Electrical injuries of the upper extremity.

Although electrical burns of the upper extremity comprise a small percentage of all thermal injuries, they present formidable clinical problems. The physician must not be lulled into a false sense of security because the visible cutaneous burn is not great. Sound surgical principles, such as the control of bacterial proliferation with topical chemotherapy, débridement of dead tissue, and timely wound closure, in conjunction with the expertise necessary in caring for thermally injured patients, are necessary for a maximally rehabilitated patient.

Infection in the burned upper extremity.

Infection invariably accompanies thermal injury. The degree to which a patient is jeopardized by infection is related to the size and depth of the burn, the density and virulence of the microorganisms colonizing the burn wound, and the competence of his immune defenses. The aim of topical therapy is to limit microbial colonization of the burn wound to levels below those associated with invasive infection of the viable tissue beneath the eschar. The use of effective topical and systemic antimicrobial agents has been associated with the emergence of other bacterial, fungal, and viral infections

Deciding on thresholds for clinical relevance of PROMIS-upper extremity v2.0 in patients with upper extremity injury.

PURPOSE: In this study we focused on gaining better interpretation of the PROMIS-Upper-Extremity v2.0 (PROMIS-UE v2.0) T-score scale for measuring upper extremity function in patients with upper extremity injuries (UEIs). This is fundamental in order to clearly understand the clinical relevance of these outcomes. The aim of this study was to identify interpretive thresholds for mild, moderate and severe upper extremity function problems for PROMIS-UE v2.0 T-scores, in patients with UEIs, by performing a vignettes study. METHODS: A qualitative vignette study, based on the PROMIS-UE v2.0 and it

Portable upper extremity robotics is as efficacious as upper extremity rehabilitative therapy: a randomized controlled pilot trial.

OBJECTIVE: To compare the efficacy of a repetitive task-specific practice regimen integrating a portable, electromyography-controlled brace called the 'Myomo' versus usual care repetitive task-specific practice in subjects with chronic, moderate upper extremity impairment. SUBJECTS: Sixteen subjects (7 males; mean age 57.0 ± 11.02 years; mean time post stroke 75.0 ± 87.63 months; 5 left-sided strokes) exhibiting chronic, stable, moderate upper extremity impairment. INTERVENTIONS: Subjects were administered repetitive task-specific practice in which they participated in valued, functional tas

Recurrent upper extremity embolism due to a crutch-induced arterial injury: a different cause of upper extremity embolism.

Acute embolism of the upper extremity is a relatively infrequent event compared to the lower extremity, but it will affect the function of the limb involved and may occasionally lead to amputation. Most upper extremity emboli are of cardiac origin, with the remainder arising from subclavian aneurysm, occlusive disease, or iatrogenic causes. Rarely, crutch-induced repetitive trauma of an upper extremity can produce recurrent embolic events. Frequently, this process is initially diagnosed and treated as a brachial artery embolism; such a misdiagnosis is associated with recurrent embolism. We rep

Investigation of the relationship between upper extremity proprioception and upper extremity functions in children with Duchenne muscular dystrophy.

BACKGROUND AND OBJECTIVE: Upper-extremity proprioception contributes to coordinated reaching and grasping, which are critical for functional independence; however, evidence on upper-limb proprioception in children with Duchenne muscular dystrophy (DMD) remains limited. Therefore, we aimed to compare wrist proprioception in children with DMD versus healthy peers and to examine its relationship with upper-limb function. METHODS: Cross-sectional observational study conducted at a pediatric neuromuscular clinic, enrolling children aged 6-15 years who were able to sit independently. Wrist proprioc

Upper Extremity Trauma Resulting From Agricultural Accidents: Mechanism and Severity for Patients With and Without Upper Extremity Injury.

BACKGROUND: Farming remains the most dangerous occupation in the United States and upper extremity (UE) injuries occur frequently in agricultural accidents. The purpose of this study is to describe the injury mechanisms, severity, and health care costs of UE injuries resulting from agricultural accidents and to compare patients with and without injuries to the UE. METHODS: We performed a 6-year retrospective review of our level I trauma center registry from January 2006 to May 2013, identifying all patients injured in an agricultural accident. Data collection included baseline demographics, i

Upper Extremity Pulse Pressure Predicts Amputation-Free Survival after Lower Extremity Bypass.

Increased pulse pressure reflects pathologic arterial stiffening and predicts cardiovascular events and mortality. The effect of pulse pressure on outcomes in lower extremity bypass patients remains unknown. We thus investigated whether preoperative pulse pressure could predict amputation-free survival in patients undergoing lower extremity bypass for atherosclerotic occlusive disease. An institutional database identified 240 included patients undergoing lower extremity bypass from 2005 to 2014. Preoperative demographics, cardiovascular risk factors, operative factors, and systolic and diastol

Lower extremity lipedema, upper extremity lipodystrophy and severe calcinosis complicating juvenile dermatomyositis.

Juvenile dermatomyositis (JDM) is a rare but complex and potentially life-threatening autoimmune disease of childhood. Significant proportions of patients have residual weakness, muscle atrophy, joint contractures, and calcinosis. Recently, new clinical findings, such as lipodystrophy accompanied with increased fat deposition in certain areas, have been reported. So far, it is not known whether the redistribution of body fat may be the type of lipedema of lower extremity. We describe a 39-year-old woman who was diagnosed with JDM at the age of 7. Later she developed symmetrical lipodystrophy o

Management of Snakebites in the Upper Extremity.

Encounters with venomous snakes can lead to substantial morbidity or mortality if managed inappropriately. Medical management is the mainstay of treatment, but surgery may be necessary in rare cases. The hand surgeon should be well versed in the types of venomous snakes, the mechanism of action of venom, and the management of these injuries, given the frequency of hand envenomation. The indications for surgery are not well established and rely on clinical judgment and practitioner's experience. An understanding of previously reported outcomes and an algorithmic approach to treatment will help

Refractures of the upper extremity in children.

PURPOSE: To investigate the etiologic factors related to refractures of the upper extremity in children. PATIENTS AND METHODS: Eighteen refractures were divided into three groups according to the location of the initial fractures. They were analyzed in terms of the type of refractures, fracture patterns, and the existence of an underlying deformity. RESULTS: Of nine supracondylar fractures of the humerus, two involved refractures at the supracondylar region, and the other seven involved the lateral condyle. Underlying cubitus varus was present in six cases. Of three lateral condylar fracture

Arterial variations of the upper extremities.

During the dissection of the right and left upper limbs of a 70-year-old female cadaver, we encountered combined vascular anomalies. On the left side, we observed the presence of a lateral inferior superficial brachial artery, large anterior interosseous artery and an absence of a radial artery, which is quite a rare anomaly. The lateral inferior superficial brachial artery that arose 39 mm distal to the brachial artery descended to the wrist. The anterior interosseous artery originated from the brachial artery at the level of 12 mm distal to the head of the radius and descended on the anterio

Arterial reconstruction in the upper extremities.

Arterial reconstruction in the upper extremities is rare relative to the incidence of reconstruction in the lower extremities. Twenty-three patients who underwent vascular reconstruction in the upper extremities from 1985 to 2000 were retrospectively reviewed. Atherosclerosis was observed in only two subclavian arteries. The most common cause of occlusion was thromboembolism from the heart, which occurred often in the brachial artery and could be treated successfully with thrombectomy. Seven of eight bypass grafts, including three procedures below the elbow, remained patent during follow-up pe

Entrapment neuropathies of the upper extremities.

The three syndromes described in this article were chosen from about a dozen nerve-compression disorders that affect the arm from scapula to digits. They illustrate the range of problems encountered. None of the other syndromes are as common, and in some instances (pronator syndrome and anterior interosseous syndrome), localized inflammation is at least as common a cause of neuropathy as is compression. Some syndromes of nerve compression have important implications for musicians, athletes, and those who place stress on the upper limbs through heavy or repeated use. Well-designed prospective t

Evaluation of impairment in the upper extremity.

A system for evaluation of physical impairment in the hand and upper extremity was developed and has been tested and used by many hand surgeons around the world. It was approved for international application by the International Federation of Societies for Surgery of the Hand. A method for evaluating amputation and sensory and motion impairments has been devised for the hand and upper extremity. Amputation impairment percentage values were determined for each digit or position thereof for the hand, wrist, elbow, and shoulder. By a principle of progressive multiplication of percentage values, i

Arterial injuries of the upper extremities.

Twenty-five consecutive treated patients with traumatic lesions of upper extremity arteries are presented. All were males, and most were young and able-bodied. Simultaneous fracture and/or dislocation of bone was present in 17 of the 25 patients, nerve lesions in 14 and venous trauma in 7 patients. Serious injuries to other organs had occurred in 8 patients, 3 of whom had multi-trauma. Three patients died, 2 of them from injuries to other organs. Seven others underwent amputation of the limb. No amputation was necessitated by the arterial lesion only. The major causes of injury were traffic ac
