[Problems in the diagnosis of Addison's disease in the absence of hyperpigmentation].

At the example of a 62-year-old female patient with a "white" Addison as a sequela of a bilateral tuberculosis of the adrenal glands the problems of the diagnostics of this clinical picture are described. It is referred to the necessity of thinking of an existing chronic primary insufficiency of the adrenal cortex, also when there is no hyperpigmentation which otherwise serves as principle sign.

Bilateral adrenal enlargement in Addison's disease caused by tuberculosis. Nephrotomographic demonstration.

Nephrotomography showed markedly enlarged adrenal glands in a 67-year-old man with acute Addison's disease due to tuberculosis. Other causes of acute adrenal insufficiency associated with enlarged adrenals are histoplasmosis and, rarely, bilateral metastatic carcinoma, amyloidosis, or hemorrhage. Radiologic demonstration of large adrenal glands in a patient with acute Addision's disease should strongly suggest an infectious etiology.

Tuberculous granulomatous inflammation associated with adenoma of parathyroid gland manifesting as primary hyperparathyroidism.

A 36-year-old female presented with generalized bone pain, muscular weakness and enlarged cervical lymph nodes. The biochemical findings and skeletal survey was suggestive of primary hyperparathyroidism (PHPT). CT of neck and thorax showed enlarged multiple lymph nodes in the cervical and superior mediastinal region. With a diagnosis of PHPT she underwent cervical exploration and excision of enlarged right inferior parathyroid gland along with biopsy of nodes were done. Histopathology revealed the features of right parathyroid adenoma with few foci of epithelioid granuloma and granulomatous ly

Pulmonary tuberculosis and diabetes mellitus: Epidemiology, pathogenesis and therapeutic management (Review).

The dual burden of pulmonary tuberculosis (PTB) and diabetes mellitus (DM) is a major global public health concern. There is increasing evidence to indicate an association between PTB and DM. DM is associated with immune dysfunction and altered immune components. Hyperglycemia weakens the innate immune response by affecting the function of macrophages, dendritic cells, neutrophils, and natural killer cells, and also disrupts the adaptive immune response, thus promoting the susceptibility of PTB in patients with DM. Antituberculosis drugs often cause the impairment of liver and kidney function

A Rare Case of Primary Pituitary Tubercular Abscess Mimicking a Pituitary Adenoma.

Primary pituitary tuberculosis (PTA) is a very rare disease. The clinical diagnosis is difficult as it is clinically radiologically indistinguishable from other sellar lesions. We present a case of PTA without any predisposing etiology and radiologically mimicking a pituitary macroadenoma. The patient underwent endoscopic transsphenoidal resection where pus admixed with mucoid was seen coming out of the lesion intraoperatively. Histology of the cyst wall was suspective of tuberculous etiology and QuantiFERON Tb Gold done for confirmation was positive. Postoperatively patient showed obvious imp

Pancreatic tuberculosis: A systematic review of symptoms, diagnosis and treatment.

INTRODUCTION: Although pancreatic tuberculosis (TB) is traditionally considered to be a rare clinical entity, in recent times, an increase in the number of reports of pancreatic TB has been noted. We conducted a systematic review in order to summarise currently available data on pancreatic TB. METHODS: A comprehensive literature search of Medline, Scopus and ISI Web of Science databases was conducted in order to identify papers reporting cases of pancreatic TB. The eligibility criteria for inclusion in the review required that the studies reported patient(s) affected by pancreatic TB and that

CT findings of abdominal tuberculosis in 12 patients.

Our purpose was to evaluate the Computed Tomography (CT) findings of the abdominal tuberculosis (TBC) retrospectively which was diagnosed histopatologically. This study included 12 patients. All patients were evaluated by abdominal CT study. Most findings of CT studies were mesenteric calcified or noncalcified lymphadenopathies, ascites, thickened intestinal wall located on the right lower quadrant of abdomen, thickening of peritoneum, mottled soft-tissue densities in omentum and mesenterium. In addition, one of the patients had bilateral calcified adrenal glands and one of them had calcified

Bilateral adrenal masses: a single-centre experience.

BACKGROUND: Bilateral adrenal masses may have aetiologies like hyperplasia and infiltrative lesions, besides tumours. Hyperplastic and infiltrative lesions may have coexisting hypocortisolism. Bilateral tumours are likely to have hereditary/syndromic associations. The data on clinical profile of bilateral adrenal masses are limited. AIMS: To analyse clinical, biochemical and radiological features, and management outcomes in patients with bilateral adrenal masses. METHODS: Retrospective analysis of 70 patients with bilateral adrenal masses presenting to a single tertiary care endocrine centre

Tuberculosis complicating tertiary hyperparathyroidism in a patient with end-stage renal disease: a case report.

A 36-year-old Nigerian woman on thrice-weekly dialysis presented with symptoms and signs of hypercalcaemia. Laboratory findings were consistent with tertiary hyperparathyroidism. Parathyroid hormone levels remained elevated and she underwent elective parathyroidectomy. Intra-operatively all 4 parathyroid glands and local lymph nodes showed necrotising granulomas with occasional acid-fast bacilli, pathognomonic of tuberculosis (TB). Post-operatively she completed a full course of anti-TB therapy and at 9 months she experienced complete resolution in her plasma biochemistry and was essentially s

Tuberculous thyroiditis and miliary tuberculosis manifested postpartum in a patient with thyroid carcinoma.

Right nodular goiter with diffuse miliary shadow on chest roentgenogram was found in a postpartum febrile woman. Transbronchial lung biopsy revealed tuberculous granuloma and acid-fast bacilli were found by aspiration cytology of the thyroid. Although chemotherapy was effective, the thyroid nodule remained palpable and the serum thyroglobulin level remained high. Subtotal thyroidectomy revealed papillary carcinoma associated with tuberculosis and lymph nodes metastasis. This seems to be the first case report of a patient with tuberculous thyroiditis, coexisting with thyroid carcinoma, diagnose

Patterns of Mortality for Children and Adolescents in Early 20th-Century Alaska.

OBJECTIVES: Odds of death from specific causes in infants, children, and adolescents in Alaska have not been quantified with vital records. We analyze the odds of death from 12 causes in individuals ≤ 18 years. METHODS: We fit a multinomial logistic regression model (n = 12 904 death records) controlling for Indigeneity, age, sex, and year of death to predict the odds of specific causes of death. RESULTS: Alaska Native individuals were most likely to die from tuberculosis and respiratory infections, while non-Alaska Native individuals were most likely to die from early infant health problems

Frailty as a framework to understand tuberculosis severity, recovery and sequelae.

Tuberculosis (TB) causes profound systemic illness, and many survivors experience long-term sequelae associated with disability and premature mortality. Current TB severity indicators capture isolated aspects of disease and may not reflect the integrated physiological impact of infection on the host. Frailty, a multidimensional syndrome characterized by reduced physiological reserve and increased vulnerability to stressors, predicts adverse outcomes and guides clinical management in geriatrics. We propose frailty as an integrative framework for understanding the systemic effects of TB across t

COVID-19 Infection in Adrenal Tuberculosis Patients with Adrenal Insufficiency Who Complicated with Adrenal Crisis.

Adrenal insufficiency can increase the risk of infection. Respiratory infections play a role in the greater number of mortalities in patients with primary adrenal insufficiency. Severe acute illness elevates the risk of adrenal crisis which can give lethal outcome.A 52-year-old woman came to the emergency unit due to worsening gastrointestinal symptoms for the past 3 days. She had chronic epigastric pain, general weakness, weight loss, and skin hyperpigmentation. She was suspected of primary adrenal insufficiency one year ago, but she had poor compliance. In this current admission, she was sus

Thyroid tuberculosis.

Although the association of thyroid disorders with tuberculosis has been known for a long time, the diagnosis of thyroid tuberculosis is rare. Differential diagnosis can be very difficult without fine needle aspiration. The clinical course of the disease may resemble toxic goiter or acute thyroiditis or may follow a subacute or chronic pattern without specific symptomatology. We describe a 49-year old male patient with thyroid tuberculosis presenting as thyroid enlargement, fever, increased erythrocyte sedimentation rate, normal thyroid function tests, and a chest x-ray indicating the presence
