The prognostic significance of intraoperative facial nerve electromyography (EMG) changes is not well-established in vestibular schwannoma (VS) surgery. We studied facial nerve EMG with a threshold >0.05mA and performed subgroup analyses based on tumor size, resection approach, and extent of resection, for prediction of long-term facial nerve outcome. A total of 477 surgically treated VS patients were included. Elevated stimulation threshold exceeding >0.05mA is a highly specific (90%), but very insensitive (29%) finding in this cohort. The positive predictive value and negative predictive val

The points of contact between the vestibular system and the trigeminal nerve remain an active area of research. Anatomically, several connections have been clearly identified, and these may play a role in the development of various disorders. Understanding these connections also proves to be extremely valuable from a clinical perspective. It is increasingly evident that the etiopathogenesis of various vestibular disorders is multifactorial. Therefore, knowledge of the points of interaction between the two systems can assist clinicians in patient assessment and, most importantly, in selecting t

In vestibular schwannoma (VS) patients hearing outcome and surgery related risks can vary and depend on the nerve of origin. Preoperative origin differentiation between inferior or superior vestibular nerve may influence the decision on treatment, and the selection of optimal treatment and counselling modalities. A novel scoring system based on functional tests was designed to predict the nerve of origin for VS and was applied to a large number of consecutive patients. A prospective, double blind, cohort study including 93 patients with suspected unilateral VS was conducted at a tertiary refer

We report a case of cochleovestibular neurovascular compressive syndrome (CVCS)-induced drop attack treated with microvascular decompression (MVD) of the superior vestibular nerve. This report discusses the merits of surgical intervention through a review of related literature. A 58-year-old woman was referred to our clinic with a chief complaint of intermittent, strong, right-sided tinnitus lasting for a few seconds immediately prior to drop attack. Magnetic resonance imaging (MRI) showed bilateral neurovascular contact between the anterior inferior cerebellar artery (AICA) and the vestibuloc

The classical surgical technique for the resection of vestibular schwannomas (VS) has emphasized the microsurgical anatomy of cranial nerves. We believe that the focus on preservation of the arachnoid membrane may serve as a safe guide for tumor removal. Method The extracisternal approach is described in detail. We reviewed charts from 120 patients treated with this technique between 2006 and 2012. Surgical results were evaluated based on the extension of resection, tumor relapse, and facial nerve function. Results Overall gross total resection was achieved in 81% of the patients. The overall

Facial nerve dysfunction after resection of a vestibular schwannoma is one of the most common indications for facial nerve habilitation. This article presents an overview of common and emerging management options for facial habilitation following resection of a vestibular schwannoma. Immediate and delayed nerve repair options, as well as adjunctive surgical, medical, and physical therapies for facial nerve dysfunction, are discussed. Two algorithms are provided as guides for the assessment and treatment of facial nerve paralysis after resection of vestibular schwannoma.

OBJECT: The goal of this article was to show that a combination of facial nerve-sparing microsurgical resection and Gamma Knife surgery (GKS) for expansion of any residual tumor can preserve good facial nerve function in patients with recurrent vestibular schwannoma (VS).
METHODS: Records of individuals treated by a single surgeon with a facial nerve-sparing technique for a VS between 1998 and 2009 were retrospectively analyzed for tumor recurrence. Of the 383 patients treated for VS, 151 underwent microsurgical resection, and 20 (13.2%) of these patients required postoperative retreatment fo

BACKGROUND: To date, 50 cases of cavernous angioma in the CPA have been reported, and previous reports did not describe the tumor's site of origin. We describe a case of a small, extraaxial cavernous angioma of the vestibular nerve. We also propose a reclassification system for cavernous angioma of the CPA based on the tumor's site of origin.
CASE DESCRIPTION: A 39-year-old female patient had recurrent deteriorating vertigo and a right hearing disturbance. Magnetic resonance imaging revealed a cavernous angioma of the right CPA. Surgery was performed through a right lateral suboccipital appro

Patients with vertigo resistant to conservative treatment require surgical management. Between March 1991 and August 1996, vestibular nerve sections were performed in 108 patients with peripheral vertigo not responding to conservative treatment. The diagnoses were classic Ménière's disease in 96 patients and recurrent vestibulopathy in 12 patients. Combined retrosigmoid retrolabyrinthine (n = 106) and retrolabyrinthine (n = 2) approaches were used. Patients were grouped according to follow-ups of less than 2 years and more than 2 years. In the former group (n = 49), hearing preservation and ve

Vestibular neurectomy (VN) results in a high success rate in the control of vertigo in Meniere's disease, although the subsequent fate of auditory function is fairly unpredictable. The present investigation reports the postoperative results obtained in a group of 30 subjects with a clinical diagnosis of Meniere's disease and vascular cross-compression of cranial nerve VIII. All subjects underwent VN using a retrosigmoid approach, and in half of them microvascular decompression (MVD) of the cochlear nerve with interposition of autogenous muscle was performed at the same time. All patients had c

In 4 cats all vestibular afferents in one labyrinth except those innervating the saccular macula were transected and allowed to degenerate. 23--53 days after the initial surgery the central connections of the remaining saccular nerve were studied under chloralose anesthesia. Stimulation of the saccular nerve evoked N1 field potentials in the ipsilateral lateral and descending vestibular nuclei; little or no field potential activity was seen in the superior nucleus. The distribution of field potentials overlapped with that of neurons of origin of the vestibulospinal tracts. Forty-two neurons in

The triad of fluctuating hearing loss, tinnitus, and episodic attacks of vertigo known as Ménière's disease can be a frustrating therapeutic problem. While most patients respond to a medical regime consisting of a low salt diet, diuretics, diazepam, and propantheline bromide, there is a significant group who remain refractory to conservative management. These individuals either continue to experience their distressing symptoms or undergo some type of surgical procedure in an attempt to seek relief. There are a multitude of such techniques available to the otologic surgeon and he must make the

OBJECTIVE: The incidence and risk factors for facial nerve dysfunction (FND) following CyberKnife® therapy for vestibular schwannoma (VS) remain poorly understood. This study investigates whether differential radiation doses to vulnerable segments of the facial nerve may be associated with FND outcomes.
METHODS: Patients were identified who underwent CyberKnife® radiosurgery for VS at a single institution. Basic demographics, tumor characteristics, and facial nerve function were collected. Total radiation doses to tumor, internal auditory canal (IAC), and labyrinthine segment of facial nerve

OBJECTIVES: Predicting the course of cranial nerve (CN) VII in the cerebellopontine angle (CPA) on preoperative imaging for vestibular schwannoma (VS) may help guide surgical resection and reduce complications. Diffusion MRI based tractography has been used to identify cranial nerve trajectory, but intraoperative validation of this novel approach is challenging. Currently, validation is based on operative report descriptions of the course of cranial nerves, but yields a simplified picture of the three-dimensional (3D) course of CN VII. In this study, we investigate the accuracy of tractography
