[Pathophysiology of dyspnea].

The sensation of dyspnea is related to pathologically increased breathing work as a result of hyperventilation and/or increased airway resistance. Low cardiac output resulting in tissue hypoxia provokes hyperventilation. In diffuse interstitial lung diseases, hyperventilation is combined with severely reduced lung compliance. In severe airway obstruction, hypoventilation is the result of excessively enhanced breathing work. Habituation and acute changes of situation have an important bearing on the subjective sensation of dyspnea.

In vivo physiologic comparison of two ventilators used for domiciliary ventilation in children with cystic fibrosis.

OBJECTIVE: Home noninvasive mechanical ventilation (NIMV) is used with increasing frequency for the treatment of patients with respiratory failure caused by cystic fibrosis, yet the optimal mode of ventilation in such children is unknown. We compared the physiologic short-term effects of two ventilators with different modes (one pressure support and the other assist control/volume-targeted [AC/VT]) commonly used for domiciliary ventilation. DESIGN: Prospective, randomized, crossover comparison of two ventilators with different modes. SETTING: Tertiary pediatric university hospital. PATIENTS

Positive effects of early continuous positive airway pressure on pulmonary function in extremely premature infants: results of a subgroup analysis of the COIN trial.

OBJECTIVE: Early continuous positive airway pressure (CPAP) may reduce lung injury in preterm infants. PATIENTS AND METHODS: Spontaneously breathing preterm infants were randomised immediately after birth to nasal CPAP or intubation, surfactant treatment and mechanical ventilation. Pulmonary function tests approximately 8 weeks post-term determined tidal breathing parameters, respiratory mechanics and functional residual capacity (FRC). RESULTS: Seventeen infants received CPAP and 22 mechanical ventilation. Infants with early CPAP had less mechanical ventilation (4 vs 7.5 days; p=0.004) and

P0.1 is an Unreliable Measure of Effort in Support Mechanical Ventilation in Comparison With Esophageal-Derived Measures of Effort: A Comparison Study.

OBJECTIVE: Respiratory effort should be assessed in patients on mechanical ventilation in support mode. With the insertion of esophageal balloons, it is possible to measure different parameters of effort, such as change in esophageal pressure (ΔPes), work of breathing (WOB), and pressure-time product (PTP). Although some studies advocate for the use of P0.1 as a measure of effort, there is significant controversy as it is considered more a measure of respiratory drive. This study investigates the correlation between P0.1 and esophageal-derived parameters of effort. DESIGN: This was a retrospe

The hyperpnoea of exercise in health: Respiratory influences on neurovascular control.

NEW FINDINGS: What is the topic of this review? Elevated demand is placed on the respiratory muscles during whole-body exercise-induced hyperpnoea. What is the role of elevated demand in neural modulation of cardiovascular control in respiratory and locomotor skeletal muscle, and what are the mechanisms involved? What advances does it highlight? There is a sympathetic restraint of blood flow to locomotor muscles during near-maximal exercise, which might function to maintain blood pressure. During submaximal exercise, respiratory muscle blood flow might be also be reduced if ventilatory load is

Physiological changes occurring with positive pressure ventilation: Part Two.

Although the physiological effects of positive pressure ventilation are numerous, sometimes undesirable and have varying degrees of significance, positive pressure ventilation still plays a major role in the resuscitation and treatment of critically ill patients. Advances in the various methods of delivering positive pressure, especially when incorporating spontaneous breathing, have reduced the severity of complications. Despite serious complications, mechanical ventilation has advantages. When it is instituted for ventilatory and hypoxaemic respiratory failure, the benefits can be viewed in

Effect of feeding on ventilation and respiratory mechanics in newborn infants.

Measurements of ventilation and respiratory mechanics were made before and after tube feeding in 24 infants. In 12 infants with the respiratory distresssyndrome tidal volume tended to fall after feeding; as the respiratory rate increased after feeding; as the respiratory rate increased after feeding, minute ventilation remained unchanged. Hypoventilation is therefore unlikely to be the cause of hypoxaemia after feeding. Compliance, resistance, and the work of breathing showed no changes after feeding. In 12 healthy infants feeding had no effects on pulmonary function. There was a slight rise i

Bedside assessment of respiratory drive and effort: The role and limitations of non-invasive occlusion maneuvers.

Accurate assessment of respiratory drive and effort is important in critically ill patients receiving invasive or noninvasive mechanical ventilation. This review summarizes the physiology of breathing, with particular emphasis on respiratory drive, inspiratory effort, and work of breathing and on the clinical significance of their measurement. Reference techniques are discussed, but the main focus is on noninvasive airway occlusion maneuvers (P0.1, Pocc, and PMI) as bedside surrogates of respiratory drive and effort. Patient variability and technical factors may lead to misinterpretation and l

Interrater agreement in the clinical evaluation of acute pediatric asthma.

Prior studies of observer agreement of the clinical exam of children with asthma have focused on small numbers of specially trained observers, often in the setting of clinical trials. Our objective was to evaluate interobserver reliability in the physical exam of acute pediatric wheezing and asthma among a large group of diverse examiners, in a setting of routine clinical practice, and without prior special training. The setting was a large urban children's hospital. Observers were attending pediatric emergency physicians and fellows; hospital respiratory therapists; and emergency department (

Oxygen cost of exercise hyperpnea: implications for performance.

We addressed two questions concerned with the metabolic cost and performance of respiratory muscles in healthy young subjects during exercise: 1) does exercise hyperpnea ever attain a "critical useful level"? and 2) is the work of breathing (WV) at maximum O2 uptake (VO2max) fatiguing to the respiratory muscles? During progressive exercise to maximum, we measured tidal expiratory flow-volume and transpulmonary pressure- (Ptp) volume loops. At rest, subjects mimicked their maximum and moderate exercise Ptp-volume loops, and we measured the O2 cost of the hyperpnea (VO2RM) and the length of time

The work-rate-dependent effect of carbon monoxide on ventilatory control during exercise.

The effect of low levels of carbon monoxide (CO) was studied during exercise in order to determine the work-rate dependent effect of CO breathing on exercise ventilation (VE). Ten normal subjects (aged 32.8 +/- 7.1 years) were studied during air breathing and air with added CO to bring carboxyhemoglobin (COHb) to approximately 11% and 20%. During the incremental exercise test, VE was not affected by CO breathing at work rates below the lactic acidosis threshold (LAT), defined as the O2 uptake above which CO2 is excreted by the lungs consequent to buffering of metabolic acid (not hyperventilati

Oxygen cost of exercise hyperpnoea is greater in women compared with men.

KEY POINTS: The oxygen cost of breathing represents a significant fraction of total oxygen uptake during intense exercise. At a given ventilation, women have a greater work of breathing compared with men, and because work is linearly related to oxygen uptake we hypothesized that their oxygen cost of breathing would also be greater. For a given ventilation, women had a greater absolute oxygen cost of breathing, and this represented a greater fraction of total oxygen uptake. Regardless of sex, those who developed expiratory flow limitation had a greater oxygen cost of breathing at maximal exerci

Does night work favor sleep-related accidents in police officers?

Several studies of sleep-breathing physiology have suggested that sleep deprivation may worsen obstructive sleep apnea. The aim of the study was to determine the direct effect of night work on breathing variables during sleep in fast-rotating shift workers. Twenty one men - police officers, fast-rotating shift workers, underwent polysomnography on 2 occasions: under a normal sleeping condition after day work and after sleep deprivations after night work. Both sleep studies were conducted within 2 to 3 weeks of each other. Approximately half of the group was tested under control conditions (day

The effect of proportional assist ventilation on the electrical activity of the human diaphragm during exercise.

NEW FINDINGS: What is the central question of this study? What is the effect of lowering the normally occurring work of breathing on the electrical activity and pressure generated by the diaphragm during submaximal exercise in healthy humans? What is the main finding and its importance? Ventilatory assist during exercise elicits a proportional lowering of both the work performed by the diaphragm and diaphragm electrical activity. These findings have implications for exercise training studies using proportional assist ventilation to reduce diaphragm work in patients with cardiopulmonary disease

Virtual respiratory system in investigation of CPAP influence on optimal breathing frequency in obstructive lungs disease.

BACKGROUND: Continuous Positive Airway Pressure (CPAP) is a commonly accepted method of spontaneous breathing support in obstructive lung disease. Previous work suggested that the cause of the CPAP efficacy in the obstructive lung disease localized in bronchi of middle order (OLDMO) is not as obvious as, for example, in the obstructive sleep apnea. Since CPAP reduces obstruction and the optimal breathing frequency (BF) depends on the obstruction level, it seems to be important to analyze the dependence of the optimal BF on CPAP. AIM: To analyze the support efficacy cause in OLDMO, esp. the re
