usually considered to be specific for foveal disruption of the distal radioulnar ligaments. A retrospective review was done of 73 patients with a positive ulnar fovea sign. Only 21 patients had a foveal tear on arthroscopic examination.Level of evidence: IV.

Arthroscopic Examination of the Triangular Fibrocartilage Complex in Galeazzi Injuries: Revisiting a Complex Injury.

Galeazzi fracture-dislocations result from severe trauma and can lead to loss of function of the wrist and forearm if not treated appropriately. To avoid such disasters, meticulous surgical management of the injury is necessary. The current literature talks about symptomatic distal radioulnar joint injuries in Galeazzi fracture-dislocations and the various approaches to managing instability. However, unfortunately, there is a dearth of data on the true incidence of injury to the triangular fibrocartilage in the backdrop of the severity of the mechanism of Galeazzi fractures and the damage it m

Arthroscopic foveal repair with suture anchors for traumatic tears of the triangular fibrocartilage complex.

BACKGROUND: Foveal tears of the traumatic triangular fibrocartilage complex (TFCC) are the most commonly neglected high-energy injuries of the wrist joint, and the patients with such tears often experience unrecovered ulnar-sided wrist pain and poor wrist function. This study investigated the functional outcomes of patients who underwent arthroscopic repair of foveal TFCC tears with suture anchors and adjuvant platelet-rich plasma (PRP) injections after the surgery. METHODS: From September 2014 to August 2018, 156 men and 45 women with diagnoses of foveal TFCC tears without wrist fractures un

Palmar reconstruction of the triangular fibrocartilage complex for static instability of the distal radioulnar joint.

This study describes a new technique that can be used for reconstructing the triangular fibrocartilage complex to correct the static palmar radius instability of the distal radioulnar joint. In the abovementioned condition, the radius is extremely unstable with respect to the ulna and dislocates palmarly in the resting position. Using a palmar approach, a palmaris longus tendon graft was sutured to the remnant of the disrupted palmar radioulnar and ulnocarpal ligaments and then anchored to the bone tunnel that was created at the ulnar fovea. This technique predominantly reinforces the palmar s

The management of ulnocarpal abutment and degenerative triangular fibrocartilage complex tears in the competitive athlete.

Ulnar-sided wrist pain is a frequent cause for loss of practice time and competitive play for athletes. Ulnocarpal abutment, a common source of ulnar-sided pain, typically burdens athletes who participate in gymnastics, racket sports, and baseball. Although many athletes respond to nonoperative management, surgical intervention should be considered when symptoms persist. Surgical options include arthroscopic debridement, arthroscopic wafer, open wafer, or ulnar-shortening osteotomy. Treatment should be tailored to the athletes' level of function, expectations, and goals. The timing of interven

Incidence of extensor carpi ulnaris pathology in patients with triangular fibrocartilage complex foveal repairs.

We analysed the association between extensor carpi ulnaris pathology and triangular fibrocartilage complex foveal tears. In total, 178 patients (58 patients and 120 controls) were retrospectively recruited. The patient group comprised individuals with foveal tears. Extensor carpi ulnaris pathology was defined as the presence of tenosynovitis, tendinosis and high-grade tendon subluxation in magnetic resonance images. Tenosynovitis and a high grade of subluxation were more prevalent in patients than in controls (43 vs. 23% and 26 vs. 14%, respectively) whereas the incidence of tendinosis did not
