Cell and Gene Therapy for Patients Suffering from Xerostomia (Dry Mouth): Positioning Extracellular Vesicles as the Bridge Between Biomarker Discovery and Regenerative Therapy in Xerostomia-A Scoping Review.

Xerostomia is a common and debilitating condition caused by salivary gland dysfunction, frequently associated with primary Sjögren's syndrome and head and neck radiotherapy. Current management is largely symptomatic and does not address underlying glandular injury. Extracellular vesicles (EVs), including exosomes, have emerged as candidate mediators of intercellular communication that have been proposed for diagnostic and therapeutic applications; however, their translational relevance to xerostomia remains uncertain and is currently supported only by exploratory evidence. This scoping review

Umbrella review: Summary of findings for acupuncture as treatment for radiation-induced xerostomia.

Acupuncture is a common part of complementary medicine. However, traditional Chinese acupuncture has been built on concepts which are not in accordance with science-based medicine. Our aim was to critically assess the evidence presented in systematic reviews (SRs) and meta-analyses (MAs) about the effectiveness of acupuncture on radiotherapy induced xerostomia. A systematic search was conducted in February 2022. We evaluated all SRs/MAs using the AMSTAR-2 instrument. Eight SRs, with 12 controlled studies were included. All SRs summarize that the methodological drawbacks do not allow to give a

Geometric Image Biomarker Changes of the Parotid Gland Are Associated With Late Xerostomia.

PURPOSE: To identify a surrogate marker for late xerostomia 12 months after radiation therapy (Xer12m), according to information obtained shortly after treatment. METHODS AND MATERIALS: Differences in parotid gland (PG) were quantified in image biomarkers (ΔIBMs) before and 6 weeks after radiation therapy in 107 patients. By performing stepwise forward selection, ΔIBMs that were associated with Xer12m were selected. Subsequently other variables, such as PG dose and acute xerostomia scores, were added to improve the prediction performance. All models were internally validated. RESULTS: Predic

Anatomic study of submandibular gland transfer in an attempt to prevent postradiation xerostomia.

Xerostomia is the most common negative sequela after radiotherapy for head and neck cancer and results from destruction of normal salivary gland tissue by radiation. We hypothesized that transferring a single submandibular gland outside the radiation field to the submental region may preserve its function, thus preventing xerostomia and its consequences. This anatomic study was designed to evaluate different methods of submandibular gland transfer to the submental region. Eight submandibular glands were dissected in four fresh human cadavers. The anatomy and blood supply of the region were doc

Oral pilocarpine: a review of its pharmacological properties and clinical potential in xerostomia.

Pilocarpine is a cholinergic agonist which stimulates salivary secretion both in individuals with normal salivary gland function and in those with impaired salivary flow (xerostomia or oral dryness). A rapid increase in salivary flow rate is observed following oral pilocarpine administration and peak levels are maintained for at least 1 to 2 hours. Mean salivary flow rates after administration of pilocarpine are 2- to 10-fold higher than after placebo, and no evidence of tolerance to the pharmacological effects of the drug has been observed during prolonged administration for up to 5 months. [The Saxon test for objective assessment of xerostomia. A contribution to the diagnosis of Sjögren's syndrome].

In 100 patients with rheumatoid arthritis and in a control group of 100 test persons with healthy joints who were exactly equal concerning age and sex with the help of a simple method (back-weighing of a sponge after defined retention time in the oral cavity) the secretion of the saliva was measured. According to the test variant in 9-10 patients and 0-1 controls, respectively, pathologic values were found. In comparison to other symptoms which are characteristic for the presence of a Sjogren syndrome, an average sensitivity and high specifity could be ascribed to the test for the evidence of
