Contemporary management of blunt aortic injury.

Blunt aortic injury is second only to head injury as the most common cause of blunt trauma related death. The past several years have seen a tremendous growth in the endovascular treatment of such trauma. Short-term and medium-term follow-up data that are emerging seem to support this trend. In this article, the authors discuss about open and endovascular management of blunt aortic injury and provide the latest information regarding endovascular repair of such injuries.

Blood and lymphatic vascular network in bone injury repair: from molecular mechanisms to therapeutic strategies.

Although the coupled blood and lymphatic vascular systems are crucial for mammalian homeostasis, they remain understudied in skeletal contexts. The blood vessel system orchestrates oxygen delivery and waste clearance, while the lymphatic network dynamically regulates interstitial fluid balance and immune surveillance. Their embryonic codevelopment allows for synergistic microcirculatory control. Recently, increasing evidence has highlighted the critical role of the blood and lymphatic vascular networks in bone tissue repair. This network effectively promotes and accelerates bone regeneration b

Effect of aldosterone and mineralocorticoid receptor blockade on vascular inflammation.

Aldosterone, the final product of the renin-angiotensin-aldosterone system, is classically viewed as a regulator of renal sodium and potassium handling, blood volume, and blood pressure. Recent studies suggest that aldosterone can cause microvascular damage, vascular inflammation, oxidative stress and endothelial dysfunction. In animal models, aldosterone-mediated vascular injury in the brain, heart, and kidneys leads to stroke, myocardial injury, and proteinuria. These effects may be modified by dietary salt intake; aldosterone-mediated vascular damage is increased in susceptible animals fed

Transmanubrial L-shaped mini-sternotomy for vascular repair in hemodynamically stable Zone I neck injuries.

ABSTRACT: Penetrating neck injuries in Zone I that involve thoracic outlet structures are rare but carry significant risks, including major airway compromise and massive hemorrhage. Exposure of vascular structures in the thoracic outlet is challenging and typically achieved through median sternotomy for right-sided injuries and left thoracotomy for left-sided injuries. These approaches are familiar to most trauma surgeons, but they may be overly invasive and unnecessary in hemodynamically stable patients. To date, less invasive alternative surgical approaches for Zone I neck injuries requiring

The management of lower extremity multilevel arterial injuries: a 10-year experience.

BACKGROUND: Limb amputation due to lower extremity arterial injury is not uncommon and multilevel arterial injury is even more limb-threatening and easily missed with potentially devastating consequences. There is limited information on multilevel arterial injuries. PURPOSE: We undertook a review of our experience to gain insight on multilevel arterial injury patterns associated with lower extremity trauma and to analyze the results of management of such injuries with a special focus on the influence of initial diagnosis on limb salvage. PATIENTS AND METHODS: Between August 2002 and Septembe

The Neurovascular Unit as a Locus of Injury in Low-Level Blast-Induced Neurotrauma.

Blast-induced neurotrauma has received much attention over the past decade. Vascular injury occurs early following blast exposure. Indeed, in animal models that approximate human mild traumatic brain injury or subclinical blast exposure, vascular pathology can occur in the presence of a normal neuropil, suggesting that the vasculature is particularly vulnerable. Brain endothelial cells and their supporting glial and neuronal elements constitute a neurovascular unit (NVU). Blast injury disrupts gliovascular and neurovascular connections in addition to damaging endothelial cells, basal laminae,

Recent advances and the future of abdominopelvic and lower extremity vascular injury management.

Despite advances in open and endovascular management of trauma, vascular injuries remain a source of devastating outcomes. This narrative review of the literature between 2018 and 2023 explored recent advances in abdominopelvic and lower extremity vascular injury management. New conduit choices, use of temporary intravascular shunts, and advances in endovascular management of vascular trauma were reviewed. Although endovascular techniques are being applied more frequently, there is a paucity of reporting on long-term outcomes. Open surgery is durable and effective and remains the gold standard

Acute Digital Ischemia Associated with Closed Injury.

Injury of an artery has a significantly worse prognosis for the patient than a venous injury. Blunt injuries of lower limb digital arteries with the development of acute ischemia present a very rare phenomenon. A crush mechanism with a defect of the non-wetted surface of vessel's inner part and the development of subsequent thromboischemic lesion is essential for the development of ischemia. We report a blunt injury of the right lower limb in a patient after incorrect stepping with subsequent lesion of digital arteries and the development of acute acral ischemia of the right toes.

Abdominal access in gynaecological laparoscopy: a comparison between direct optical and blind closed access by Verres needle.

OBJECTIVE: Complications associated with initial abdominal entry are a prime concern for laparoscopic surgeons. In order to minimize first access-related complications in laparoscopy, several techniques and technologies have been introduced in the past years. This investigation compares two laparoscopic access techniques. STUDY DESIGN: 194 women underwent laparoscopic surgery for simple ovarian cysts: 93 were assigned to direct optical access (DOA) abdominal entry (group I), and 101 women to classical closed method by Verres needle, pneumoperitoneum and trocar entry (group II). The following

Closed Digital Artery Injury in Children: A Case Report and Review of the Literature.

Closed digital artery injury is a very rare phenomenon, even more so in children, with only 5 cases reported in the literature. We report a case of closed digital arterial interruption in a child after crush injury to the left hand. We further discuss possible etiologies and describe adequate management as found in the pertinent literature. Digital arterial injury should be suspected even in closed hand trauma because a delay in recognition may lead to devastating consequences.

Management of Major Vascular Injuries: Neck, Extremities, and Other Things that Bleed.

Vascular injuries represent a significant burden of mortality and disability. Blunt injuries to the neck vessels can present with signs of stroke either immediately or in a delayed fashion. Most injuries are detected with computed tomography angiography and managed with either antiplatelet medications or anticoagulation. In contrast, patients with penetrating injuries to the neck vessels require airway management, hemorrhage control, and damage control resuscitation before surgical repair. The keys to diagnosis and management of peripheral vascular injury include early recognition of the injur

Cordocytes-stem cells cooperation in the human brain with emphasis on pivotal role of cordocytes in perivascular areas of broken and thrombosed vessels.

This study is based on data analysis by light and transmission electron microscopy of the surgical cases in cerebral tumors, cerebrovascular malformations, thromboses in the carotid system, and other injuries such as perivascular hemorrhage. We examined cortical arteries and veins, perivascular areas with old hematic masses, vasculogenic foci, and broken large vessels. We identified, characterized, and compared both undifferentiated cells and well-differentiated cordocytes within periadventitial areas where these cells cooperate very well with precursor/stem cells to perform vital functions fo

Detrimental Roles of Hypoxia-Inducible Factor-1α in Severe Hypoxic Brain Diseases.

Hypoxia stabilizes hypoxia-inducible factors (HIFs), facilitating adaptation to hypoxic conditions. Appropriate hypoxia is pivotal for neurovascular regeneration and immune cell mobilization. However, in central nervous system (CNS) injury, prolonged and severe hypoxia harms the brain by triggering neurovascular inflammation, oxidative stress, glial activation, vascular damage, mitochondrial dysfunction, and cell death. Diminished hypoxia in the brain improves cognitive function in individuals with CNS injuries. This review discusses the current evidence regarding the contribution of severe hy

New diagnostic techniques for the evaluation of orbital trauma.

Midfacial trauma can produce fractures and soft tissue injuries and can embed foreign bodies within the eye, orbit, and brain. A careful history and ocular examination are fundamental to the evaluation of any patient who has suffered orbital injuries. Damage to the paranasal sinuses and the nasolacrimal system should be considered as a possible consequence of trauma to the orbit. Fractures are best evaluated by conventional roentgenograms with linear or hypocycloidal tomography. Bony fragments can be well visualized by xeroradiography. Positive contrast orbitography may be hazardous and has be

Novel Classification of Thrombotic Disorders Based on Molecular Hemostasis and Thrombogenesis Producing Primary and Secondary Phenotypes of Thrombosis.

Thrombosis, the common and deadliest disorder among human diseases, develops as a result of the intravascular hemostasis following an intravascular injury, which can be caused by a variety of trauma, non-traumatic insults or clinical illnesses. Thrombosis can occur at any location of the vascular system supplied by blood from the heart to large and smallest arterial and venous systems and may affect the function and anatomy of the organ and tissue. It more commonly occurs in the smaller circulatory system of the vascular tree such as arterioles and capillaries, and venules of the organs, espec

VSMCs and the immune microenvironment: a multidimensional regulatory network driving vascular injury and repair.

The dynamic interplay between vascular smooth muscle cells (VSMCs) and the immune microenvironment governs vascular repair and disease progression. This review systematically elucidates this multidimensional regulatory network. We primarily dissect the molecular mechanisms driving pathological VSMC remodeling, encompassing inflammatory signaling, regulated cell death, senescence-associated DNA damage, and organelle dysfunction. Furthermore, we delineate the bidirectional crosstalk connecting VSMCs with the extracellular matrix (ECM) and tissue-infiltrating leukocytes, notably macrophages, T ce
