[Atrial flutter in neonates and infants: diagnosis and treatment].

Between 1995 and 2002, 5 patients with a mean age of 32 years have been treated for atrial flutter in the Department of Paediatric Cardiology at Hospital La Rabta. In all cases, diagnosis was established on surface electrocardiogram (12 leads. All patients had a normal heart. The 3 infants were in heart failure and In the 2 neonates, the arrhythmia was well tolerated despite a very fast ventricular rate. Reduction of the first episode of atrial flutter was possible by cardioversion in 3 patients, 2 of them had a good impregnation with Amiodarone. The two neonates died at 8th and 10th day of li

Life-threatening alterations in heart rate after the use of adenosine in atrial flutter.

Adenosine has become the preferred treatment for common types of supraventricular tachycardia because it is extremely effective and rarely associated with with serious side effects. It has also been advocated as an intervention for diagnostic use to assess uncommon types of tachycardia. Evidence is shown in this report that adenosine was associated with dangerous worsening of arrhythmia in patients with atrial flutter. In two patients, adenosine precipitated acceleration of ventricular response, in one case necessitating emergent cardioversion. Both patients had atrial flutter with 2 to 1 atri

Electrophysiological Cardiovascular Magnetic Resonance (EP-CMR)-Guided Interventional Procedures: Challenges and Opportunities.

PURPOSE OF REVIEW: Cardiovascular magnetic resonance (CMR) imaging excels in providing detailed three-dimensional anatomical information together with excellent soft tissue contrast and has already become a valuable tool for diagnostic evaluation, electrophysiological procedure (EP) planning, and therapeutical stratification of atrial or ventricular rhythm disorders. CMR-based identification of ablation targets may significantly impact existing concepts of interventional electrophysiology. In order to exploit the inherent advantages of CMR imaging to the fullest, CMR-guided ablation procedures

Incidence of arrhythmias after atrial or dual-chamber pacemaker implantation.

The incidence of sustained atrial, pacemaker-mediated and ventricular rhythm disturbances was studied retrospectively in a consecutive series of 112 patients without a history of preexisting atrial tachyarrhythmias, receiving an atrial or dual-chamber pacemaker. Early atrial fibrillation (during the first week) was recorded twice. Late atrial fibrillation was seen in seven patients, flutter in one, yielding a total incidence of 8.9% for 22 months. There were no significant differences with respect to age, aetiology, electrocardiographic diagnosis, pacing history, or the measured intracardiac P

Treatment of ventricular and supraventricular tachyarrhythmias by transcutaneous cardiac pacing.

The efficacy of noninvasive transcutaneous cardiac pacing (TCP) in the treatment of tachyarrhythmic events was tested in 24 patient: 14 with ventricular tachycardia, seven with supraventricular tachycardia and three with atrial flutter. Six (42.9%) ventricular tachycardias were interrupted: in two of the ten patients on whom underdrive pacing was attempted and in all four cases in which overdrive stimulation was possible. Five of the six supraventricular tachycardias utilizing an atrioventricular bypass tract were interrupted, while the TCP was unsuccessful on the only patient with atrioventri

Prosthetic mitral valve motion during cardiac dysrhythmias as determined by echocardiography.

To assess the changes and physiologic mechanisms of prosthetic valve motion during cardiac dysrhythmias as well as the role of atrial systole in the closure of the mitral valve, M-mode echocardiography was performed in 36 patients with normally functioning prosthetic mitral valves (Björk-Shiley, Starr-Edwards, and Beall valve). Premature closure of the prosthetic mitral valve in diastole with a "sharp" closing motion was seen during first-degree atrioventricular block, atrial fibrillation with ventricular rates less than 60 beats/min, and atrial flutter. A "rounded" premature valve closure due

Double Trouble in a Patient with Ischemic Cardiomyopathy and Severe Mitral Regurgitation: A Case Report.

Bidirectional ventricular tachycardia (VT) is a rare arrhythmia characterized by alternating QRS morphologies and axis changes. Atrial flutter (AFL) can coexist with ventricular arrhythmias, complicating diagnosis. We present a case of a 56-year-old man with a history of ischemic heart disease and severe mitral regurgitation admitted with palpitations who was diagnosed with dual tachycardia (bidirectional VT and AFL).

Atrial flutter associated with obstructive sleep apnea syndrome. A case report.

An obese 57-year-old man with the obstructive sleep apnea syndrome had partial tracheostomy occlusion and palpitations. Continuous ECG monitoring demonstrated atrial flutter, occasional premature ventricular beats, and a high degree of atrioventricular block during apneic episodes. A 7-s period of ventricular asystole was demonstrated during one such episode. Revision of the tracheostomy produced an improvement in the patient's condition.

Implanted atrial pacemakers for paroxysmal atrial flutter. Long-term efficacy.

Five patients with drug-resistant paroxysmal atrial flutter received permanent burst atrial pacemakers for the treatment of tachycardia. All patients had extensive electrophysiologic evaluations to determine the safety and efficacy of atrial pacing. The absence of prolonged spontaneous or electrically induced atrial fibrillation was also documented in all patients. Three pulse generators were patient activated (nonautomatic) and two were multiprogrammable and automatic. The atrial pacemakers terminated many attacks of paroxysmal atrial flutter safely and reliably in a follow-up period ranging

Polymorphous ventricular tachycardia in a newborn with the long QT interval.

We have reported a case of long QT syndrome in a neonate who had ventricular tachycardia 30 minutes after birth. On the third day, while being weaned from lidocaine, the patient had ventricular flutter and required cardioversion. Treatment with propranolol (5 mg/kg/day) maintained normal sinus rhythm. We believe this may be the first reported case of long QT syndrome and normal AV conduction in a neonate who had multiple episodes of ventricular tachycardia despite medical treatment.

Incessant non-sustained ventricular tachycardia after stimulus of electroconvulsive therapy with atropine premedication?

Electroconvulsive therapy (ECT) is an effective and safe treatment for a variety of neuropsychiatric disorders. Premedication with atropine has been recommended in order to avoid bradycardia and transient asystole induced by ECT. In contrast, some other arrhythmias can happen such as atrial flutter and fibrillation. But ventricular tachycardia is rare. Reported herein is a case of incessant non-sustained ventricular tachycardia, possibly triggered by atropine premedication.

Myotonic dystrophy initially presenting as tachycardiomyopathy successful catheter ablation of atrial flutter.

Myotonic dystrophy is a genetic muscular disease that is frequently associated with cardiac arrhythmias. Bradyarrhythmias, such as sinus bradycardia and atrioventricular block, are more common than tachyarrhythmias. Rarely, previously undiagnosed patients with myotonic dystrophy initially present with a tachyarrhythmia. We describe the case of a 14-year-old boy, who was admitted to the hospital with clinical signs and symptoms of decompensated heart failure and severely reduced left ventricular function. Electrocardiography showed common-type atrial flutter with 2 : 1 conduction resulting in a

Prospective evaluation of catheter ablation in patients with implantable cardioverter defibrillators and multiple inappropriate ICD therapies due to atrial fibrillation and type I atrial flutter.

The high incidence of inappropriate therapies due to drug refractory supraventricular tachycardia remains a major unsolved problem of the ICD. Most of the inappropriate therapies for supraventricular tachycardia are caused by AF and type I atrial flutter with rapid ventricular response. The purpose of this prospective study was to determine the usefulness of AVN modulation or ablation for rapid AF and ablation of the tricuspid annulus-inferior vena cava (TA-IVC) isthmus for type I atrial flutter in ICD patients with frequent inappropriate ICD interventions. Eighteen consecutive patients were e

[Radiofrequency catheter ablation of atrial flutter and atrial fibrillation].

Radiofrequency catheter ablation is now considered as a curative approach in patients with typical atrial flutter. Typical atrial flutter is due to a macrore-entrant circuit within the right atrium and it can be eliminated by a linear lesion in the isthmus between the tricuspid annulus and the vena cava inferior. The electrophysiological criterion of a bidirectional isthmus block has been shown to reduce the recurrence rate of atrial flutter after catheter ablation, thus achieving long-term cure of typical atrial flutter. Acute success rates of 85 to 90% and recurrence rates of 10 to 15% have

Signs of membranous subaortic stenosis appearing after correction of persistent common atrioventricular canal.

Two cases are presented of persistent common atrioventricular (A-V) canal with interventricular communication in which associated membranous subaortic stenosis became apparent only after correction of the basic malformation and replacement of the mitral valve. The features of the membranous subaortic stenosis present only after operation were a left ventricular outflow pressure gradient and angiocardiographic demonstration of the membrane. In one case, additional echocardiograms demonstrated systolic flutter of the aortic cusps after the first operation (no echocardiograms were taken preoperat

[Anti-arrhythmic therapy with verapamil by intravenous infusion].

Rapid and slow venous infusion of various doses of Verapamil in a mixed series of 185 cases of arrhythmia since 1968 is reported. Results and electrophysiological and ECG changes observed for each type of arrhythmia examined are considered separately: atrial fibrillation-flutter, supraventricular paroxystic tachycardia (atrial and/or junctional), and hyperkinetic ventricular arrhythmia. An association of i.v. Verapamil and a quinidine salt per os is suggested as an alternative to cardioversion in cases of recent atrial fibrillation-flutter. Blanked atrial flutter in patients with cardiac resynchronization therapy: clinical significance and implications for device programming.

BACKGROUND: Atrial arrhythmias are frequently observed in patients with heart failure and may be a primary cause for decompensation during cardiac resynchronization therapy (CRT). The accurate detection of organized atrial tachyarrhythmias poses a challenge to the function of mode-switching biventricular pacemakers/defibrillators. METHODS: The purpose of the study was to determine retrospectively the incidence of blanked atrial flutter and mode switch failure (2:1 lock-in), and to look for factors predisposing to this problem. A total number of 65 patients with CRT devices has been followed r
