IMPORTANCE: Tympanic membrane perforations (TMPs) are frequent events leading to evaluation in the primary care and otolaryngology offices or the emergency department (ED). Despite specific warning labels on packaging of cotton-tipped applicators regarding the risk of injury to the ear canal with personal use, these products are commonly used to remove ear cerumen.
OBJECTIVE: To analyze the mechanism of injury for traumatic TMPs among patients presenting to the ED.
DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional analysis of cases from 100 emergency departments in the United States. The Na

BACKGROUND: The use of cotton swabs is well known as a cause of a traumatic membrane perforation. More reasons are known. A detailed description of the aetiology and therapy in Germany does not exist. The treatment consist of a readaptation of the perforation an splinting of the tympanic membrane with paper sheets or silicon foils.
MATERIALS AND METHODS: 238 patients with an acute tympanic membrane perforation were included in this retrospective study. They were treated between 2005 and 2010.
RESULTS: The mean causes of a traumatic tympanic membrane perforation were slaps with the open hand

Our objectives were to review all reported staging systems of tympanic membrane (TM) retraction pockets (RP) and to report their reliability and utility to our daily clinical practice in terms of follow-up and decision making in the management of RP. We aim to propose a new management algorithm of TMRPs. We conducted a thorough research on Ovid Medline, Pubmed and Cochrane databases for English and French languages studies published between 1963 and 2012 on the retraction pocket. Studies were excluded if it were a short comments, photo clinical cases, experimental studies or round table articl

OBJECTIVE: To assess different clinical scales of TM perforation size; to evaluate the effect of the size and the site of a perforation on the hearing level and frequencies.
METHODS: Prospective study. Observers had subjectively estimated the size in millimeter and in percentage of a particular perforation; objectively computerized measures of TM perforations area were analyzed. Agreement between different measures was studied. Cases with postoperative intact TM and an air-bone gap (ABG) of 10 dB or less were studied.
RESULTS: Global mean preoperative ABG was 21.8 ± 17 dB. Preoperative ABG w

We reviewed 165 cases of traumatic tympanic membrane perforation treated in the last 9 years (2000-2008). Of these, 103 sustained direct injury and 62 indirect injury. Ear picking accounted for 90.3% of direct injuries. Perforation size followed the classification of Yoshikawa, with Grade I perforation the most common, according for 129 (78.2%). The anteroinferior quadrant perforation site was the most common, with 98 (59.4%). Of the 165, 66 were lost during followup. Of the remaining 99, perforations closed spontaneously in 85 (85.9%), within a mean 25.9 days. Tympanoplasty was done in 14, in

CONTEXT: The knowledge of variations of the tympanic membrane (TM) perforations with the climatic changes in the West African subregion would help clinicians in its prevention and management.
OBJECTIVE: To analyze the pattern of clinical presentations and associated features of TM perforation in adults in West Africa.
DESIGN: A prospective study.
SETTING: Tertiary referral centre, University hospital.
PATIENTS OR OTHER PARTICIPANTS: Thirty-five (35) consecutive adults with TM perforations during a 1-year period had clinical evaluation of each TM using head mirror, video otoscopy, and micro

OBJECTIVES: We studied the experimental use of Seprafilm, a sheet-like film form of hyaluronic acid, in the repair of subtotal perforations of the tympanic membrane (TM), and the durability of such repairs.
METHOD: The research was conducted on 42 Sprague-Dawley rats. Right TMs of all rats were subtotally and thermally perforated in the same fashion. Seprafilm was applied to the perforated TMs of the 21 rats comprising the experiment group. The perforated TMs of the remaining 21 rats, the control group, were left to heal spontaneously.
RESULTS: The mean healing times were recorded as 7.8 +/-

Development of the eardrum retraction pocket, as pathologic finding, depends on Eustachian tube dysfunction, onset of the middle ear infection and site of development of retraction on the eardrum. The study is aimed at: 1. Determining the incidence of eardrum retraction pocket and cholesteatoma within it, as well as at the degree of eardrum retraction; 2. Determining of association between eardrum retraction pocket and changes of the eardrum mucosa and pars tensa of the tympanic membrane; 3. Determining of onset and intensity of the bone destruction in eardrum retraction pocket; 4. Examining o

This article discusses how several diagnostic tools used by audiologists inter-relate to anatomical abnormalities viewed by video otoscopy. Background of common middle ear pathology is reviewed, with emphasis on new research findings regarding pathophysiology. A review of video otoscopy, multifrequency tympanometry, and otoreflectance is provided. Case studies illustrate the integrated use of these diagnostic tools. Audiologic results are integrated with video otoscopy through case study analysis and interpretation. Understanding of the complexity of the physiologic and behavioral measurements

The stiffness properties of the gerbil tympanic membrane (TM) were investigated during the early course of experimental purulent otitis media produced by middle ear inoculation (n = 49) with type 3 Streptococcus pneumoniae. In a first approach the acoustic admittance and susceptance were measured in vitro with tympanometry and in a second approach the tympanic membrane displacement in response to static pressure was measured with moire interferometry. A histological assessment of the tympanic membrane was made concurrently. The acoustic admittance and susceptance reduced during the initial day

Fifty cases of dry, central, tympanic membrane perforation were studied for closure by application of saturated solution of trichloracetic acid. Aetiologically 17 cases belonged to traumatic and 33 to the inflammatory group. Eustachian tube patency and good air-bone gap on audiometry were the pre-requisites. Study concluded with 78% success with average of 2.8 applications. The patients had definite improvement in the hearing acuity and relief from other symptoms. The study highlights the achievements by using this old method.

The effects of topically applied 1% sodium hyaluronate as an alternative treatment to surgery for tympanic membrane perforations was studied. In an open study, the inclusion criteria were increased by accepting patients previously treated by surgery or with placebo and, compared with a previous study, the frequency at which sodium hyaluronate was applied was reduced. Reduction in the size of the perforation was detected in 12/16 (75.0%) patients, six (37.5%) of whom showed complete tympanic healing, and in the remaining four (25.0%) there was no healing. The average percentage reduction in the

Previous investigations into the healing and reconstruction of tympanic membrane (TM) perforations have involved animal models with acute TM perforations. A problem with the acute TM perforation model is that most acute TM perforations will heal spontaneously, both in animals and human beings. A second inadequacy of acute perforation models is that they are not analogous to the salient problem in human beings: long-standing TM perforation. The ideal animal model must have a TM perforation that is permanent, well-epithelialized, and free from infection. The perforation must also be subtotal to

Complications originating from the petrous bone may occur in the absence of any tympanic membrane pathology. This is known from a couple of cases in the literature and is underlined by two own case reports. Today, computed tomography and/or magnetic resonance imaging play a major role in the diagnosis of such rare but dangerous diseases.

Three large tympanic membrane (TM) perforations (occupying more than one quadrant) were treated with hyaluronan (1% hyaluronic acid) without delay after the accident. Using the highly viscous hyaluronan as an aid, the margins of the perforations could be noticeably restored. The size of the perforation could be immediately reduced to roughly half of its original size. Immediate restoration and covering of a traumatically ruptured TM wound improve the healing potential of the drum and counteract middle ear infection.
