Transition management as an intervention for survivor syndrome.

In today's health care environment of merged organizations, downsizing and restructuring, employees can be experiencing a debilitating syndrome called "layoff survivor syndrome." This syndrome can have a crippling effect on workers and organizations as employees struggle to adapt to the changed working environment. This article represents my self-reflection as a nursing unit manager who personally experienced survivor sickness and witnessed its impact on the unit staff that I was leading at the time. The work of Noer (1993) is explored to clarify the syndrome and describe how the nursing staff

Role, impact, and feasibility of a comprehensive pharmacist medication review for patients in the older adult cancer clinic (OACC) at Princess Margaret Cancer Centre.

INTRODUCTION: Older adults with cancer are at greater risk of experiencing drug therapy problems (DTPs), often related to multiple comorbidities, advanced age, and cancer treatments. Increasing evidence demonstrates the value of a pharmacist in geriatric oncology clinics, but little information exists on their practical integration. Our objective is to evaluate the logistics and impact of a pharmacist review of patients referred to a geriatric oncology clinic at an academic cancer centre. MATERIALS AND METHODS: Starting January 2023, a one-year quality improvement pilot project integrated a p

Identifying Barriers to Fall Prevention for Community-Dwelling Older Adults in Delaware:: A Qualitative Study.

OBJECTIVE: Falls are a major public health concern among older adults in the United States, and Delaware reflects this growing burden. Fall-related mortality increased steadily from 2012 to 2021, coinciding with a rapidly aging population. Despite the availability of effective evidence-based fall-prevention programs, adoption remains limited among high-risk populations. Grounded in implementation science, this study aimed to identify barriers and facilitators influencing the uptake of fall-prevention interventions among community-dwelling older adults and healthcare providers in Delaware. This

The impact of an intervention by a mobile geriatrics team including a pharmacist on potentially inappropriate drug prescription: results of the PharMoG study.

BACKGROUND: Polypharmacy and pharmacokinetic alterations expose elderly patients to an increased risk of iatrogenic events, hospitalisation, and healthcare costs. Therapeutic optimisation is based on identifying potentially inappropriate drug prescriptions (PIDPs) using an explicit approach based on standardised criteria such as STOPP & START and/or an implicit approach. Mobile Geriatric Teams (hereinafter MGT) and clinical pharmacists work together to reduce PIDPs, but pharmacists are not always part of these teams. The PharMoG study included a pharmacist in the MGT (PharMoG team) from a Fren

Identification and management of cancer cachexia in patients: Assessment of healthcare providers' knowledge and practice gaps.

BACKGROUND: Cancer cachexia negatively impacts patient outcomes, quality of life and survival. Identification and management of cancer cachexia remains challenging to healthcare professionals (HCPs). The aim of this assessment was to identify current gaps in HCPs' knowledge and practice for identifying and managing adults with cancer-related cachexia. Results may guide development of new educational programmes to close identified gaps and improve outcomes of cancer patients. METHODS: An international assessment was conducted using a mixed-methods approach including focus group interviews with

Addiction Consult Service and Inpatient Outcomes Among Patients with OUD.

BACKGROUND: Despite rising hospitalizations for opioid use disorder (OUD), rates of inpatient medications for OUD (MOUD) initiation are low. Addiction consult services (ACSs) facilitate inpatient MOUD initiation and linkage to post-discharge MOUD, but few studies have rigorously examined ACS OUD outcomes. OBJECTIVE: To determine the association between ACS consultation and inpatient MOUD initiation, discharge MOUD provision, and post-discharge MOUD linkage. DESIGN: Retrospective study comparing admissions that received an ACS consult and propensity score-matched historical control admissions

Exploring the use of extended release opioids at shortened dosing intervals in people with chronic pain and high risk medication or substance use.

Background Critical attention to rational opioid prescribing has emerged from the opioid epidemic in North America. Individuals with chronic pain are prescribed extended release opioids in an effort to maintain stable drug levels and for more convenient dosing, though evidence to support improvements in pain or function is lacking. It has been observed that extended release opioid products are used at intervals shorter than recommended by product monographs. The need for shortened intervals has been linked with potential inter-patient variability in pharmacokinetics, among other rationale. Imp

Integrating Oral Health Into Occupational Therapy: Knowledge, Practices, Barriers and Facilitators-A Systematic Review.

BACKGROUND: Oral health is essential for overall wellbeing and daily functioning, yet its integration into occupational therapy remains limited. Despite the profession's potential to promote oral health through person-centred, occupation-based approaches, evidence of this integration is scarce. This systematic review explores occupational therapists' (OTs) knowledge and practices in oral health care and examines the barriers and facilitators to incorporating oral health care into their clinical practice. METHODS: A comprehensive search was conducted across four databases: MEDLINE (Ovid), CINA

Perspectives of intensive care professionals on extubation practices, causes of delays, and proposed solutions: A cross-sectional study.

BACKGROUND: Timely extubation is essential to ensure patient safety and efficient intensive care unit (ICU) management. Delays in extubation can lead to adverse outcomes and extended hospital stays. Although several factors affect timely extubation, those contributing to delays, especially in Saudi Arabian ICUs, have not been well described. This study aimed to identify factors contributing to delayed extubation in Saudi Arabian ICUs from the perspective of ICU healthcare professionals (HCPs), including physicians, nurses, and respiratory therapists (RTs). METHODS: A cross-sectional survey wa

Shaping the Future: Recommendations from a Palliative Social Work Summit-A National Call to Action.

BACKGROUND: Palliative social workers play a critical role in serious illness care by addressing the biopsychosocial, spiritual, existential, and cultural needs of patients and families. Despite substantial professional infrastructure and contributions to equity and person-centered care, the profession faces persistent challenges related to recognition, funding, and visibility. These challenges are intensified by widening health inequities, policy threats to access to care, and national workforce shortages. METHODS: To assess the current landscape and advance a coordinated professional agenda

Implementation of problem-based learning pedagogy with integration of tamsulosin management in traditional urology residency training under the multidisciplinary team model.

BACKGROUND: Traditional urology residency programs often depend heavily on lectures, which can limit resident engagement and teamwork. Problem-based learning (PBL) in a multidisciplinary team setting has been shown to enhance clinical reasoning skills and improve knowledge retention. This article examines the effectiveness of a PBL approach, specifically focused on a tamsulosin module related to benign prostatic hyperplasia (BPH) in urology training. OBJECTIVES: To evaluate whether a PBL-based educational intervention improves pharmacology knowledge, clinical reasoning, and interprofessional

Patient and public involvement in the co-design of a multicenter randomized controlled trial on prehabilitation: results from the PREHAB-PEER conceptual phase.

BACKGROUND: Multimodal prehabilitation before colorectal cancer surgery (CRC) aims to enhance patients' physical and psychological resilience before surgery and support postoperative recovery. However, previous prehabilitation trials have reported heterogeneous findings and faced challenges related to intervention complexity, adherence, patient burden, and outcome selection. These limitations highlight the need to involve patients and other stakeholders early in the design of complex prehabilitation trials. METHODS: This mixed-methods conceptual phase was designed to translate patient and sta
