Congenital anorectal atresia: MR imaging of late post-operative appearances in adult patients with anal incontinence.

OBJECTIVES: To describe the MR imaging findings in adults presenting with anal incontinence following pull-through perineoplasty for anorectal atresia. METHODS: 15 adults (12 male, 3 female; age 22-52 years) with anal incontinence following a prior perineal pull-through procedure as an infant for anorectal atresia were identified retrospectively. MR imaging was performed using either an endoanal coil or body coil. MR images were reviewed by three observers who noted whether pelvic floor and sphincter muscles were present and, if so, whether they were thinned or not. Data were tabulated and ra

Exploring quality of life in Italian patients with rare disease: a computer-aided content analysis of illness stories.

The present study used a narrative-based approach to identify common themes that characterized the illness experience and quality of life of patients affected by rare disease (RD). Textual data were comprised of illness stories written by 32 adult Italian patients (eight men and 24 women), with the following RD diagnoses: amyotrophic lateral sclerosis (n = 12), anorectal atresia (n = 4), Poland syndrome (n = 4), and idiopathic pulmonary hypertension (n = 12). Computer-aided content analysis was performed to detect the main themes (cluster analysis) and latent factors (correspondence analysis)

MRI findings in patients with defecatory dysfunction after surgical correction of anorectal malformation.

BACKGROUND: Postoperative anorectal malformation patients frequently have defecatory dysfunction. MRI may be useful in the management of these patients. OBJECTIVE: To analyze static and dynamic MRI findings in patients with defecatory dysfunction after correction of anorectal malformation (ARM), and compare differences between patients with constipation and fecal incontinence. MATERIALS AND METHODS: Pelvic MRI studies of 20 constipated and 32 incontinent postoperative ARM patients were analyzed retrospectively to determine the location and morphology of the neorectum, presence of peritoneal

Longitudinal study of lower urinary tract function in children with anorectal malformation.

UNLABELLED: Neurogenic bladder dysfunction (NBD) in children with anorectal malformation (ARM) has been reported in many publications, but with variations in prevalence and connection to type of ARM and spinal cord/sacral malformations. The prevalence of ARM patients with functional nonneurogenic dysfunction, on the contrary, has been addressed only sparsely. Since constipation and soiling have been shown to often accompany functional urinary tract symptoms in children without malformations, and treatment of constipation often improves bladder symptoms, it is interesting to assess such a conne

Postoperative complications in adults with anorectal malformation: a need for transition. German Network for Congenital Uro-REctal Malformations (CURE-Net).

PURPOSE: The aim of the German Network for Congenital Uro-REctal malformations (CURE-Net) is to collect data of affected patients with anorectal malformation (ARM) to investigate molecular causes, clinical implications and psychosocial outcome. The current issue was to examine the transition to adulthood in adults with ARM and to explore condition-related needs and skills required. METHODS: This qualitative study is part of a larger multi-center study of clinical queries and quality of life in patients with ARM. The guided interview focused on the analysis of medical data and personal questio

Cloacal exstrophy--pull-through or permanent stoma? A review of 53 patients.

PURPOSE: Patients with cloacal exstrophy have complex anomalies of the genitourinary and gastrointestinal tract with a spectrum of colonic length. Often, colon is lost during the initial management by use of ileostomies and for urologic and genital reconstruction. It is a common belief that these patients require permanent stomas, which we hypothesized is inaccurate, and therefore reviewed our experience with exstrophy, focusing specifically on a patient's potential to undergo a colonic pull-through. METHODS: All patients with exstrophy or exstrophy variant treated by the authors were retrosp

The Peristeen Anal Plug: a modern solution for an embarrassing problem.

Faecal incontinence is a problem that affects over 500,000 people in the UK, but unlike urinary incontinence is rarely spoken about or adequately addressed. The problem may arise from a range of causes, including obstetric trauma, surgery and neurological disorders. Like urinary incontinence, faecal incontinence can be managed. This article discusses one management option--the Peristeen Anal Plug--and illustrates with case histories the beneficial effect proper management of faecal incontinence can have on patients' lives.

Sigmoid irrigation tube for the management of chronic evacuation disorders.

BACKGROUND/PURPOSE: Antegrade colonic irrigation, in which the right colon is accessed via appendicostomy or cecostomy, now is an important adjunct in the management of children with chronic evacuation disorders. However, in most children, the major area of dysfunction is the left rather than the right colon. The authors developed a simple, percutaneous endoscopic, laparoscopically controlled sigmoid irrigation tube placement and evaluated the results in 4 children. METHODS: A rigid sigmoidoscope is advanced into the upper sigmoid and the loop brought in contact with the abdominal wall under

A novel syndrome involving primary skeletal growth and retardation in siblings.

An identical pattern of malformations was found in two brothers both having microcephaly and severe developmental delay. Additionally, they had hypotelorism, epicanthic folds, and convergent strabismus. There was shortening of either the radius or the tibia and shortening of the first metacarpals. Persistently dorsally flexed fingers and toes were noted, all of which are unusually long. Both boys had a high-pitched voice and were unable to communicate verbally at the age of 4.5 years. They both developed short stature. One brother has anal atresia; the other had a pulmonary artery atresia, VSD

[Atypical cat eye syndrome. Fluorescence in situ hybridization of metaphase chromosomes].

In 1983, a chromosome analysis was carried out in a newborn preterm infant with minor anomalies (preauricular skin tag, maldescensus testis). All analysed metaphases showed a small extra chromosome, which was symmetric, dicentric and bi-satellited. In spite of in depth analysis, its origin remained obscure. Recent re-evaluation using fluorescence in situ hybridization (FISH) led to the diagnosis of a dicentric chromosome 22. The FISH technique is an important new tool in chromosome diagnostics. The phenotype of this infant only vaguely resembles the cat eye syndrome. The syndrome should be dia

Urinary incontinence in children: treatment with free autogenous muscle transplantation.

Free autogenous muscle transplantation has now been carried out in 13 patients with anal incontinence and 16 patients with urinary incontinence. In anal incontinence the results have been excellent; all patients were improved and 12 out of 13 reached an acceptable level of continence. In 10 patients with urinary incontinence the results were good, with increase of functional bladder capacity and abandonment of nappies during the day. Two patients were improved and 4 were failures. Cinematographic studies in both anal and urinary incontinence clearly demonstrated the muscular activity of the tr

Multiple Hemivertebrae: The Natural History and Treatment of 50 Patients.

OBJECTIVE: Multiple hemivertebrae (MHV) is defined as three or more hemivertebrae, and is relatively uncommon among patients with congenital scoliosis. This study aimed to compare the natural history of different kinds of MHV and describe the surgical outcome of MHV. METHODS: In this retrospective cohort study, a total of 50 patients diagnosed with MHV were enrolled from June 2007 to June 2018. The medical records and radiographs of these patients were reviewed to summarize the characteristics of MHV. Patients with MHV were divided into the unbalanced (UB) group, partially unbalanced (PUB) gr

Cloacal exstrophy: an epidemiologic study from the International Clearinghouse for Birth Defects Surveillance and Research.

Cloacal exstrophy presents as a complex abdominal wall defect thought to result from a mesodermal abnormality. Anatomically, its main components are Omphalocele, bladder Exstrophy and Imperforate anus. Other associated malformations include renal malformations and Spine defects (OEIS complex). Historically, the prevalence ranges from 1 in 200,000 to 400,000 births, with higher rates in females. Cloacal exstrophy is likely etiologically heterogeneous as suggested by its recurrence in families and occurrence in monozygotic twins. The defect has been described in infants with limb-body wall, with

Thoracoscopic repair of esophageal atresia and tracheoesophageal fistula: a multi-institutional analysis.

OBJECTIVES: For the past 60 years, successful repair of esophageal atresia (EA) and distal tracheoesophageal fistula (TEF) has been performed via a thoracotomy. However, a number of reports have described adverse musculoskeletal sequelae following thoracotomy in infants and young children. Until now, only a few scattered case reports have detailed an individual surgeon's success with thoracoscopic repair of EA/TEF. This multi-institutional review represents the largest experience describing the results with this approach. METHODS: A cohort of international pediatric surgeons from centers that

Combined Laparoscopic and Posterior Approach Resection of Presacral Neuroblastoma with Rectobulbar Urethral Fistula.

INTRODUCTION: We herein report a case of presacral neuroblastoma (NB) with a rectobulbar urethral fistula. We successfully resected the tumor with a combined laparoscopic and posterior approach and simultaneously performed posterior sagittal anorectoplasty (PSARP). CASE PRESENTATION: A 1-year-old boy underwent laparoscopic surgery. He had a surgical history of transverse colostomy for an imperforate anus (later diagnosed as a rectobulbar urethral fistula) on the 2nd day after birth. Before radical surgery for a rectobulbar urethral fistula at 1 year of age, an imaging study incidentally showe

Müllerian duct anomalies with term pregnancy: a case report.

BACKGROUND: Müllerian duct anomaly is a rare condition. Many cases remain unidentified, especially if asymptomatic. Thus, it is difficult to determine the actual incidence. Müllerian duct anomaly is associated with a wide range of gynecological and obstetric complications, namely infertility, endometriosis, urinary tract anomalies, and preterm delivery. Furthermore, congenital anomalies in pregnant mothers have a high risk of being genetically transmitted to their offspring. CASE PRESENTATION: We report a case of a patient with unsuspected müllerian duct anomaly in a term pregnancy. A 33-year
