Association between pulmonary function and left ventricular volume and function in duchenne muscular dystrophy.

INTRODUCTION: Duchenne muscular dystrophy (DMD) is characterized by absence of the subsarcolemmal protein dystrophin, present in skeletal muscles and cardiomyocytes. We hypothesized that progressive respiratory and left ventricular (LV) insufficiencies in DMD could be parallel and interrelated phenomena. METHODS: We conducted a retrospective chart review of 27 patients with DMD. Our primary objective was to compare the rates of decline between pulmonary function test (PFT) measures (forced expiratory volume in the first second, forced vital capacity, peak expiratory flow rate, maximal inspira

Effects of radiofrequency catheter ablation for premature ventricular complexes originating from the right ventricular outflow tract on right ventricular function.

INTRODUCTION: This study aimed to elucidate the relationship between premature ventricular complexes (PVCs) and right ventricular (RV) dysfunction, and the effects of radiofrequency catheter ablation (RFCA) on RV function. METHODS: A total of 110 patients (age, 50.8 ± 14.4 years; 30 men) without structural heart disease who had undergone RFCA for RV outflow tract (RVOT) PVCs were retrospectively included. RV function was assessed using fractional area change (FAC) and global longitudinal strain (GLS) before and after RFCA. Clinical data were compared between the RV dysfunction (n = 63) and pr

Coronary microvascular dysfunction and right ventricular structure and function.

Previous studies have suggested associations between coronary microvascular dysfunction (CMD) and alterations in left ventricular (LV) structure and function Data are however scarce regarding the right ventricular (RV). In the context of CMD, the RV could be affected via mechanisms potentially involving ischemia from CMD, shared pathophysiological milieu leading to adverse ventricular remodeling, and/or increased afterload secondary to increased LV end-diastolic pressure or heart failure with preserved ejection fraction (HFpEF) with pulmonary hypertension. We evaluated the relationship between

Pulmonary Function in Older Patients With Ventricular Septal Defect.

To better understand the evolvement of the pulmonary dysfunction in the aging ventricular septal defect (VSD) patient, we invited adult patients with a congenital VSD and healthy age- and gender-matched controls for static and dynamic spirometry, impulse oscillometry, multiple breath washout, and diffusion capacity for carbon monoxide testing. Primary outcome was forced expiratory volume in 1 second. In total, 30 patients with a surgically corrected VSD (mean age 51 ± 8 years), 30 patients with a small, unrepaired VSD (mean age 55 ± 1 years), and 2 groups of 30 age- and gender-matched, healthy

Pulmonary function assessment post-left ventricular assist device implantation.

AIM: The lungs-and particularly the alveolar-capillary membrane-may be sensitive to continuous flow (CF) and pulmonary pressure alterations in heart failure (HF). We aimed to investigate long-term effects of CF pumps on respiratory function. METHODS AND RESULTS: We conducted a retrospective study of patients with end-stage HF at our institution. We analysed pulmonary function tests [e.g. forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1 )] and diffusing capacity of the lung for carbon monoxide (DLCO ) from before and after left ventricular assist device (LVAD) implantation

Gout, not hyperuricemia alone, impairs left ventricular diastolic function.

INTRODUCTION: Gout is a common metabolic disorder characterized by hyperuricemia and chronic inflammation. Previous studies show that hyperuricemia accelerates the occurrence and worsening of cardiovascular disease due to LV remodeling. However, it is still unclear whether hyperuricemia is the sole contributor to organic heart remodeling in patients with gout. In addition, there is a paucity of data regarding the association between LV diastolic function and gout. The objective of this study was to investigate the effects of gout on LV diastolic function. METHODS: A total of 173 patients were

Right ventricular function in fetal hypoplastic left heart syndrome.

BACKGROUND: The systemic right ventricle in palliated hypoplastic left heart syndrome (HLHS) has relatively reduced longitudinal compared with circumferential deformation, a pattern of contraction more akin to the normal left ventricle, which presumably improves right ventricular (RV) pumping efficiency. The aim of this study was to test the hypothesis that these changes in the RV contraction pattern in infants with HLHS are present prenatally. METHODS: Echocardiograms from 48 fetuses with HLHS were retrospectively compared with those from appropriately grown RV and left ventricular controls.

Vasovagal syncope in patients with reduced left ventricular function.

Vasovagal syncope (VVS) is mediated by arterial mechanoreceptors, resulting in reflexive changes in heart rate and vascular tone. The Bezold-Jarisch reflex was originally described as enhanced contraction and activation of left ventricular mechanoreceptors, but later studies implicated other triggers, including coronary, carotid, and cerebral arterial mechanoreceptors. VVS is uncommon in patients with left ventricular dysfunction. We hypothesized that VVS could occur in this subset and examined patient characteristics and hemodynamic responses during tilt table testing. From 1996 through 1998,

Quantifying regional right ventricular function in tetralogy of Fallot.

Right ventricular (RV) function is notoriously difficult to quantify. Patients with tetralogy of Fallot (TOF) have decreased systolic performance. We measure regional RV performance using MRI with 1-dimensional myocardial tissue tagging. By tagging cine-MRI in two views, we measured regional shortening in 12 regions throughout the RV. We image 32 pediatric patients: 21 normal patients and 11 patients with repaired TOF. We establish a normal range for each RV region. TOF patients have decreased shortening on a region-by-region basis. We conclude that regional RV performance can be measured usin

[Evaluation of left ventricular diastolic function using Doppler echocardiography].

INTRODUCTION: Clinical studies have shown that approximately 40% of patients with congestive heart failure have predominantly diastolic left ventricular dysfunction. Doppler echocardiography is a simple, noninvasive and safe technique that can be used for measurement of diastolic filling and, therefore for assessment of diastolic function. PHYSIOLOGY OF DIASTOLE: Diastole is divided into four phases: isovolumetric relaxation, rapid filling, slow filling (diastasis) and atrial contraction. Diastolic filling can be defined as the period from the onset of mitral valve opening to mitral valve clo

Left ventricular function and geometry in juvenile mitral stenosis.

The purpose of this study was to investigate possible differences between juvenile and adult variety of mitral stenosis (MS) with regard to left ventricular preload, afterload, contractile function and geometry by two-dimensional echocardiography. Thirty six consecutive children and adolescents with MS (Group 1, mean age +/- 1 standard deviation 16 +/- 3 yrs) were compared with an equal number of adults with MS of comparable mitral valve area (Group II, mean age 30 +/- 8 yrs). Patients with juvenile versus adult MS had similar left ventricular ejection fraction (0.57 +/- 0.09 vs 0.57 +/- 0.11,

Intracoronary papaverine but not adenosine reduces regional ventricular function.

OBJECTIVES: Whether intracoronary papaverine or adenosine leads to reductions in regional left ventricular function was tested. METHODS: Fifteen anaesthetised dogs were prepared to record aortic pressure, left ventricular pressure dP/dt, electrocardiogram, regional ventricular shortening, and phasic and mean left circumflex coronary blood flow, and to give intracoronary boluses of papaverine (2, 4, or 6 mg) or adenosine (0.37 and 1.87 mg). RESULTS: Injected doses were chosen to mimic those given in the clinical setting. Papaverine (6 mg) reduced aortic pressure (mean 96(SD 17) to 89(18) mm H

Right ventricular function in severe non-ischaemic mitral insufficiency.

Mitral insufficiency (regurgitation) is a disease of both ventricles. To determine the pathophysiological implications and clinical value of assessment of right ventricular function in this disease, right ventricular ejection fraction was determined by radionuclide cineangiography (r = 0.73, P less than 0.01, vs contrast angiography, n = 30) in patients with severe, non-ischaemic mitral regurgitation. Among 31 patients with isolated mitral regurgitation treated medically, five died, all within 2 years of follow-up; all five were among the eight patients with left ventricular ejection fraction

Left ventricular function in acute non-ischaemic mitral regurgitation.

Acute non-ischaemic mitral regurgitation (MR) has recently generated considerable interest because of its causal relationship to ruptured chordae tendineae and infective endocarditis, advances in its diagnosis by echo Doppler studies, and its management by afterload reduction and reconstructive surgery. It is fundamentally different from chronic MR because the previously normal, unprepared left ventricle (LV) and left atrium (LA) confront a sudden dramatic increase in volume. As the normal-sized left atrium suddenly receives a marked regurgitant flow, its pressure rises and is transmitted into

[Disorders of diastolic function in chronic left ventricular insufficiency].

Abnormalities in the diastolic function of the left ventricular pump are the common determinant and, above all, the earliest manifestation of all forms of chronic left ventricular failure, whether or not the left ventricular systolic function is abnormal. Congestive signs, in particular, are directly related to abnormalities of ventricular filling. Primary diastolic dysfunction is the cause of left ventricular failure in about 40 p. 100 of the cases, but it may also be observed in almost all cardiopathies. In myocardial ischaemia the pressure-volume relation is displaced upwards owing to a slo

Importance of right ventricular function in congestive heart failure.

Failure of the right ventricle may be due to a congenital anomaly, intrinsic disease, pulmonary stenosis or pulmonary hypertension. Left ventricular failure may also lead to right ventricular failure if the heart fails totally or secondary to pulmonary hypertension, or if filling of the right ventricle is decreased due to left ventricular dilation or hypertrophy. Treatment of right ventricular failure has yielded disappointing results, except when caused by left ventricular failure that responds to therapy. Digitalis and diuretics may have more adverse than beneficial effects. In patients with
