Whipworms are responsible for up to 500 million cases of trichuriasis worldwide, with higher endemicity in tropical and sub-tropical countries. In non-endemic countries, trichuriasis can be accidentally diagnosed upon colonoscopy, often in the presence of negative microscopy. Trichuris Dysentery Syndrome (TDS) is a severe persistent trichuriasis associated with heavy worm build-up in the colon that continues to be neglected and underestimated in endemic countries. We present a human infection with the canine whipworm, Trichuris vulpis, in a child suffering from rhinitis with a diagnosis of rhinitis. A community study on the age and sex related prevalence, intensity infection and frequency distribution of Ascaris, Trichuris and hookworm was carried out in 205 Orang Asli (Aborigines) children (95 boys, 110 girls) aged 1-13 years. Two hundred and forty-five children infected with Trichuris trichiura, 143 of whom were also infected with Ascaris lumbricoides, were treated with albendazole as follows: single doses of either 600 mg or 800 mg, or daily doses of 400 mg for either 3 d or 5 d. The whipworm (Trichuris trichiura) is an occasional finding in anorectal disease. All cases of anorectal discharge should include a test for ova and parasites. Trichuris infestations can be eliminated by the use of mebendazole (Vermox), a specific drug which has almost no side effects. It should be remembered that structural diseases of the anorectum, occurring during an infection such as fissure, fistula, and abscess, may persist after the infection has been cured and usually must be treated surgically.

Single doses of 100 mg, 200 mg, 400 mg, and 600 mg of mebendazole in the treatment of very light to heavy Trichuris infections all reduced egg output by over 80%. Children with severe Trichuris trichiura infection pose significant social, clinical, and therapeutic problems in Malaysia.
