Postoperative Nocardia cyriacigeorgica infection after glioblastoma resection: correlating metagenomic next-generation sequencing with conventional microbiology.

Postoperative nocardial infection after cranial surgery is rare and difficult to diagnose because Nocardia spp. grow slowly in conventional culture. Metagenomic next-generation sequencing (mNGS) can shorten the interval to microbial detection, but read abundance is influenced by commensal DNA and by contamination during sampling, laboratory processing, and sequencing. mNGS findings therefore require correlation with conventional microbiology and with the clinical context. An 84-year-old woman developed purulent surgical wound discharge with epidural empyema approximately 2 weeks after resectio

An Assessment of C-Reactive Protein and Erythrocyte Sedimentation Rate in Ruling Out Acute Infectious Spinal Pathology in Emergency Department Patients: A Retrospective Cohort Study.

OBJECTIVES: Acute infectious spinal pathologies (AISP), including discitis, osteomyelitis, spinal epidural abscess, and septic arthritis are rare, carry significant morbidity and mortality, and are difficult to diagnose. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) are frequently elevated in AISP, although there are no consensus recommendations on using them. This retrospective study aims to calculate the biomarkers' sensitivity to rule out AISP. METHODS: We retrospectively reviewed emergency department patients from a national hospital system database over 4 years who pr

Description and outcomes of patients with substance use disorder with serious bacterial infections who had a multidisciplinary care conference.

BACKGROUND: Patients with substance use disorders (SUDs) and severe bacterial infections requiring prolonged antibiotic therapy represent a significant challenge to providers due to complexity of care coordination required to ensure safe and effective treatment. Our institution developed a patient-centered multidisciplinary discharge planning conference, OPTIONS-DC, to address this challenge. METHODS: We conducted a retrospective review to evaluates parameters between patients who received an OPTIONS-DC and those who did not. RESULTS: We identified 73 patients receiving an OPTIONS-DC and 100

Surgical management and outcome of tuberculous atlantoaxial dislocation: a 15-year experience.

OBJECTIVE: Tuberculous atlantoaxial dislocation is a rare disease entity. However, tuberculosis continues to be endemic in developing countries. Its earliest clinical presentation may be nonspecific, and delay in diagnosis may lead to irreversible neurological deficit. The management of tuberculous atlantoaxial dislocation includes ventral cervicomedullary decompression, occipitocervical arthrodesis, and administration of antituberculous medications. METHODS: Eighteen patients with tuberculous atlantoaxial dislocation who presented with neck pain and/or occipital headache, restriction of neck

Clinical consequences of infected arteriovenous grafts in hemodialysis patients.

Arteriovenous (AV) graft infection is a serious adverse event in hemodialysis patients; however, there is little published literature describing its consequences. We identified prospectively all AV graft infections occurring at our institution during a 4.5-year period. We analyzed immediate complications, as well as long-term consequences, including the need for subsequent vascular-access procedures and duration of catheter-dependent dialysis therapy. Ninety graft infections were identified in 78 patients, yielding a rate of 8.2 infections/100 graft-years. Patients with graft infection were mu

Vertebral remodeling with denosumab after infection control in Escherichia coli spondylodiscitis: case report.

Infectious spondylodiscitis can cause progressive vertebral destruction and instability even after infection control, and some patients decline or are unsuitable for surgery. We report a 67-yr-old man with poorly controlled type 2 diabetes mellitus and hypertension who developed Escherichia coli bacteremia and subsequent infectious spondylodiscitis. Four months after treatment of a traumatic open tibial wound with debridement and split-thickness skin grafting, he presented with progressive low back pain, intermittent fever, and systemic illness with acute kidney injury and hepatic dysfunction.

Surgical Management of Pott's Paraplegia in a 27-Week Pregnant Woman with Continuation of Pregnancy: A Case Report.

INTRODUCTION: Spinal tuberculosis is primarily managed with antitubercular therapy, while surgical intervention is reserved for cases with neurological compromise or spinal instability. The occurrence of Pott's paraplegia during pregnancy is rare and presents unique diagnostic and therapeutic challenges due to concerns regarding fetal safety, imaging limitations, and surgical risks. Management during the second trimester is particularly complex, as options, such as medical termination or delivery are not feasible. Only a limited number of case reports describing Pott's paraplegia during pregna

Predictive Performance of MRI for Antibiotic Treatment Failure of Pyogenic Vertebral Osteomyelitis: A Validation Study.

INTRODUCTION: Intravenous antibiotics are the primary treatment of choice for pyogenic vertebral osteomyelitis (PVO). Surgical intervention is required when the initial antibiotic treatment fails but is often difficult to perform, especially in older adults with multiple comorbidities, because of the reduced physical activity. The size of the infection signal in the spinal bone on magnetic resonance imaging (MRI) at the time of diagnosis was reported to have a high predictive accuracy for antibiotic treatment failure. However, the sample size was too small for this result to be adopted in clin

Social and structural determinants of injecting-related bacterial and fungal infections among people who inject drugs: protocol for a mixed studies systematic review.

INTRODUCTION: Injecting-related bacterial and fungal infections are a common complication among people who inject drugs (PWID), associated with significant morbidity and mortality. Invasive infections, including infective endocarditis, appear to be increasing in incidence. To date, preventive efforts have focused on modifying individual-level risk behaviours (eg, hand-washing and skin-cleaning) without much success in reducing the population-level impact of these infections. Learning from successes in HIV prevention, there may be great value in looking beyond individual-level risk behaviours t

Intentional Self-Inflicted Transcranial Screwdriver Injury: A Case Report.

Transcranial screwdriver injuries are rare forms of low-velocity penetrating cranial trauma that are most often described after interpersonal violence and may result in neurologic impairment or death. These injuries can be difficult to recognize when external wounds are small and may be incompletely characterized on initial imaging, particularly when retained metal produces artifact. Plain radiography and computed tomography (CT) are useful for defining foreign-body trajectory and guiding operative planning, but the full severity of injury may only become apparent through serial imaging and su

Real-World Use of Dalbavancin in a United States Tertiary Referral Center.

Introduction Outpatient management of serious bacterial infections can be difficult particularly in situations where outpatient parenteral antibiotic therapy may be difficult due to patient-specific factors such as history of medical non-compliance, unstable housing situations, and individuals who use injection drugs. Dalbavancin is a long-acting lipoglycopeptide antibiotic currently approved for the treatment of bacterial skin and soft tissue infections; however, it is often employed in the management of other infections caused by gram-positive organisms. Data regarding the off-label usage of

Treatment of Staphylococcus aureus Infections.

Staphylococcus aureus, although generally identified as a commensal, is also a common cause of human bacterial infections, including of the skin and other soft tissues, bones, bloodstream, and respiratory tract. The history of S. aureus treatment is marked by the development of resistance to each new class of antistaphylococcal antimicrobial drugs, including the penicillins, sulfonamides, tetracyclines, glycopeptides, and others, complicating therapy. S. aureus isolates identified in the 1960s were sometimes resistant to methicillin, a ß-lactam antimicrobial active initially against a majority

Percutaneous Endoscopic Surgery Alone to Treat Severe Infectious Spondylodiscitis in the Thoracolumbar Spine: A Reparative Mechanism of Spontaneous Spinal Arthrodesis.

BACKGROUND: Infective spondylodiscitis has been treated solely with antibiotics based on the pathogen identified. Surgical intervention was used in cases of unidentified pathogens, failed antibiotic treatment, neurological deficit, or instability. The standard surgical procedure was debridement and interbody fusion with a bone graft through the anterior approach, followed by posterior instrumentation. Recently, percutaneous endoscopic surgery has been proven to be safe and effective for treating infectious spondylodiscitis. The results of endoscopy surgery treatment alone for infectious spondy
