Revised ESTS guidelines for preoperative mediastinal lymph node staging for non-small-cell lung cancer.

Accurate preoperative staging and restaging of mediastinal lymph nodes in patients with potentially resectable non-small-cell lung cancer (NSCLC) is of paramount importance. In 2007, the European Society of Thoracic Surgeons (ESTS) published an algorithm on preoperative mediastinal staging integrating imaging, endoscopic and surgical techniques. In 2009, the International Association for the Study of Lung Cancer (IASLC) introduced a new lymph node map. Some changes in this map have an important impact on mediastinal staging. Moreover, more evidence of the different mediastinal staging techniqu

The safety and versatility of video-thoracoscopy: a prospective analysis of 895 consecutive cases.

BACKGROUND: The application of video-endoscopy to general thoracic surgery is radically changing the approach to many benign and malignant diseases of the chest. Since July 1991, we have performed 794 purely thoracoscopic and 101 video-assisted thoracic surgical (VATS) procedures on 860 patients. STUDY DESIGN: Comprehensive, prospectively acquired data examining the specific indications for and outcomes of this new technique were prospectively entered into a thoracic surgical database. Preoperative, intraoperative, postoperative, and outcome variables were studied for the entire group as well

Video versus direct laryngoscopy for urgent tracheal intubation in neonates: a systematic review and meta-analysis.

INTRODUCTION: Intubation is most often performed electively by anaesthetists in controlled conditions in operating theatres. In neonates, however, it is most often performed by neonatologists or paediatricians in urgent circumstances in the neonatal intensive care unit (NICU) or delivery room (DR). Neonatal intubation is a difficult skill to learn and maintain, and success rates are suboptimal both in the NICU and DR. Video laryngoscopy (VL) has the potential to increase intubation success and safety as it may offer a better view of the airway, which can be shared by the intubator and other cl

Video-assisted thoracoscopic diaphragmatic plication.

The authors demonstrate a video-assisted thoracoscopic surgical technique for diaphragmatic plication, which is used to treat acquired diaphragmatic paralysis resulting from injury to the phrenic nerve. The objective of the surgical procedure is to return the abdominal contents to their normal position and restore optimal lung expansion by reducing the size of the diaphragmatic surface. Successful diaphragmatic plication improves lung function, reduces dyspnea, and restores quality of life.

Perventricular device closure of ventricular septal defect using a video-assisted thoracoscopic approach.

We describe a new technique of perventricular closure of a perimembranous ventricular septal defect on a beating heart using transesophageal echocardiography guidance and video-assisted thoracoscopy. Ventricular septal defects were closed successfully, and no shunts, rhythm disorders, or valve incompetence developed during the short hospital stay. This procedure is safe and effective for selected patients. These early and mid-term results encourage further evaluation.

Video-assisted thoracoscopic treatment of spinal lesions in the thoracolumbar junction.

BACKGROUND: The endoscopic treatment of spinal lesions in the thoracolumbar junction (T11-L2) poses a great challenge to the surgeon. From November 1, 1995 to December 31, 1996, we successfully used a combination of video-assisted thoracoscopy and conventional spinal instruments to treat 38 patients with anterior spinal lesions. Twelve of them had lesions in the thoracolumbar junction. METHODS: The so-called extended manipulating channel method was used to perform vertebral biopsy, discectomy, decompressive corpectomy, interbody fusions, and/or internal fixations in these patients. The size o

Minimal invasive surgery for resection of parathyroid tumor in the aortopulmonary window.

Video-assisted thoracoscopic surgery for parathyroid glands deep in the mediastinum is a safe and successful procedure, but its success depends on accurate localization before operation. Recently, a variety of minimally invasive techniques to approach cardiovascular disease have been proposed to eliminate the morbidity of standard sternotomy. We describe a case involving a 71-year-old woman with persistent hyperparathyroidism resulting from an ectopic mediastinal parathyroid gland, which was removed through an upper sternotomy. The technique has proved safe, effective, and aesthetically accept

Video-assisted transumbilical Meckel's diverticulectomy in children.

The treatment of Meckel's diverticulum (MD) in children is resection. Some data exist for the use of laparoscopic resection. The Video-Assisted Transumbilical (VAT) single-trocar technique has been recently described for appendectomy. We also have used this technique for the resection of MD. The purpose of this study is to report our experience with laparoscopic-assisted resection of MD using both the three-trocar and the single-trocar techniques. The Institutional Review Board approved our retrospective chart review of all patients with the diagnosis of MD. Only the cases that were treated vi

Robot-Assisted Hysterectomy Using the Double-Bipolar Method.

OBJECTIVE: To demonstrate the efficacy of the double-bipolar method in a benign hysterectomy. DESIGN: Stepwise demonstration of the technique with a narrative video. SETTING: The double-bipolar method was first reported in 2011 [1] and is gaining popularity in Japan; however, its usefulness in robot-assisted hysterectomy is under-reported. When unexpected bleeding occurs during robot-assisted hysterectomy using a monopolar technique, corrective measures may be prolonged and often require changing forceps. The Maryland forceps have 4 functions, including incision, dissection, grasping, and co

Trans-oral robotic cleft surgery (TORCS) for palate and posterior pharyngeal wall reconstruction: A feasibility study.

BACKGROUND/AIM: Robot-assisted surgery has become increasingly routine, replacing open and laparoscopic techniques in certain domains, with recent extension to head and neck surgery through trans-oral access. Some advantages of the robot-assisted surgery include the ability to access confined spaces, enhanced dexterity instrumentation with intuitive movement, motion scaling, tremor elimination and three-dimensional (3D) endoscopic viewing with true depth perception. The aim of this study was to investigate the technical feasibility of trans-oral robotic cleft surgery (TORCS) to access the post

Enhancing Surgical Robotic Skills Through Performance-Guided Training: A Swimmer's Approach for Defining Key Metrics.

BACKGROUND: In swimming, a combination of strength, endurance, and targeted skill training significantly enhances performance, particularly by balancing aerobic and anaerobic conditioning. Similarly, in surgery, improving motor control and stamina during lengthy operations, particularly for precise movements like suturing or tissue manipulation, is essential. This literature review aims to explore the parallels between the training paradigms of elite swimming and robotic-assisted surgical practice, focusing on skill acquisition, training methodologies, and performance-guided feedback systems.

A safe technique for closure of patent ductus arteriosus in adult patients.

A patent ductus arteriosus (PDA) can be closed using various procedures, including transcatheter closure, video-assisted thoracoscopic surgery, and conventional surgery with division and suture. These procedures are feasible for treatment of PDA in a child, but are believed to be challenging when employed in an adult. The main danger is intraoperative bleeding. With a hemorrhage, inadvertent ductus damage/perforation during the isolated ductus, the traditional surgical approach was accomplished with considerable difficulty. This study reports a new procedure for these adult patients using an i

Cannula-assisted sternal displacement for VATS correction of pectus excavatum in three cats.

CASE SERIES SUMMARY: Three skeletally immature cats were treated for pectus excavatum using a modified video-assisted thoracoscopic surgery (VATS) technique that involved using a thoracoscopic cannula to intraoperatively ventrally displace affected sternebrae, thereby assisting the placement of circumsternal sutures while simultaneously protecting underlying viscera. All cases recovered well from their procedure, and the described technique was successful in allowing good radiographic improvement of pectus excavatum in all cases. No intraoperative complications were encountered and all cases w

Minimally invasive parathyroid surgery.

Traditionally, bilateral cervical exploration for localization of all four parathyroid glands and removal of any that are grossly enlarged has been the standard surgical treatment for primary hyperparathyroidism (PHPT). With the advances in preoperative localization studies and greater public demand for less invasive procedures, novel targeted, minimally invasive techniques to the parathyroid glands have been described and practiced over the past 2 decades. Minimally invasive parathyroidectomy (MIP) can be done either through the standard Kocher incision, a smaller midline incision, with video

Deep learning to assess laryngoscope insertion depth during neonatal intubation with video laryngoscopy.

OBJECTIVE: Video laryngoscopy (VL) improves glottic visualization during neonatal intubation, but real-time guidance on blade insertion depth is lacking. We developed a deep learning model to classify insertion depth during VL as shallow, glottic zone, or deep. STUDY DESIGN: A deep learning model was trained on 298,955 annotated frames from 132 neonatal VL videos from two NICUs on one platform with fivefold cross-validation. 31 clinicians were surveyed regarding preferred device feedback modalities. RESULTS: The model detected glottic zone and shallow insertion depths with F1 scores of 0.894

Endoscopic Posterior Cervical Foraminotomy: 2-Dimensional Operative Video.

In the 1940s, the posterior cervical foraminotomy (PCF) was first described. At that time, this technique represented a big step ahead for the treatment of cervical radiculopathy. Rightly, a multitude of studies demonstrated that open microsurgical posterior foraminotomy is an effective treatment for cervical radiculopathy. Open posterior approaches have still the disadvantage of detaching the extensor cervical muscles from the laminae and the spinous processes, which can lead to severe collateral tissue and muscle damage, followed by postoperative complications, such as axial neck pain, shoul
