A clinical study of changes in the volume of bone grafts in the atrophic maxilla.

OBJECTIVES: To evaluate the volumetric changes in bone grafts to the severely atrophic edentulous maxilla over 6 months. METHODS: Ten edentulous patients received autogenous bone as buccal onlay grafts to the maxilla and bilaterally particulate bone grafts to the maxillary sinus. Two mm contiguous axial CT scans were obtained 1 to 2 weeks and 6 to 7 months postoperatively. The area of each graft was measured and the volume calculated according to the formula V(graft)=Sigma of areas x the thickness of each section. RESULTS: The volume of the inlay and onlay grafts was reduced by an average of

Rehabilitation of an edentulous atrophic maxilla with four unsplinted narrow diameter titanium-zirconium implants supporting an overdenture.

The edentulous maxilla is often affected by bone resorption, sometimes making it difficult to place standard diameter implants. Narrow diameter implants made of titanium-zirconium (Ti-Zr) alloy, which has superior mechanical properties compared with titanium, have been proposed for these difficult situations. This retrospective clinical observation reports the outcome of the use of reduced diameter implants made of Ti-Zr alloy supporting maxillary overdentures retained with locator abutments. The charts of all patients who received maxillary overdentures supported by four unsplinted implants f

Microcomputed tomographic and histologic analysis of anorganic bone matrix coupled with cell-binding peptide suspended in sodium hyaluronate carrier after sinus augmentation: a clinical study.

PURPOSE: Anorganic bovine hydroxyapatite matrix (ABM), when coupled with synthetic cell-binding peptide (P15), mimics the cell-binding region of type 1 collagen and is commercially available suspended in a sodium hyaluronate carrier. The aim of the present study, therefore, was to test the efficacy of ABM/P-15 Putty (DENTSPLY Friadent CeraMed) as a sole graft material for sinus augmentation in patients with severely resorbed posterior maxillae. MATERIALS AND METHODS: Sinus augmentation was performed in 10 patients using ABM/P-15 Putty and two provisional dental implants (3.0 mm in diameter).

Transalveolar maxillary sinus floor elevation using osteotomes with or without grafting material. Part II: Radiographic tissue remodeling.

OBJECTIVES: To evaluate the pattern of tissue remodeling after maxillary sinus floor elevation using the transalveolar osteotome technique with or without utilizing grafting materials. METHODS: During the period of 2000-2005, 252 Straumann dental implants were inserted using the transalveolar sinus floor elevation technique in a group of 181 patients. For 88 or 35% of those implants, deproteinized bovine bone mineral with a particle size of 0.25-1 mm was used as the grafting material, but for the remaining 164 implants, no grafting material was utilized. Periapical radiographs were obtained w

Observations on a new collagen barrier membrane in 16 consecutively treated patients. Clinical and histological findings.

BACKGROUND: Space-maintaining capacity, cell disclusive potential, and stability over time are crucial factors to achieving sufficient bone augmentation with membrane barriers. The case series presented here assessed a new collagen barrier used in bone augmentation. Clinically, the healing pattern, especially in cases of secondary healing, was studied. METHODS: Soft tissue healing was documented by photographs, and the size of the dehiscences calculated by image analysis. The measurements were performed on digitized photographs. During reentry, barrier remnants were dissected and histological

Bimaxillary protrusion with an atrophic alveolar defect: orthodontics, autogenous chin-block graft, soft tissue augmentation, and an implant.

Bimaxillary protrusion in a 28-year-old woman was complicated by multiple missing, restoratively compromised, or hopeless teeth. The maxillary right central incisor had a history of avulsion and replantation that subsequently evolved into generalized external root resorption with Class III mobility and severe loss of the supporting periodontium. This complex malocclusion had a discrepancy index of 21, and 8 additional points were scored for the atrophic dental implant site (maxillary right central incisor). The comprehensive treatment plan included extraction of 4 teeth (both maxillary first p

Surgical and prosthetic management of ridge deficiency for an implant-supported restoration in the esthetic zone.

This clinical report presents the surgical and prosthetic management of a patient with complex needs involving anterior implants. After extraction and ridge augmentation, unexpected ridge resorption occurred. Two implants were placed in conjunction with an allograft skin material. Upon healing, 2 additional soft tissue augmentation procedures were performed with autologous pedicle connective tissue grafts to correct the residual ridge deficiency. At the second stage surgery, a modified flap approach was used to further enhance the soft tissue volume and contour. Definitive implant-supported sp

A histologic and clinical evaluation of ridge preservation following grafting with demineralized bone matrix, cancellous bone chips, and resorbable extracellular matrix membrane.

In an attempt to reduce postextraction alveolar bone resorption, ridge preservation grafting procedures with or without resorbable membranes have become standard-of-care treatments following tooth removal. This prospective case series examined histologic and clinical outcomes following socket grafting with a syringeable paste allograft and a resorbable extracellular matrix membrane at three different time periods following postextraction grafting: 6, 12, and 24 weeks. At each time period, bone core specimens were retrieved for microscopic examination, and implants were placed. Following prosth

Effect of platelet-rich plasma on sinus lifting: a randomized-controlled clinical trial.

OBJECTIVE: The combination of anorganic bovine bone (ABB) with platelet-rich plasma (PRP) has been widely used in bone regeneration procedures although its benefits are still unclear. The purpose of this study was to evaluate whether or not PRP improves the efficacy of ABB in sinus floor augmentation. In addition, we have investigated the effect of residual bone height and tobacco on implant survival in sinus augmentation procedures. PATIENT AND METHODS: Eighty-seven patients recruited for this study underwent 144 sinus floor augmentation procedures using ABB alone or ABB plus PRP (ABB+PRP) i

Orthodontic-periodontal interactions: Orthodontic extrusion in interdisciplinary regenerative treatments.

Orthodontics is a periodontal treatment. "Guided orthodontic regeneration" (GOR) procedures use orthodontic movements in perio-restorative patients. The GOR technique includes a guided orthodontic "soft tissue" regeneration (GOTR) and a guided orthodontic "bone" regeneration (GOBR) with a plastic soft tissue approach and a regenerating reality. The increased amount of soft tissue gained with orthodontic movement can be used for subsequent periodontal regenerative techniques. The increased amount of bone can as well improve primary implant stability and, eventually, simplify a GTR technique to
