We relate our own experience about bilateral inferior rectus recession as first surgery for traumatic bilateral superior oblique palsy. An adjustable suture is performed on one side. This procedure is combined or not with an inferior oblique recession. Measurements of ocular deviation and torsion, Lancaster tests and binocular single vision fields are reported pre- and postoperatively. Reduction of diplopia, cyclodeviation and V esotropia with an expansion of the binocular single vision field is achieved in all cases, relieving patients' complaints. This surgery is easy to perform and free of

Aberrant innervations of oculomotor muscles are a special group of clinical entities, the common sign of which is an abnormal innervation of the oculomotor muscles with nerve fibres which originally were not meant for this purpose. The majority of these anomalies develops during the embryonic period of lesions at the level of nuclei of cranial nerves or peripheral fibres. Congenital aberrations affect most frequently n. VI, less frequently n. III and rarely n. IV. Acquired affections are most frequent in n. III. Abnormal innervation connections concern not only oculomotor muscles but also musc

The ocular motor system finds, focuses, fixates and follows objects to ensure optimal vision as we move through our environment. To see clearly, images must be held steady on the retina. If images move across the retina at more than a few degrees per second they appear blurred. Thus, one function of eye movements is to hold images of the world steady on the retina, and this is accomplished by the gaze-holding mechanisms--fixation, vestibular and optokinetic. Visual acuity is best at the fovea, and so a second function of eye movements is to change the line of sight so that the image of an obje

This report documents an unusual phenomenon. A 6-year-old girl with trochlear-oculomotor synkinesis presented with superior oblique and palpebral levator co-contraction. The literature was reviewed and the possibility of classifying this entity as a congenital cranial dysinnervation disorder was speculated.

The patient with bacterial infection sequelae in the form of damage to cranial nerves III, IV, and VI was followed up. He had exhibited clinical and radiographic signs of paranasal sinusitis. Before his physiotherapy, the patient received standard treatment with natural and synthetic antibiotics and steroids. After acute signs of infection resolved without any functional improvement, the patient was referred to a rehabilitation unit to undergo neuromuscular re-education of the paralyzed extraocular muscles. Periorbital hydrocortisone iontophoresis and visuomotor exercises with intense ideomoto

Each year brings new scientific knowledge that builds on itself in a geometric fashion. Ocular motility basic and clinical neurosciences continue to advance with this accelerating pace. The years 1997 through 1998 brought new knowledge to the motility world. This review focuses on the clinical advances within this realm, presented in supranuclear to myopathic organization. Part II of this review will appear in the September 2000 (20:3) issue.

Strabismus and amblyopia are among the most common eye problems encountered in children. Proper assessment and management require a knowledge of the various types, methods of detection, and principles of treatment. The role of the pediatrician is emphasized because of the importance of early detection.

The study of the ocular-motor nerves must be exhaustive from their source (nuclei in the brainstem) down to the effector muscles (orbit). Visual disturbances have to be analysed by differentiating between a decrease in visual acuity and ocular-motor disorders. Imaging tests are dominated by MRI, including fine slices and gadolinium injection. A study of the Circle of Willis vessels is often useful, and essential in the case of type III impairment. A further CT scan is essential for analysis of the foramina, base of the skull and orbital walls. Impairment of CN VI requires a CT scan of the apex

BACKGROUND: Adjustable bilateral Harada-Ito procedures have been described, sometimes with asymmetric adjustment used to correct vertical misalignment when coexisting with torsional strabismus. We investigated the causes of significant postoperative torsional incomitance noted in some patients undergoing these procedures.
METHODS: The medical records of patients who underwent bilateral Harada-Ito procedures for bilateral trochlear nerve palsy between 1980 and 2018 were reviewed retrospectively. Cases with simultaneous operation on any other oblique or vertical rectus muscle were excluded. Sur

Neurocysticercosis is the most common disease of the nervous system caused by a parasite. Ophthalmological manifestations of cysticercosis typically are due to direct intraorbital or intraocular involvement, parenchymal brainstem involvement, or secondary to hydrocephalus or increased intracranial pressure. We describe a patient with a unilateral fourth nerve palsy, a rare presentation of neurocyticercosis.

Vertical strabismus can be associated with paretic or restrictive causes. Paretic causes may be due to a weak muscle or paresis, or to a total lack of muscle function caused by a third, fourth or sixth cranial nerve palsy. When examining a patient with vertical strabismus, it is paramount that we differentiate between a restricted cause and paretic cause. This paper discusses the nonsurgical techniques used in evaluating paretic vertical strabismus. There are no easy methods, and evaluation techniques can vary depending on the cooperation of the patient. It is essential to look at versions and

Palsies of cranial nerves are well-known complications after lumbar puncture. Sixth nerve palsies are the most common. They normally occur 4 to 14 days after the lumbar puncture and spontaneously recover in a few weeks or months. The occurrence of a fourth nerve palsy following lumbar puncture however is extremely rare. We report on a patient who developed a combined contralateral fourth and sixth nerve palsy after lumbar puncture (Syndrome of Intracranial Hypotension), requiring surgical correction for secondary diplopia.

A patient with a unique form of large granular lymphocytosis and multiple cranial neuritis is reported. The patient presented with facial weakness, diplopia and dysarthria. An increase in large granular lymphocytes (LGLs) was seen in blood (1.8 x 10(9)/l), CSF (237/microliters) and bone marrow (20% in a normocellular bone marrow). The phenotype of the LGLs in CSF, blood and bone marrow was CD2+ CD3+ CD4+ CD8- CD16- CD56- and CD57-. The unique features of this case include the CD4+ phenotype, the relative abundance of CSF LGL and the clinical presentation.

PURPOSE OF REVIEW: Describe the range of infectious causes of ocular motor neuropathies, from common presentations to unusual manifestations of diseases less frequently seen in the developed world. Provide information on recent developments in diagnostic testing for pathogens that may cause ocular motor neuropathies.
RECENT FINDINGS: Antigen detection in serum or CSF has improved the diagnosis of cryptococcal disease. Cartridge PCR testing for tuberculosis has increased diagnostic accuracy, though tuberculous meningitis remains difficult to diagnose. Rapid, multiplex PCR and unbiased sequenci

A 61-year-old man presented with 2 days of "flickering" vision. The symptom resolved with closure of the left eye. Examination demonstrated involuntary high-frequency, low-amplitude intorting movements of the left eye, consistent with superior oblique myokymia (Video 1). Ocular ductions were full, and there were no abnormal movements of the right eye.
