Continuously monitored vital signs for detection of myocardial injury in high-risk patients - An observational study.

BACKGROUND: Patients are at risk of myocardial injury after major non-cardiac surgery and during acute illness. Myocardial injury is associated with mortality, but often asymptomatic and currently detected through intermittent cardiac biomarker screening. This delays diagnosis, where vital signs deviations may serve as a proxy for early signs of myocardial injury. This study aimed to assess the association between continuous monitored vital sign deviations and subsequent myocardial injury following major abdominal cancer surgery and during acute exacerbation of chronic obstructive pulmonary di

Circadian Rhythmicity of Vital Signs at Intensive Care Unit Discharge and Outcome of Traumatic Brain Injury.

BACKGROUND: Physiological functions with circadian rhythmicity are often disrupted during illness. OBJECTIVE: To assess the utility of circadian rhythmicity of vital signs in predicting outcome of traumatic brain injury (TBI). METHODS: A retrospective single-center cohort study of adult intensive care unit (ICU) patients with largely isolated TBI to explore the relationship between the circadian rhythmicity of vital signs during the last 24 hours before ICU discharge and clinical markers of TBI severity and score on the Glasgow Outcome Scale 6 months after injury (GOS-6). RESULTS: The 130 s

Initial emergency department vital signs may predict PICU admission in pediatric patients presenting with asthma exacerbation.

OBJECTIVE: Severe asthma exacerbations account for a large share of asthma morbidity, mortality, and costs. Here, we aim to identify early predictive factors associated with pediatric intensive care unit (PICU) admission. METHODS: We performed a retrospective observational study of 5,185 emergency department (ED) encounters at a large children's hospital, including 86 (1.7%) resulting in PICU admission between 10/1/2015 and 8/7/2018 with ICD9/ICD10 codes for "asthma," "bronchospasm," or "wheezing." Vital signs and demographic information were obtained from electronic health record data and an

A preliminary assessment of vital-signs-integrated patient-assisted intravenous opioid analgesia (VPIA) for postsurgical pain.

BACKGROUND: We developed a Vital-signs-integrated Patient-assisted Intravenous opioid Analgesia (VPIA) analgesic infusion pump, a closed-loop vital signs monitoring and drug delivery system which embodied in a novel algorithm that took into account patients' vital signs (oxygen saturation, heart rate). The system aimed to allow responsive titration of personalized pain relief to optimize pain relief and reduce the risk of respiratory depression. Moreover, the system would be important to enable continuous monitoring of patients during delivery of opioid analgesia. METHODS: Nineteen patients w

The value of traditional vital signs, shock index, and age-based markers in predicting trauma mortality.

BACKGROUND: Systolic blood pressure (SBP), heart rate (HR), and respiratory rate are poor predictors of trauma outcome. We postulate that HR/SBP (shock index [SI]) and novel new markers SI × age (SIA), SBP / age (BPAI), maximum HR (220 - age) - HR (minpulse [MP]), and HR / maximum HR (pulse max index [PMI]) are better predictors of 48-hour mortality compared with traditional vital signs. METHODS: Data were extracted from the Trauma Audit and Research Network database. Exclusions included any head or spine injury and prehospital intubation or cardiac arrest. Area under receiver operator charac

Race and Sex Differences in Vital Signs Associated with COVID-19 and Flu Diagnoses in Mississippi.

Early detection of viral infections, such as COVID-19 and flu, have potential to reduce risk of morbidity, mortality, and disease transmission through earlier intervention strategies. For example, detecting changes in vital signs have the potential to more rapidly diagnose respiratory virus diseases. The objective of this study was to utilize the University of Mississippi Medical Center's extensive clinical database (EPIC) to investigate associations between temperature, pulse rate, blood pressure (BP), and respiration rate in COVID-19 and flu diagnosed patients. Data from 1,363 COVID-19 (Marc

Continuous In-Bed Monitoring of Vital Signs Using a Multi Radar Setup for Freely Moving Patients.

In hospitals, continuous monitoring of vital parameters can provide valuable information about the course of a patient's illness and allows early warning of emergencies. To enable such monitoring without restricting the patient's freedom of movement and comfort, a radar system is attached under the mattress which consists of four individual radar modules to cover the entire width of the bed. Using radar, heartbeat and respiration can be measured without contact and through clothing. By processing the raw radar data, the presence of a patient can be determined and movements are categorized into
