Autoregulatory vasodilation of parenchymal vessels is impaired during cerebral vasospasm.

Impaired CBF autoregulation during vasospasm after aneurysmal subarachnoid hemorrhage (SAH) could reflect impaired capacity of distal vessels to dilate in response to reduced local perfusion pressure or simply indicate that the perfusion pressure distal to large arteries in spasm is so low that vessels are already maximally dilated. Autoregulatory vasodilation can be detected in vivo as an increase in the parenchymal cerebral blood volume (CBV). Regional CBV, CBF, and oxygen extraction fraction in regions with and without angiographic vasospasm obtained from 29 positron emission tomography stu

Transcranial Doppler in cerebrovascular disease.

Doppler analysis of flow in intracranial arteries is now possible using a 2 MHz probe allowing sufficient penetration of bone to obtain signals noninvasively. Thirty-two normal subjects, and 11 patients with cerebrovascular diseases including vasospasm following subarachnoid hemorrhage, middle cerebral artery stenosis, and extracranial internal carotid artery stenosis were studied by transcranial Doppler. Increased peak velocity and spectral broadening of the reflected signal corresponded to clinical and angiographic evidence of middle cerebral artery vasospasm or stenosis. Decreased peak velo

High-Grade Subarachnoid Hemorrhage - Beyond Guidelines.

Subarachnnoid hemorrhage (SAH) present in a spectrum of clinical severity, from alert with a headache to comatose. High-grade SAH has higher mortality and risk for severe complications including acute respiratory distress syndrome (ARDS) and delayed cerebral ischemia (DCI). Existing treatment approaches for ARDS in SAH require special consideration because of potential impact on intracranial pressure and cerebral perfusion. DCI is a major cause of SAH morbidity and is often discordant with angiographic vasospasm. Current treatment approaches for DCI and vasospasm require further investigation

Intracranial stenting after tumor exeresis complicated by delayed kinking of the middle cerebral artery.

During intracranial tumor resection, the delayed kinking of a major encased vessel has never been described in literature. We present a case which required urgent endovascular treatment performed through a stent positioning. A patient was hospitalized with symptomatic sphenoid meningioma in the left middle cranial fossa. Twelve days after surgery, right-sided hemiplegia and aphasia occurred. Digital subtraction arteriography revealed a kinking of the M1 segment of the left middle cerebral artery and diffuse vasospasm. At first, intra-arterial nimodipine has been administered, obtaining the rem

Fenestration of the lamina terminalis as a valuable adjunct in aneurysm surgery.

OBJECTIVE: Hydrocephalus, vasospasm, and frontobasal injury are common complications after aneurysmal subarachnoid hemorrhage (SAH) from anterior communicating artery aneurysms. Previous studies have suggested that fenestration of the lamina terminalis (FLT) during surgery may be associated with reduced rates of shunt-dependent hydrocephalus and vasospasm. We report 106 patients affected by anterior communicating artery aneurysms and Fisher Grade 3 aneurysmal SAH and the affect of FLT on shunt-dependent hydrocephalus, vasospasm, and frontobasal injury. METHODS: During a 3-year period, 53 pati

Transcranial Doppler grading criteria for basilar artery vasospasm.

OBJECTIVE: Transcranial Doppler (TCD) criteria for basilar artery (BA) vasospasm are poorly defined, and grading criteria for vertebrobasilar vasospasm are unavailable. The purpose of the present study was to define TCD grading criteria for BA vasospasm on the basis of the absolute flow velocities and the intracranial to extracranial flow velocity ratios for the posterior circulation, and to improve the sensitivity and specificity of TCD for diagnosis of BA vasospasm. METHODS: One hundred twenty-three patients with aneurysmal subarachnoid hemorrhage underwent 144 cerebral arteriograms with vi

Mechanisms of vascular dysfunction after subarachnoid hemorrhage.

The main consequence of subarachnoid hemorrhage, for those who survive bleeding, is delayed, persistent vasospasm of intracranial conduit arteries which occurs between the third and seventh day after the insult and results in symptomatic brain ischemia in about 40% of cases. This vasospasm is considered to be a major cause of disability of post-SAH patients. Despite extensive experimental and clinical research, mechanisms of vasospasm are not fully understood. Dysfunction of the endothelium resulting in enhanced production of vasoconstrictors, phenotypic changes of the receptors in endothelium

Continuous Intrathecal Milrinone Administration via IRRAflow Intraventricular Catheter for Refractory Cerebral Vasospasm in Subarachnoid Hemorrhage: A Case Report.

BACKGROUND/OBJECTIVE: The IRRAflow device combines intracerebroventricular (ICV) medication infusion, cerebrospinal fluid (CSF) irrigation, and continuous intracranial pressure (ICP) monitoring. While ICV milrinone is conventionally given as a bolus to manage vasospasm after aneurysmal subarachnoid hemorrhage (aSAH), this case presents the use of continuous ICV milrinone infusion via IRRAflow in a patient with refractory cerebral vasospasm following aSAH. RESULTS: A 47-year-old female with aSAH (Hunt Hess 2, Modified Fisher Grade 4) underwent coil embolization for a ruptured left PICA aneurys

Cerebral vasospasm following subarachnoid hemorrhage: time for a new world of thought.

OBJECTIVE: Delayed cerebral vasospasm has long been recognized as an important cause of poor outcome after an otherwise successful treatment of a ruptured intracranial aneurysm, but it remains a pathophysiological enigma despite intensive research for more than half a century. METHOD: Summarized in this review are highlights of research from North America, Europe and Asia reflecting recent advances in the understanding of delayed ischemic deficit. RESULT: It will focus on current accepted mechanisms and on new frontiers in vasospasm research. CONCLUSION: A key issue is the recognition of ev

Cerebrovascular sensitivity to vasoconstricting agents induced by subarachnoid hemorrhage and vasospasm in dogs.

In anesthetized dogs, subarachnoid hemorrhage (SAH) was induced by the mechanical rupture of the unilateral intracranial internal carotid artery. Vasospasm was angiographically determined 24 hours and 7 days after SAH. Contractile responses to serotonin, norepinephrine, histamine, and K+ were compared in control and bleeding sides of the middle cerebral arteries removed from dogs with SAH, and from sham-operated dogs. Under sham operation and 2 hours after SAH, responses in the arteries of both sides did not appreciably differ but response was significantly less in arteries from the bleeding s

Intracranial and Extracranial Evaluation.

The vascular lab is an essential tool in diagnosing intracranial and extracranial disease including vasospasm from subarachnoid hemorrhage and carotid artery stenosis in the setting of stroke or transient ischemic attack. This article discusses the indications, protocol, and diagnostic criteria for transcranial doppler (TCD) and carotid artery duplex ultrasound. Intracranial and extracranial arterial testing by way of TCD and carotid imaging carries enormous implications and can provide life or death information. The learning curve for these techniques is steep but can be mastered with repetit

Manipulation of intracranial arteries is associated with consistent vessel diameter reductions during microsurgical aneurysm clipping.

To report the rate of diameter reduction of intracranial arteries during aneurysm clipping. Also, in the subset of ruptured aneurysm (RA) the relationship between diameter reduction and symptomatic vasospasm (SVS) Is investigated. A retrospective dataset containing demographics, aneurysm characteristics, vessel dynamics and SVS in adults with surgically clipped intracranial aneurysms was analyzed. The vessel diameters were recorded intraoperatively, both pre- and postclipping. SVS was assessed by transcranial doppler ultrasound or CT-angiogram in the presence of neurological deterioration. Vasospasm after cranial base tumor resection: pathogenesis, diagnosis, and therapy.

OBJECTIVE: Cerebral vasospasm is well known to occur after various cerebral neurosurgical events that cause subarachnoid hemorrhage. However, cerebral vasospasm can occur after cranial base tumor resection. We present a series of nine patients with angiographically evident vasospasm that was clinically symptomatic in eight of them. METHODS: A total of 470 consecutive patients with cranial base tumors were operated in our institution between April 1993 and December 1996. Nine had evidence of cerebral vasospasm postoperatively (1.9% of the total population), of whom eight were asymptomatic. The

Systemic multiple aneurysms of the intracranial arteries and visceral arteries: case report.

BACKGROUND: Systemic multiple aneurysms are rare, and an association between intracranial aneurysms and visceral aneurysms is an extremely rare occurrence. CASE PRESENTATION: This 56-year-old woman was referred to our hospital with subarachnoid hemorrhage. Physical examination revealed no particular morphologic features suggesting connective tissue diseases. Cerebral angiography demonstrated two aneurysms of the left internal carotid artery and the right middle cerebral artery. The internal carotid artery aneurysm was occluded surgically. Four days later, she had right hemiparesis and mild ap

Continuous cerebral spinal fluid drainage associated with complications in patients admitted with subarachnoid hemorrhage.

OBJECT: Cerebral artery vasospasm is a major cause of death and disability in patients recovering from subarachnoid hemorrhage (SAH). Although the exact cause of vasospasm is unknown, one body of research suggests that clearing blood products by CSF drainage is associated with a lower frequency and severity of vasospasm. There are multiple approaches to facilitating CSF drainage, but there is inadequate evidence to determine the best practice. The purpose of this study was to explore whether continuous or intermittent CSF drainage was superior for reducing vasospasm. METHODS: The authors perf

Effect of N-acetylcysteine on vasospasm in subarachnoid hemorrhage.

Vasospasm remains an extremely serious complication that affects patients presenting with subarachnoid hemorrhage (SAH) due to ruptured intracranial aneurysms. The current therapeutic armamentarium is still insufficient in many cases, and the search for new therapies is necessary. In this study, we evaluated the effect of N-acetylcysteine (NAC) on cerebral arterial vasospasm using an experimental model. Twenty-four wistar rats were divided into 4 groups: [1] Control, [2] SAH, [3] SAH+NAC and [4] SAH+Placebo. The experimental model employed double subarachnoid injections of autologous blood. Th
