Psoas abscess in Bristol: a 10-year review.

A consecutive series of 16 cases of psoas abscess managed over a 10-year period at the Bristol Royal Infirmary is presented. Tuberculosis accounted for 4 patients all normally resident in the United Kingdom. Intraabdominal inflammatory disorders accounted for 9 of the cases with Crohn's disease being the commonest of these with 5 cases. The remaining patients comprised 3 with primary staphylococcal abscesses, one appendicitis, one diverticulitis and 2 with colonic carcinoma. Diagnostic delay was common. Ultrasonography together with guided aspiration of pus was the most useful investigation gi

Duodenal tuberculosis with a choledocho-duodenal fistula.

A 22-year-old man visited our hospital (National Cancer Center Hospital East) complaining of fatigue and anorexia. A laboratory investigation demonstrated a biochemical 'picture' of obstructive jaundice. An abdominal CT showed a low density mass in the retropancreatic area with multiple enlarged periportal lymph nodes. Upper gastrointestinal endoscopy revealed active ulceration on the dorsal wall of the descending part of the duodenum, and histopathology of the biopsy specimen revealed an ulcer with reactive inflammatory cell infiltration; no tumor cells were detected. The possibility of neopl

Hepatic tuberculosis: myriad of hues.

INTRODUCTION: Gastrointestinal tuberculosis is an important form of extrapulmonary tuberculosis but liver involvement is uncommon. Hepatic tuberculosis can manifest in variable manners including as a space occupying lesion, diffuse infiltration by granulomas (granulomatous hepatitis) and as liver abscess. Hepatic tuberculosis is usually associated with lesions elsewhere but the diagnosis is often not considered because of rarity. CASE REPORT: We report two cases of hepatic involvement by tuberculosis: one in the form of liver abscess and the other having multiple hypodense hepatic lesions alo

Pulmonary tuberculosis and renal amyloidosis: A rare entity in children.

Amyloidosis is a group of disorders with extracellular accumulation of autologous fibrillary insoluble proteins in various tissues and organs such as the kidneys, liver, spleen, heart, and gastrointestinal tract leading to impaired function. Renal amyloidosis is usually considered to be a progressive disease manifesting as nephrotic syndrome and renal insufficiency with tuberculosis being one of the most common causes in adults. However, the occurrence of similar associations is rarely described in children. We report a case of an 11-year old girl who presented with nephrotic syndrome and pulm

Diagnostic delay in a multi-organ tuberculosis immunocompetent patient: a case report.

A 67-year-old immunocompetent male presented with intermittent fever for 3 months associated with urinary incontinence, altered bowel habits and history of loss of appetite and weight. He was treated as having enteric fever at various clinics in the city by different physicians. On evaluation the patient was found to have disseminated tuberculosis with involvement of the lungs, eyes, testes, brain, bone, kidneys, liver, spleen and possibly the gastrointestinal tract. This paper reports a case of disseminated tuberculosis to many organs with significant diagnostic delay more than twelve decades

Idiopathic granulomatous gastritis resembling a gastrointestinal stromal tumor.

A 41-year-old female presented with a 2 cm gastric submucosal tumor that was suspected to be a gastrointestinal stromal tumor or other malignancy, and local resection of the stomach was performed. However, histopathological examination showed granulomatous gastritis (GG) with a variety of chronic inflammatory cells and multinodular granulomas. Although she had a past history of tuberculosis and advanced breast cancer after surgery, there was no apparent evidence of either tuberculosis or a metastatic tumor. Other causes of GG, such as mycosis, syphilis, sarcoidosis or foreign body reaction wer

Colonofiberscopic diagnosis of intestinal tuberculosis.

The purpose of this study was to assess the predictive value of colonofiberscopy in the diagnosis of intestinal tuberculosis. The records of more than 14,000 colonofiberscopic examinations performed at the National Taiwan University Hospital from 1971 to 1995 were studied retrospectively. Colonofiberscopic diagnosis of intestinal tuberculosis required at least one of the following: circumferential ulcers, a single transverse axis ulcer with uneven base and elevated or nodular margin surrounded by nodular and hyperemic mucosa, or pseudopolyps. Sixty cases were diagnosed as intestinal tuberculos

Myocardial tuberculosis: a diagnostic challenge illustrated by two cases and review of contemporary management.

Myocardial tuberculosis is a rare form of extrapulmonary tuberculosis, characterized by variable clinical presentations and a potentially misleading diagnosis. We report two cases of myocardial tuberculosis: one case of disseminated disease involving the brain, lungs, lymph nodes, gastrointestinal tract, skin, and myocardium, and another case of isolated myocardial involvement. Both patients were treated with first-line anti-tuberculosis treatment. In addition, one patient received corticosteroids and therapeutic anticoagulation therapy. Clinical and radiological outcomes were favorable in bot

Disseminated Tuberculosis Mimicking as Crohn's Disease in a Paediatric Patient.

Tuberculosis is an important infectious disease for children worldwide. The clinical presentation of tuberculosis in children is diverse and, depending on the affected organs, it is often accompanied with nonspecific symptoms that can mimic other diseases. In this report, we present a case of disseminated tuberculosis in an 11-year-old boy with intestinal followed by pulmonary involvement. The diagnosis was delayed for several weeks due to the clinical picture which was mimicking Crohn's disease, the known difficulties in diagnostic tests and the improvement on meropenem. This case demonstrate

Anal tuberculosis: A non-Healing anal lesion.

Anal tuberculosis is an extremely rare extrapulmonary presentation of tuberculosis (TB). Less than 1% of the individuals who contract TB manifests as gastrointestinal TB, and anoperineal TB is much less frequently encountered, 1% of the TB cases of the digestive tract. A rare case of anal tuberculosis is reported in a 37-year-old male patient with a recent anal fistula surgery and relapsing anal lesions. AFB were detected by biopsy and culture. In total, the treatment course lasted 6 months and the patient showed signs of recovery in the early stages of the treatment (after 2 weeks), and compl

Cecal Perforation Following Intraperitoneal Abscess after Anti-tubercular Therapy: A Case Report.

UNLABELLED: Abdominal tuberculosis is defined as infection of gastrointestinal tract, peritoneum, abdominal solid organs, and/or abdominal lymphatics constituting approximately 12% of extra-pulmonary tuberculosis cases. Intestinal perforation is an acute presentation of abdominal tuberculosis. Intestinal perforation can occur before or at the beginning of anti-tubercular therapy. It is considered to be a paradoxical reaction if it occurs during or after treatment. Intestinal perforation is uncommon but serious and life-threatening as complication-mortality rate secondary to perforation are est

Resurgence of active tuberculosis among pregnant women.

OBJECTIVE: To determine the frequency of active tuberculosis during pregnancy in two hospitals located in an area where tuberculosis is epidemic and to describe its course and association with human immunodeficiency virus (HIV) infection. METHODS: We reviewed and analyzed the medical records of 16 pregnant women diagnosed with tuberculosis between 1985-1992 at Kings County Hospital (n = 12) and Saint Vincent's Hospital (n = 4) in New York City. RESULTS: Ten of the 16 pregnant women with proven active tuberculosis had pulmonary tuberculosis, two had tuberculous meningitis, and one each had me

Multimodal Diagnosis and Management of Gastric Tuberculosis: A Case Report.

Gastric tuberculosis is an exceptionally rare form of extrapulmonary tuberculosis and often presents with non-specific symptoms and endoscopic appearances, frequently mimicking submucosal tumors, peptic ulcer disease, or malignancy. These overlaps, together with the limited yield of superficial biopsies, can delay definitive diagnosis. Here, researchers report a case of gastric tuberculosis and emphasize the methodological advantage of a multimodal diagnostic strategy that integrates endoscopy, cross-sectional imaging, histopathology, and molecular testing to progressively narrow diagnostic un

Tuberculosis from head to toe.

Tuberculosis can affect virtually any organ system in the body and can be devastating if left untreated. The increasing prevalence of tuberculosis in both immunocompetent and immunocompromised individuals in recent years makes this disease a topic of universal concern. Because tuberculosis demonstrates a variety of clinical and radiologic findings and has a known propensity for dissemination from its primary site, it can mimic numerous other disease entities. Primary pulmonary tuberculosis typically manifests radiologically as parenchymal disease, lymphadenopathy, pleural effusion, miliary dis
