ALS--decision making under uncertainty: a positive approach.

Amyotrophic lateral sclerosis (ALS) is a progressive motor neuron disease. It is characterized by feelings of uncertainty and predictable crises points where difficult decisions have to be made by both the client and the family. These crises points involve progressive and sometimes sudden impairment in mobility, communication, nutrition and ultimately, respiratory function. This paper will use case analysis to illustrate the decisions that have to be made at each crisis point. A model will be presented that will focus on the nursing assessment of decision making behaviours of families experien

Optimizing vaccination strategies under uncertainty to prevent epidemics.

To be prepared for future infectious disease epidemics, this paper considers a data-driven approach for determining optimal vaccination strategies for multi-community settings with heterogeneous populations under uncertain social mixing, disease transmission, and vaccine efficacy. Specifically, we derive an integrated chance constraints stochastic programming (ICC-SP) disease spread model for optimal vaccination strategies to prevent epidemics by keeping the uncertain post-vaccination reproduction number below one at a specified level of risk. A vaccination strategy specifies the proportion of

Shame among medical learners: An uncertainty-focused conception.

OBJECTIVE: To analyze the nature and etiology of shame, a common problem that diminishes the well-being of learners and clinicians in medical education and practice, and to explore the relationship between shame and medical uncertainty. DISCUSSION: We draw upon various theoretical insights on both shame and uncertainty, and argue that shame is ultimately a product of uncertainty and its transformation by medical learners. We argue that this transformation involves two key processes. The first is a personalization of uncertainty-i.e., a transformation of medical uncertainties focused on clinic

Human exploration strategically balances approaching and avoiding uncertainty.

A central purpose of exploration is to reduce goal-relevant uncertainty. Consequently, individuals often explore by focusing on areas of uncertainty in the environment. However, people sometimes adopt the opposite strategy, one of avoiding uncertainty. How are the conflicting tendencies to approach and avoid uncertainty reconciled in human exploration? We hypothesized that the balance between avoiding and approaching uncertainty can be understood by considering capacity constraints. Accordingly, people are expected to approach uncertainty in most cases, but to avoid it when overall uncertainty

Uncertainty aware stochastic sampling for efficient object detection.

The effectiveness of deep learning models is strongly influenced by the quality of training data. Traditional training approaches assume that all samples contribute equally to the learning process, leading to uniform data sampling. However, this assumption overlooks the substantial variation in informational content across samples. This paper introduces a novel and computationally lightweight data prioritization methodology for object detection that dynamically adjusts the sampling probability of training data according to its relevance during learning. The proposed Relative Detection Error (RD

Anticoagulation in Advanced Chronic Kidney Disease: Navigating Uncertainty.

Advanced chronic kidney disease (CKD) is characterized by a complex hemostatic state in which thrombotic and bleeding risks coexist. As kidney function declines, balancing thrombotic protection against bleeding risk becomes increasingly challenging. Although atrial fibrillation is common and associated with a high stroke risk in advanced CKD, the net clinical benefit of anticoagulation remains uncertain because this population has been largely excluded from pivotal randomized trials. Existing risk scores incompletely capture CKD-specific and dynamic determinants of both thrombosis and bleeding

Quantifying uncertainty of predictions from cancer progression models.

MOTIVATION: Cancer progresses through the accumulation of genomic events. Cancer progression models such as Mutual Hazard Networks (MHNs) describe this dynamic, enabling prediction of temporal event positions and patient-specific risks of acquiring mutations. However, current MHN analyses rely on single most likely models and do not quantify the uncertainty inherent to parameter estimation. Assessing forecast stability is essential before using them to anticipate treatment-relevant mutations, adapt targeted therapies, or prioritize monitoring of patients at elevated progression risk. RESULTS:

Impact of temporal uncertainty on sign-tracking behavior.

Sign-tracking behavior, also known as "autoshaping," is defined as the approach and interaction with reward-predictive cues. It is associated with addiction-related phenotypes and compulsive behavior. Several previous studies have demonstrated that when there is uncertainty about reward properties (e.g., probability and magnitude), sign tracking is increased. However, the effect of cue uncertainty on sign-tracking behavior is not known. Here, using a Pavlovian conditioning paradigm, we held the duration of cues constant and manipulated the temporal uncertainty by implementing either fixed or v

Uncertainty-aware image classification on 3D CT lung.

Early detection is crucial for lung cancer to prolong the patient's survival. Existing model architectures used in such systems have shown promising results. However, they lack reliability and robustness in their predictions and the models are typically evaluated on a single dataset, making them overconfident when a new class is present. With the existence of uncertainty, uncertain images can be referred to medical experts for a second opinion. Thus, we propose an uncertainty-aware framework that includes three phases: data preprocessing and model selection and evaluation, uncertainty quantifi

Predictive uncertainty in state-estimation drives active sensing.

Animals use active sensing movements to shape the spatiotemporal characteristics of sensory signals to better perceive their environment under varying conditions. However, the underlying mechanisms governing the generation of active sensing movements are not known. To address this, we investigated the role of active sensing movements in the refuge tracking behavior ofEigenmannia virescens, a species of weakly electric fish. These fish track the longitudinal movements of a refuge in which they hide by swimming back and forth in a single linear dimension. During refuge tracking,Eigenmanniaexhibi

Leveraging anatomical constraints with uncertainty for pneumothorax segmentation.

BACKGROUND: Pneumothorax is a medical emergency caused by the abnormal accumulation of air in the pleural space-the potential space between the lungs and chest wall. On 2D chest radiographs, pneumothorax occurs within the thoracic cavity and outside of the mediastinum, and we refer to this area as "lung + space." While deep learning (DL) has increasingly been utilized to segment pneumothorax lesions in chest radiographs, many existing DL models employ an end-to-end approach. These models directly map chest radiographs to clinician-annotated lesion areas, often neglecting the vital domain knowl

Angle-Only Cooperative Orbit Determination Considering Attitude Uncertainty.

In this paper, a novel concept for cooperative orbit determination (OD) using inter-spacecraft angle-only measurements is proposed. Different from the conventional cooperative OD that only estimates orbit states, the attitude of the observer spacecraft is considered by incorporating the attitude into the estimated vector. The observability of a two-spacecraft system is analyzed based on the observability matrix. Observability analysis reveals that inter-spacecraft angle-only measurements are inadequate to estimate both the attitude and the orbit states in two-body dynamics. The observability o

Learned uncertainty: The free energy principle in anxiety.

Generalized anxiety disorder is among the world's most prevalent psychiatric disorders and often manifests as persistent and difficult to control apprehension. Despite its prevalence, there is no integrative, formal model of how anxiety and anxiety disorders arise. Here, we offer a perspective derived from the free energy principle; one that shares similarities with established constructs such as learned helplessness. Our account is simple: anxiety can be formalized as learned uncertainty. A biological system, having had persistent uncertainty in its past, will expect uncertainty in its future

Ambiguity and conflict: Dissecting uncertainty in decision-making.

Making decisions is fundamental to how we navigate, survive, and thrive in our environment. The quality of information used to support decisions is rarely perfect. Many decisions are made under conditions of uncertainty, arising from ambiguous or conflicting information. Conflict and ambiguity, though conceptually distinct, both generate uncertainty, a commonality that has led to overlapping and inconsistent terminology in the literature. Evidence from human and animal research suggests a behavioral dissociation in responding to conflict and ambiguity. This dissociation can be studied through

Intolerance of uncertainty and obsessive-compulsive personality disorder.

Obsessive-compulsive personality disorder (OCPD) remains understudied despite its high community prevalence and substantial societal burden. Elucidating the factors involved in OCPD traits and behaviors could improve conceptual models of OCPD and guide treatment development. Intolerance of uncertainty (IU) has been investigated as a transdiagnostic factor linked to many conditions, including anxiety disorders and obsessive-compulsive disorder (OCD). Despite OCPD's links to OCD, very little research has investigated whether IU may also be a relevant factor in OCPD. The present study administere
