Comprehensive Equilibrium Function Tests for an Accurate Diagnosis in Vertigo: A Retrospective Analysis.

Background/Objectives: An accurate diagnosis of vertigo is crucial in patient care. Traditional balance function tests often fail to offer independent, conclusive diagnoses. This study aimed to bridge the gap between traditional diagnostic approaches and the evolving landscape of automated diagnostic tools, laying the groundwork for advancements in vertigo care. Methods: A cohort of 1400 individuals with dizziness underwent a battery of equilibrium function tests, and diagnoses were established based on the criteria by the Japanese Society for Vertigo and Equilibrium. A multivariate analysis i

Posturography: head stabilisation compared with platform recording. Application in vestibular disorders.

Posturography with simultaneous recording of the head movements (H-recording) and of the pressure of the feet on the platform (P-recording) has been applied in patients with peripheral vestibular disorders. For the whole testing (four tests with visual and proprioceptive interaction) 40% showed an identical degree of abnormality for the measurements of surface in P- and H-recording. In a large majority of the patients, showing a difference between H- and P-recording, this difference indicated a lower degree for the H-recording. Concerning the four tests separately, the differences for eyes clo

A New Proposal for Severity Evaluation of Menière's Disease by Using the Evidence From a Comprehensive Battery of Auditory and Vestibular Tests.

To date, no widely accepted criteria exist to quantify the severity of Menière's disease (MD) by using vestibular tests. This study aimed to compare association of hearing loss and vertigo severity with association of accurate assessments of vestibular function and the vertigo severity. The severity of vertigo was documented by a comprehensive medical history with number of vertigo attacks in the past 6 months and a Dizziness Handicap Inventory (DHI) questionnaire. The involvement of vestibular organs was verified by audio-vestibular tests in 80 definite MD patients. Correlations between DHI s

[Auditory and vestibular lesions in Behcet's disease].

Auditory and vestibular function tests were conducted in 16 patients with Behcet's disease. Vestibular lesions included spontaneous nystagmus in 5 cases, and a bilateral vestibular syndrome after provoked tests in 13 cases (7 of the deficiency type and 6 irritative in nature). Findings after auditory tests were bilateral symmetric deafness in 2 cases; one of the perception type and the other showing mixed features. Various etiopathogenic theories are discussed. The possibility of a bilateral central lesion of the VIIIth nerve was also considered, 8 patients in the series having cerebral manife

Vestibular evaluation--electronystagmography, rotational testing, and posturography.

This report describes a battery of tests that is utilized by several clinical laboratories to investigate vestibular and balance function. The electronystagmogram evaluates eye movements, inner ear function, brainstem function and cerebellar function. Sinusoidal vertical axis rotation is a sensitive measure of horizontal semicircular canal function that is often complimentary to the caloric stimulation portion of the electronystagmogram. Visual-vestibular interaction rotation testing is designed to evaluate brainstem, cerebellar and oculomotor function. Dynamic posturography facilitates measur

Vertigo: a physiological approach.

An understanding of vestibular function and a careful history will quickly separate patients with disease of the vestibular system from others. Further history and physical examination, often coupled with audiometric and vestibular function tests, will aid in localizing the lesion to the central or peripheral portions of the system and in determining the affected side. Medical management, which is often nonspecific and involves considerable psychological support, usually suffices.

Posturography: uses and limitations.

The field of posturography has been advanced by the development of computerized dynamic posturography, wherein a force platform has been combined with visual stimuli as a means of determining the relative importance of the various sensory inputs critical for balance, namely vision, somatosensation and vestibular sensation. When compared with other tests currently available for the assessment of vestibular function, computerized dynamic posturography is unique in that it assesses 'balance' rather than attempting to assess peripheral or central vestibular function more directly. This discussion

Vestibular function at the end of intratympanic gentamicin treatment of patients with Ménière's disease.

The aim of this study is to analyze the effects of intratympanic gentamicin injections on vestibular function in 33 patients with unilateral Meniere's Disease (according to AAO-HNS guidelines 1995) that had been unresponsive to medical therapy for at least one year. In such patients, the results of bedside examination of vestibular function vestibular examination is compared to those from laboratory tests. Intratympanic gentamicin injections (27 mg/ml) were performed at weekly intervals until symptoms or signs of vestibular hypofunction developed in the treated ear. Vestibular function was eva

Clinical utility of ocular vestibular-evoked myogenic potentials (oVEMPs).

Over the last years, vestibular-evoked myogenic potentials (VEMPs) have been established as clinical tests of otolith function. Complementary to the cervical VEMPs, which assess mainly saccular function, ocular VEMPs (oVEMPs) test predominantly utricular otolith function. oVEMPs are elicited either with air-conducted (AC) sound or bone-conducted (BC) skull vibration and are recorded from beneath the eyes during up-gaze. They assess the vestibulo-ocular reflex and are a crossed excitatory response originating from the inferior oblique eye muscle. Enlarged oVEMPs have proven to be sensitive for

[Dissociation of the results of caloric and video head impulse tests as a marker of Meniere's disease].

The study of vestibular function in patients with Meniere's disease (MD) is an urgent task, since it influences the choice of treatment tactic. We evaluated the results of caloric and video head impulse (vHIT) tests in 76 patients who met the clinical criteria for significant Meniere's disease (AAO-HNS modified by the Barany Society, 2015). Dissociation of the results of caloric and vHIT tests is observed in the majority (74%) of patients with MD: at normal gain, according to vHIT data, hypofunction of peripheral vestibular structures is recorded according to the caloric test (KASL 47 ± 7.8%).

Clinical vestibular testing assessed with machine-learning algorithms.

IMPORTANCE: Dizziness and imbalance are common clinical problems, and accurate diagnosis depends on determining whether damage is localized to the peripheral vestibular system. Vestibular testing guides this determination, but the accuracy of the different tests is not known. OBJECTIVE: To determine how well each element of the vestibular test battery segregates patients with normal peripheral vestibular function from those with unilateral reductions in vestibular function. DESIGN, SETTING, AND PARTICIPANTS: Retrospective analysis of vestibular test batteries in 8080 patients. Clinical medic

Using HINTS in acute vestibular syndrome: A practical guide for the acute care physician.

Acute vestibular syndrome (AVS) is a common clinical presentation. Common causes include peripheral, self-limiting labyrinthine disorders, but a significant proportion are caused by posterior circulation stroke (PoCS). Delineating between a peripheral versus central cause in AVS is challenging for acute care physicians, but a simple, three-step bedside test known collectively as the HINTS examination may be useful. Substantial evidence supports the use of the HINTS examination in detection of PoCS in AVS, but it is important that performer is trained and experienced to ensure reliability of th

[Inferior vestibular neuritis: diagnosis using VEMP].

Vestibular evoked myogenic potentials (VEMP) are a new method to establish the functional status of the otolith organs. The sacculocollic reflex of the cervical VEMP to air conduction (AC) reflects predominantly saccular function due to saccular afferents to the inferior vestibular nerve. We describe a case of inferior vestibular neuritis as a rare differential diagnosis of vestibular neuritis. Clinical signs were a normal caloric response, unilaterally absent AC cVEMPs and bilaterally preserved ocular VEMPs (AC oVEMPs).

Recovery of vestibular ocular reflex function and balance control after a unilateral peripheral vestibular deficit.

This review describes the effect of unilateral peripheral vestibular deficit (UPVD) on balance control for stance and gait tests. Because a UPVD is normally defined based on vestibular ocular reflex (VOR) tests, we compared recovery observed in balance control with patterns of recovery in VOR function. Two general types of UPVD are considered; acute vestibular neuritis (AVN) and vestibular neurectomy. The latter was subdivided into vestibular loss after cerebellar pontine angle tumor surgery during which a vestibular neurectomy was performed, and vestibular loss following neurectomy to elimina
