Transient apnoea for motion control during flexible ureteroscopic lithotripsy: a randomised trial.

OBJECTIVES: To determine whether transient apnoea (TA) during flexible ureteroscopic laser lithotripsy improves active lithotripsy efficiency compared with regular mechanical ventilation (RMV) and small tidal-volume ventilation (STVV). PATIENTS AND METHODS: In this single-centre, three-arm randomised trial, 150 adults with a single computed tomography (CT)-confirmed renal, ureteropelvic, or proximal ureteric stone measuring 10-<20 mm were assigned one-to-one-to-one to RMV, STVV, or TA. TA followed 5-min 100% oxygen pre-oxygenation, with 9-min maximum and pre-specified stopping criteria under

Exploring ureteroscope design with computational fluid dynamics for improved intra-pelvic pressure.

High intra-pelvic pressure (IPP) during ureteroscopy can lead to complications including pyelovenous backflow, bleeding, and infection. Our primary goal was to identify the best cross-section and orientation of a ureteroscope within a Ureteral Access Sheath (UAS) to minimize IPP and maximize outflow. Our secondary goal was to validate our findings with a UAS prototype. To determine the optimal ureteroscope cross-section within a UAS, four ureteroscopes of equivalent cross-sectional area were simulated within a 10 Fr UAS using computational fluid dynamics software COMSOL. We then created a corr

A prospective and randomized comparison of rigid ureteroscopic to flexible cystoscopic retrieval of ureteral stents.

BACKGROUND: Flexible cystoscopy has become an accepted alternative for stent retrieval. However, it is associated with higher cost. Some reports have described experiences of using rigid ureteroscope to retrieve ureteral stents. We compared rigid ureteroscopic to flexible cystoscopic retrieval of ureteral stents in a prospective and randomized clinical trial. METHODS: Three hundred patients treated with ureteral stents between July 2012 and July 2013 were accrued in this study. These patients were divided into two groups using the random number table method. Group A, with 162 patients, had st

Defining the Rate of Primary Ureteroscopic Failure in Unstented Patients: A Multi-Institutional Study.

PURPOSE: Primary ureteroscopic intervention for kidney or ureteral stones occasionally encounters difficulty with passage of the ureteroscope in the initial procedure. These patients require a second procedure after stenting. We aim to define the contemporary failure rate of primary ureteroscopy (URS) and identify predictive factors that necessitate prestenting. This will assist in preoperative patient counseling, informed consent, and clinical decision-making. MATERIALS AND METHODS: We conducted a multi-institutional retrospective review of 535 unstented patients undergoing primary URS from

Urolithiasis in pregnancy: a cost-effectiveness analysis of ureteroscopic management vs ureteral stenting.

OBJECTIVE: The purpose of this study was to determine the cost-effectiveness of serial stenting vs ureteroscopy for treatment of urolithiasis during pregnancy as a function of gestational age (GA) at diagnosis. STUDY DESIGN: We built decision analytic models for a hypothetical cohort of pregnant women who had received a diagnosis of symptomatic ureteral calculi and compared serial stenting to ureteroscopy. We assumed ureteral stent replacement every 4 weeks during pregnancy, intravenous sedation for stent placement, and spinal anesthetic for ureteroscopy. Outcomes were derived from the litera

Ureteroscopic and percutaneous management of upper tract urothelial carcinoma (UTUC): systematic review.

UNLABELLED: What's known on the subject? and What does the study add? Endoscopic management of upper tract urothelial carcinoma (UTUC) using either ureteroscopy and laser ablation, or percutaneous resection, is a management option for treating selected low-grade tumours with favourable characteristics. However, the evidence base for such practice is relatively weak, as the reported experience is mainly limited to small case series (level of evidence 4), or non-randomised comparative studies that are unmatched for tumour stage (level of evidence 3b), with variability of follow-up duration and r
