Iatrogenic perforation of the medial circumflex artery following femoral venous cannulation for transcatheter aortic valve replacement, presenting with retroperitoneal hematoma and successfully managed by percutaneous embolization and coiling.

Vascular complications are a major concern during transcatheter aortic valve replacement and can occur independently of introducer size. Arterial and venous access must be meticulous to avoid potentially life-threatening bleeding, which most commonly results from injury to the common femoral or iliac arteries. We report an unusual case of perforation of the medial circumflex branch of the common femoral artery resulting from attempted femoral venous cannulation. Injury to this vessel is characteristically associated with silent retroperitoneal extravasation and is notoriously difficult to reco

Morphological and clinical aspects of scapular fasciocutaneous free flap transfer for treatment of venous insufficiency in the lower extremity.

We have recently shown that free scapular fasciocutaneous flaps transferred to the lower extremities of patients with chronic venous insufficiency and cutaneous ulcers have resulted in improvement in venous refilling times measured by photoplethysmography in the flap areas and that recurrent ulceration does not recur for up to 7 years. We hypothesized that the transferred flaps contained valves in their microvascular bed, which facilitated venous return, and using scanning electron microscopy of vascular corrosion casts and light and transmission electron microscopy of tissue sections prepared

Comparative Study of Morphology and Distribution of Valves in Human Retromandibular Vein.

The objective of this study was to analyze the distribution and morphology of the valves in the human retromandibular vein. The retromandibular, internal thoracic, azygos, femoral, and brachial veins were harvested from 46 cadavers donated to the Department of Anatomy at Tokyo Dental College for dissection. The frequency of the valves in each vein, the length of the cusps, and the thickness of the vein itself were measured. Valves were present at high frequency (92.1%) in the veins of the limbs and had cusps at least twice as long as the internal diameter of the vein. Veins in the trunk contai

Vascular surgical society of great britain and ireland: new technique of in situ autogenous venous valve construction using varicose vein and bench hydrostatic testing of its properties in vitro.

BACKGROUND: Long-term results of valve transposition and autogenous valve grafts (from axillary and brachial vein) for chronic venous insufficiency due to post-thrombotic valve destruction are poor. Attempts at constructing in situ autogenous venous valves using varicose veins have been ongoing since 1953 but have been unsuccessful. A technique has been developed to create new autogenous in situ valve. METHODS: Varicose long saphenous veins were obtained following stripping during routine varicose vein surgery. A longitudinal elliptical segment of vein wall was excised from opposite sides; thi

In vitro and in vivo hemocompatibility study of fish swim bladder-derived biomaterials for the application of chronic venous disease.

Chronic venous disease (CVD) is a prevalent condition affecting the venous system of the lower limbs, characterized by venous hypertension and regurgitation resulting from congenital or acquired venous valve insufficiency. The global morbidity of CVD is as high as 69.94%. Prosthetic venous valve replacement provides a new therapeutic option. However, to mimic natural venous valves, the leaflets of prosthetic venous valves need to be as thin as ~200 μm, and better resistance to thrombosis is required due to the physiological characteristics of the venous system. Identifying suitable materials f

Pathogenesis of primary chronic venous disease: Insights from animal models of venous hypertension.

BACKGROUND: Reflux of blood through incompetent venous valves is a major cause of the venous hypertension that underlies clinical manifestations of chronic venous disease, including varicose veins, lipodermatosclerosis, and venous ulcers. OBJECTIVE: To review published literature relating to animal models in which venous hypertension has been produced and which have yielded information on the mechanisms by which venous hypertension may trigger inflammation and cause changes in the skin and venous valves. METHODS: Medline searches, with additional papers identified from reference lists in pub

[The structure and function of the vena spermatica interna--the etiology of varicocele].

From 60 male corpse in the age of 13 to 77 both venae spermaticae internae were completely removed. On the basis of these specimens we investigated the incidence, localization, form and scale of the venous valves in the vena spermatica interna. All venous valves were remodeled and examined for their sufficiency on the basis of these date. In the left vena spermatica interna we found in 83% and in the right vena spermatica in 97% of the cases one or more valves. On the average the left vena spermatica contains 3.5 and the right 3.8 valves. From each vein a drawing was made, showing the remifica

To what extent might deep venous thrombosis and chronic venous insufficiency share a common etiology?

According to the valve cusp hypoxia hypothesis (VCHH), deep venous thrombosis is caused by sustained non-pulsatile (streamline) venous blood flow. This leads to hypoxemia in the valve pockets; hypoxic injury to the inner (parietalis) endothelium of the cusp leaflets activates the elk-1/egr-1 pathway, leading to leukocyte and platelet swarming at the site of injury and, potentially, blood coagulation. Here, we propose an extension of the VCHH to account for chronic venous insufficiency. First, should the foregoing events not proceed to frank thrombogenesis, the valves may nevertheless be chroni

Valvular density alone cannot account for sites of chronic venous insufficiency and ulceration in the lower extremity.

OBJECTIVE: Class 6 chronic venous stasis is associated with abnormal venous hemodynamics and ulceration. Ulcers primarily occur over bones and tendon prominences but very rarely over muscular compartments. We hypothesized that the anatomical distribution of venous stasis ulcers in the lower extremity is related to a lower density of venous valves. METHODS: The venous vasculature of six normal human legs was cast with resin, and their microvenous valvular anatomy was examined. Skin samples were obtained from the skin overlying the 1) Achilles' tendon, 2) anterior tibia, 3) medial malleolus, 4)

Partial anomalous pulmonary venous drainage repair concomitant with bilateral semilunar valve replacements and pulmonary artery reconstruction for an adult female 20 years after initial truncus arteriosus repair.

We describe the case of a 21-year-old patient who underwent repairs for multiple lesions including aortic and pulmonary valve replacements, right ventricular outflow tract reconstruction, revision of the right pulmonary artery route, and a repair of partial anomalous pulmonary venous drainage, which was diagnosed during this fourth sternotomy. For these patients with adult CHD, it is most important to address all underlying factors as much as possible at the redo surgery.

Surgical reconstruction of incompetent deep vein valves.

Conventional treatment of deep venous insufficiency results in a rather high failure rate. The introduction of methods for surgical repair of incompetent deep vein valves has implied new possibilities for active treatment and has added increased knowledge to venous pathophysiology. This study summarizes our clinical experience with surgical reconstruction of deep vein valves during a nine-year period.

[Valve injury: a new complication of internal jugular vein cannulation].

Although internal jugular vein cannulation is performed commonly, most physicians are not aware of existence of the internal jugular venous valve. This is the only valve between the right atrium of the heart and the brain, and it has an important role of preventing retrograde blood flow and increased back pressure to the brain. In this study, anatomic appearance of human internal jugular valves is described, and the competence of these valves is assessed. We examined also a safe approach to percutaneous cannulation of the internal jugular vein to avoid this complication. In the cadaveric subje

Stasis Dermatitis: An Overview of Its Clinical Presentation, Pathogenesis, and Management.

Stasis dermatitis is a chronic inflammatory skin disease of the lower extremities. It typically occurs in older individuals and is the cutaneous manifestation of venous hypertension caused by venous reflux. Such retrograde venous blood flow is the result of incompetent venous valves, valve destruction, or venous obstruction. Stasis dermatitis is eczematous. The associated impairment of venous valves may cause swelling of the legs, leading to serious conditions including venous ulcerations. Diagnosis can be challenging because of its clinical resemblance to other skin conditions and poor clinic

Venous valvular assessment after retrograde catheterization.

PURPOSE: To determine whether retrograde passage of a coaxial infusion system (Mewissen-Katzen catheter and guide wire) into extremity veins with valves causes damage. MATERIALS AND METHODS: Retrograde common femoral vein puncture and passage of an infusion wire-catheter system through the superficial femoral and popliteal veins of one leg were performed in five anesthetized pigs. Heparinized saline was then infused via the catheter and wire for approximately 8 hours. The pigs were then killed, and the femoropopliteal veins were removed from both legs of each pig and examined by a pathologist

Correlative Fluorescence and Scanning Electron Microscopy to Study Lymphovenous Valve Development.

Lymph collected from throughout the body is exclusively returned to blood circulation via two pairs of bilaterally located lymphovenous valves. Lymphovenous valves share numerous similarities with lymphatic and venous valves and are defective in multiple mouse models of lymphedema or lymphatic dysfunction. Here we describe a protocol that combines the strengths of fluorescence microscopy and scanning electron microscopy to precisely locate and analyze the topography of developing lymphovenous valves at high resolution.

Postthrombotic syndrome: surgical possibilities.

Postthrombotic syndrome (PTS) is a late outcome of deep vein thrombosis characterized by cramping pain, swelling, hyperpigmentation, eczema, lipodermatosclerosis, and ulceration in the leg due to increased venous outflow resistance and reflux venous flow. Newer surgical and endovascular interventions have a promising result in the management of postthrombotic syndrome. Early surgical or endovascular interventions in appropriately selected patients may decrease the incidence of recurrent ulceration and skin changes and provide a better quality of life. [Surgical treatment of patients with chronic lower limb venous insufficiency and trophic disorders].

The experience in surgical treatment of 326 patients with IV-VI degree chronic lower limb venous insufficiency (CEAP) is analyzed. Correction of disorders of venous hemodynamics was performed with surgical methods and sclerotherapy. The best clinical and functional results in long-term period were achieved in patients undergone surgery in an early stage of the disease without deep veins valves insufficiency and external trophic disorders. Pathology of deep veins required correction of valves function. It is concluded that these patients require complex approach to the treatment.

Mechanisms of varicose vein formation: valve dysfunction and wall dilation.

Varicose veins are a common venous disease of the lower extremity. Although the mechanisms and determinants in the development of varicosities are not clearly defined, recent clinical studies and basic science research have cast some light on possible mechanisms of the disease. In varicose veins, there are reflux and incompetent valves as well as vein wall dilation. Primary structural changes in the valves may make them 'leaky', with progressive reflux causing secondary changes in the vein wall. Alternatively, or concurrently, the valves may become incompetent secondary to structural abnormali
