More than step counts: Slow walking speed, limited running, and fewer long walks predict cardiovascular mortality in the Walk Watch UK Biobank study.

BACKGROUND: Low daily step counts have traditionally been associated with cardiovascular death risk, suggesting other objective real-world gait measures may be complementary or better predictors. This study examined the relationship between real-world walking speed, quality, and walking bout distributions, measured using a wrist-worn device, and cardiovascular death in a large cohort of older people. METHODS: Participants aged 60 to 78 years from the UK Biobank who wore a wrist-worn device were included in this population-based observational cohort study. Gait data were analyzed using Watch W

Wearable-sensor based walking and non-walking measures as progression markers in early to mid-stage Parkinson's disease.

Digital measures of walking and sedentariness may objectively quantify Parkinson's disease (PD) progression. We analyzed longitudinal wearable-sensor data to evaluate the sensitivity and specificity of digital walking and non-walking measures in ambulatory people with PD. We selected 26 individual and 6 composite measures with sufficient sensitivity and test-retest reliability in a development dataset (N = 171). Twenty measures showed significant within-participant changes, and 7 showed larger 2-year effect-size than gold-standard clinical measures in people with early-stage PD (N = 101, mean

Exploring cortical activation patterns during linear vs. curvilinear walking in people with multiple sclerosis using a walking aid.

Multiple Sclerosis (MS) is a chronic disease of the central nervous system characterized by inflammation, demyelination, axon loss and gliosis. Mobility impairment is one of the key clinical symptoms of MS, and a decline in walking abilities can significantly affect quality of life in people with MS (PwMS). As the disease progresses, these symptoms typically worsen, requiring the support of walking aid during gait. Here, we aimed to identify the cortical activation patterns associated with walking in PwMS using walking aids, by means of functional Near-Infrared Spectroscopy (fNIRS). Also, sinc

Effects of simulated reduced gravity and walking speed on ankle, knee, and hip quasi-stiffness in overground walking.

Quasi-stiffness characterizes the dynamics of a joint in specific sections of stance-phase and is used in the design of wearable devices to assist walking. We sought to investigate the effect of simulated reduced gravity and walking speed on quasi-stiffness of the hip, knee, and ankle in overground walking. 12 participants walked at 0.4, 0.8, 1.2, and 1.6 m/s in 1, 0.76, 0.54, and 0.31 gravity. We defined 11 delimiting points in stance phase (4 each for the ankle and hip, 3 for the knee) and calculated the quasi-stiffness for 4 phases for both the hip and ankle, and 2 phases for the knee. The

Effectiveness of an innovative hip energy storage walking orthosis for improving paraplegic walking: A pilot randomized controlled study.

BACKGROUND: The high energy cost of paraplegic walking using a reciprocating gait orthosis (RGO) is attributed to limited hip motion and excessive upper limb loading for support. To address the limitation, we designed the hip energy storage walking orthosis (HESWO) which uses a spring assembly on the pelvic shell to store energy from the movements of the healthy upper limbs and flexion-extension of the lumbar spine and hip and returns this energy to lift the pelvis and lower limb to assist with the swing and stance components of a stride. Our aim was to evaluate gait and energy cost indices fo

Age-Related Differences in Joint Kinematics and Spatiotemporal Parameters During Ramp Walking and Level Walking: Cross-Sectional Study.

BACKGROUND: Gait function is essential for mobility and independence in older adults. Although age-related gait decline on level walking has been studied extensively, there is less information on how aging affects gait during ramp walking, despite its relevance in daily life. Different biomechanical strategies are used during ramp ascent and descent; however, detailed joint kinematics remain unclear, particularly under real-world conditions. OBJECTIVE: This study aims to investigate age-related differences in lower-limb joint angles and spatiotemporal parameters between level and ramp walking

Differences in prolonged walking intensity and duration between older people reporting difficulty, modifications or no difficulty in walking.

Older people report modifying their walking by slowing down speed or using walking aids to accommodate increased exertion and the decline in physical capacity. Self-reported walking modifications and difficulties are associated with daily walking accumulation; however, the relationship between self-reported walking ability and prolonged walking intensity and duration is not well understood. This cross-sectional study compared walking speed, duration, and changes in intensity during prolonged walking bout between older people self-reporting walking difficulty, modifications, or no difficulty in

Relationship between walking motor imagery ability and actual walking ability in stroke hemiplegic patients: A cross-sectional study.

BACKGROUND: In stroke hemiplegic patients (hemiplegic patients), motor imagery ability is associated with physical activity. In particular, kinesthetic imagery may be associated with walking patterns related to muscle activity, as muscular contractions and intrinsic sensations are mentally perceived. However, no studies have examined whether the level of walking imagery ability (WIA) in hemiplegic patients differs from their actual walking ability, including walking patterns. RESEARCH QUESTION: Is there a relationship between WIA and actual walking ability, including walking pattern, in hemip

Walking Stability During Normal Walking and Its Association with Slip Intensity Among Individuals with Incomplete Spinal Cord Injury.

BACKGROUND: Ambulatory individuals with incomplete spinal cord injury (iSCI) experience frequent falls suggesting impairments in their balance control. Individuals with iSCI are more stable during normal walking as compared to able-bodied (AB) individuals; however, it is not known whether this increased stability helps prevent hazardous slips. OBJECTIVE: To compare walking stability during normal walking between iSCI and AB individuals, and to study the association between stability during normal walking and the intensity of an unexpected slip perturbation. DESIGN: Cross-sectional. SETTING:

Visual control of walking using terrain reconstructions.

In rough terrain, walkers must use vision to select viable paths and to find suitable footholds within the time frame allowed by the gait cycle. How is this visuo-motor loop controlled? Recent work demonstrated that walkers look closer to the body in complex terrain, presumably to improve visual information about the terrain structure. However, because the 3D geometry of the terrain was not known, precision of the estimates of gaze and foothold location was limited. Using a 3D modeling technique called photogrammetry, we were able to create 3D representations of the terrain using a head-mounte

[Benefits of dog walking in cardiovascular rehabilitation].

Dog walking is emerging as a potential intervention in cardiovascular health. Meta-analyses and cohort studies suggest an association between dog walking and reduced cardiovascular mortality in patients with established diseases. Beyond physical activity, human-animal interactions favorably influence stress regulation, neuroendocrine balance, exercise motivation, and social connectedness. These synergistic mechanisms remain incompletely quantified, relying on observational associations. Dog walking represents a relevant, low-cost, and sustainable complement to conventional phase III cardiovasc

Electromechanical-assisted training for walking after stroke.

RATIONALE: Walking difficulties are common after a stroke. During rehabilitation, electromechanical and robotic gait-training devices can help improve walking. As the evidence and certainty of the evidence may have changed since our last update in 2020, we aimed to update the scientific evidence on the benefits and acceptability of these technologies to ensure they remain a viable option for stroke rehabilitation. OBJECTIVES: Primary • To determine whether electromechanical- and robot-assisted gait training versus physiotherapy (or usual care) improves walking in adults after stroke. Secondar

Blood flow restricted walking alters gait kinematics.

This study investigated the impact of blood flow restriction (BFR) during treadmill walking on gait kinematics. Twenty-one participants completed one familiarisation and four experimental sessions, including two walking speeds (moderate [5.0 ± 0.3km·h-1] and fast [6.4 ± 0.4km·h-1]) and two occlusion conditions (BFR [60% of arterial occlusion pressure] and unrestricted). For each exercise intensity, the BFR session was performed first. Participants were instructed to walk as long as possible, with sessions capped at 20 min. Unrestricted sessions were time-matched, and the order of exercise inte

Emotional states affect steady state walking performance.

Gait is a large component and indicator of health. Many factors affect gait including age, disease, and even mood disorders. Few studies have looked at the influence of emotional states on gait. This study aimed to investigate the influence of emotional states on walking performance to understand whether an emotional state may be an important factor to consider when evaluating gait. Thirty-six young adults were recruited (23F, 13M) and performed a neutral baseline condition of walking which included six passes of walking across an 8m walkway (a total of 48m of walking). Participants then compl
