Central nervous system tuberculosis following delayed and initially missed lung miliary tuberculosis: a case report.

Central nervous system (CNS) tuberculosis includes three clinical entities: tuberculous meningitis, intracranial tuberculoma, and spinal tuberculous arachnoiditis. All three categories are encountered frequently in regions of the world where the incidence of TB is high. Meningeal tuberculosis is a medical emergency: it is the most severe, lethal and disabling form of tuberculosis. Early diagnosis and treatment can be lifesaving. Even, in developed countries the diagnosis of tuberculous meningitis is difficult, frequently delayed or missed, and is often not microbiologically confirmed. Here I r

Navigating complex presentations of central nervous system tuberculosis: A two-case analysis.

Tuberculosis (TB) remains a significant global health concern, and the COVID-19 pandemic has negatively impacted TB control. This report presents 2 challenging cases of central nervous system (CNS) tuberculosis with atypical presentation in adult patients. The first case involved a young male patient with disseminated tuberculosis who presented with meningoencephalitis, multiple brain abscesses, vasculitis, and extensive myelopathy, resulting in paraplegia. The patient required multiple neurosurgical interventions and experienced complications such as drug-induced hepatotoxicity and neurogenic

Multiple presentations of drug-resistant central nervous system tuberculosis: a case report.

INTRODUCTION AND IMPORTANCE: Central nervous system tuberculosis (CNS-TB) is a severe manifestation of Mycobacterium tuberculosis infection that can cause substantial morbidity and mortality. Tuberculous ventriculitis and intramedullary tuberculoma are two rare complications, and their coexistence poses significant diagnostic and therapeutic challenges. CASE PRESENTATION: A 19-year-old woman with a history of systemic lupus erythematosus, treated with prednisolone and methotrexate for the past 2 years, was diagnosed with isoniazid-resistant tuberculous meningitis and multiple intracranial tub

Imaging manifestations of pediatric central nervous system tuberculosis in an endemic region.

Tuberculosis of the central nervous system is a deadly disease with a high mortality and morbidity rate. While tuberculosis is prevalent worldwide, most active cases are found in underdeveloped and developing countries, including Pakistan, where it is highly endemic. The pediatric population is particularly at high risk of developing CNS tuberculosis. This disease can manifest in various forms, such as tuberculous meningitis, tuberculoma, tubercular abscess, Pott's spine, and tuberculous arachnoiditis. Given that the prognosis of CNS tuberculosis is closely tied to the stage at which treatment

Paradoxical Reactions of Central Nervous System Tuberculosis: Report of Three Immunocompetent Cases.

Introduction: Paradoxical reactions during tuberculosis (TBC) therapy are characterized by clinical or radiological worsening of preexisting tuberculous lesions or the appearance of new manifestations following appropriate TBC treatment. Identifying this phenomenon is crucial, since it can be mistaken with treatment failure or relapse. Although widely described in HIV patients following immune reconstitution inflammatory syndrome, the literature on HIV-negative patients is scarce. Case Series: We present three cases of immunocompetent patients with central nervous system tuberculosis (CNS-TBC)

Magnetic resonance imaging findings in central nervous system tuberculosis: A pictorial review.

Central nervous system (CNS) tuberculosis is a post-primary form of tuberculosis. It has high mortality and morbidity rates despite early diagnosis and treatment. CNS tuberculosis can manifest as subacute/chronic meningitis, parenchymal tuberculous lesions, and spinal tuberculosis. Hematogenous spread of tuberculous bacilli to the brain results in the development of so called "rich foci" on the pial surface, ependyma, and grey-white matter junction. Rupture of these "rich foci" into the subarachnoid space triggers an intense granulomatous inflammatory reaction. Tuberculous meningitis can manif

Bioinformation Analysis Reveals IFIT1 as Potential Biomarkers in Central Nervous System Tuberculosis.

OBJECTIVE: Central nervous system tuberculosis is the most serious form of extrapulmonary tuberculosis. We aim to discover potential biomarkers involved in the development of the disease. METHODS: Through gene difference analysis, construction of a protein interaction network and tissue specific analysis and other bioinformatics analysis methods, we found out the relatively high expression of important substances in the central nervous system, interferon induced protein with tetratricopeptide repeats 1. Subsequently, the lesion tissue and the resection margin tissue away from the lesion were

[Other Central Nervous System Infections(Including Fungal Infection, Tuberculosis, and Parasitic Infection)].

This review describes cryptococcal meningoencephalitis, tuberculous meningitis, neurosyphilis, and toxoplasma encephalitis. Central nervous system infections are neurological emergencies associated with mortality or other outcomes. Therefore, early diagnosis and treatment are critical. Fungal or gondii infections are rare and affect compromised hosts who are HIV positive, have diabetes, or take immunosuppressive or anticancer drugs. Cryptococcal antigens in the serum and cerebrospinal fluid are useful for the diagnosis of cryptococcal meningoencephalitis. RPR and TPHA tests are useful for the

Diagnostic Challenges and Prospects Associated With Zoonotic Tuberculosis of Central Nervous System.

INTRODUCTION: The diagnosis of Tuberculous Meningitis (TBM) has remained a challenge due to its insidious onset and the failure of conventional diagnostic tests. The present study aimed to identify the mycobacterial pathogen in the CSF of patients with TBM and a poor prognosis. METHODS: We retrospectively recruited 224 TBM and 34 non-TBM patients admitted to the Central India Institute of Medical Sciences, Nagpur, India, in 2014. The CSF samples of these patients were subjected to a duplex PCR assay for the species-specific identification of the causative pathogen. RESULTS: M. bovis and infe

Bedaquiline has potential for targeting tuberculosis reservoirs in the central nervous system.

Bedaquiline (BDQ) is the first-in-class United States Food and Drug Administration (US FDA) approved anti-tuberculosis (anti-TB) drug, which is a novel diarylquinoline antibiotic that has recently been utilized as an effective adjunct to existing therapies for multidrug-resistant tuberculosis (MDR-TB). BDQ is especially promising due to its novel mechanism of action, activity against drug-sensitive and drug-resistant tuberculosis (TB) in addition to having the potential to shorten treatment duration. Drug delivery to the central nervous system (CNS) is a major concern in TB chemotherapy, espec

Myeloid and T Cell-Derived TNF Protects against Central Nervous System Tuberculosis.

Tuberculosis of the central nervous system (CNS-TB) is a devastating complication of tuberculosis, and tumor necrosis factor (TNF) is crucial for innate immunity and controlling the infection. TNF is produced by many cell types upon activation, in particularly the myeloid and T cells during neuroinflammation. Here we used mice with TNF ablation targeted to myeloid and T cell (MT-TNF-/-) to assess the contribution of myeloid and T cell-derived TNF in immune responses during CNS-TB. These mice exhibited impaired innate immunity and high susceptibility to cerebral Mycobacterium tuberculosis infec

Surface-modified gatifloxacin nanoparticles with potential for treating central nervous system tuberculosis.

A new nanocarrier is developed for the passage of gatifloxacin through the blood-brain barrier to treat central nervous system tuberculosis. Gatifloxacin nanoparticles were prepared by nanoprecipitation using poly(lactic-co-glycolic acid) (PLGA) 502 and polysorbate 80 or Labrafil as surface modifiers. The evaluation of in vivo blood-brain barrier transport was carried out in male Wistar rats using rhodamine-loaded PLGA nanoparticles prepared with and without the surface modifiers. At 30 and 60 minutes after administration, nanoparticle biodistribution into the brain (hippocampus and cortex), l

Central nervous system Tuberculosis in a man from Cambodia with worsening headaches.

Central nervous system (CNS) tuberculosis should be considered in patients from endemic nations with worsening neurological symptoms. If imaging reveals possible CNS tuberculomas, potentially life-threatening lesions should be excised and analyzed. When disease is less severe, other tissues possibly infected should be biopsied first for diagnosis to avoid neurosurgery.

Central nervous system tuberculosis: an imaging-focused review of a reemerging disease.

Central nervous system (CNS) tuberculosis is a potentially life threatening condition which is curable if the correct diagnosis is made in the early stages. Its clinical and radiologic manifestations may mimic other infectious and noninfectious neurological conditions. Hence, familiarity with the imaging presentations of various forms of CNS tuberculosis is essential in timely diagnosis, and thereby reducing the morbidity and mortality of this disease. In this review, we describe the imaging characteristics of the different forms of CNS tuberculosis, including meningitis, tuberculoma, miliary

The dual face of central nervous system tuberculosis: a new Janus Bifrons?

Tuberculosis (TB) is still a common infectious disease in developing countries, but it is also re-emerging in industrialized nations due to the HIV/AIDS pandemic. In addition to bacillary virulence, the host immune response plays a major role in the development of an active disease (either as a primary infection or reactivation) and in controlling the infection. Even though several mechanisms are involved in regulating the human immune response, biological environment seems to be determinant. In this context, the integrated neuro-immune-endocrine system strongly influences TB clinical outcome.

Childhood tuberculosis in southern Taiwan, with emphasis on central nervous system complications.

BACKGROUND/PURPOSE: Childhood tuberculosis (TB) continues to be a major public health problem in Taiwan. Taiwan remains a highly endemic area despite neonatal Bacillus Calmette-Guérin (BCG) vaccination and the availability of anti-TB therapy. The presentation is highly variable and it is often difficult to make an accurate diagnosis. This study was designed to evaluate the demographic, clinical, and laboratory findings and outcomes of TB in children with emphasis on central nervous system (CNS) complications. METHODS: The medical records of 80 children diagnosed with TB at a medical center in

The PCR-Based Diagnosis of Central Nervous System Tuberculosis: Up to Date.

Central nervous system (CNS) tuberculosis, particularly tuberculous meningitis (TBM), is the severest form of Mycobacterium tuberculosis (M.Tb) infection, causing death or severe neurological defects in more than half of those affected, in spite of recent advancements in available anti-tuberculosis treatment. The definitive diagnosis of CNS tuberculosis depends upon the detection of M.Tb bacilli in the cerebrospinal fluid (CSF). At present, the diagnosis of CNS tuberculosis remains a complex issue because the most widely used conventional "gold standard" based on bacteriological detection meth

Unusual Periventricular Hemorrhage as the Initial Manifestation of Central Nervous System Tuberculosis.

BACKGROUND: Intracranial hemorrhage (ICH) is a rare complication of central nervous system (CNS) tuberculosis, and intratuberculoma hemorrhage is even more rare. To the best of our knowledge, periventricular hemorrhage caused by CNS tuberculosis has not yet been reported. In the present report, we have described the peculiar neuroradiological manifestations of periventricular hemorrhage secondary to CNS tuberculosis and discussed the possible mechanisms of intratuberculoma hemorrhage supported by the neuroradiological findings. CASE DESCRIPTION: We have reported the case of a 50-year-old man
