Histologic results from a human implant retrieved due to fracture 5 years after insertion in a sinus augmented with anorganic bovine bone.

BACKGROUND: Little is known about the healing pattern and the osseointegration processes at the interface of implants placed into different grafting materials in man. Anorganic bovine bone (ABB) is a xenogenic material with a high biocompatibility and osteoconductivity. METHODS: A 47-year-old patient underwent a monolateral sinus-augmentation procedure using 100% ABB. Two titanium dental implants were inserted at the same time as the grafting procedure. After 6 months, a fixed prosthetic restoration was placed. After a 5-year loading period, the distal implant fractured, and the implant was r

Esthetic restorations: observations and insights gained over a 5-year period demonstrated with three case reports.

This article presents two prosthodontically treated patient cases that were observed over a period of at least 5 years after treatment. The evaluation, diagnosis, treatment planning, and treatment stages were critically reviewed and reassessed from different perspectives. The conclusions drawn from this evaluation were subsequently implemented in a third clinical case. To ensure the long-term success of a restoration, certain biologic and mechanical principles must be observed, and the appropriate prosthodontic treatment must be chosen accordingly.

Maxillary sinus reconstruction with calvarium bone grafts and endosseous implants.

PURPOSE: The objectives of this study were 1) to evaluate the survival rate for implants placed in maxillary sinuses augmented with autogenous calvarium bone graft and 2) to evaluate the feasibility, complications, and morbidity of this technique as well as patient satisfaction. PATIENTS AND METHODS: Seventy-nine maxillary sinuses in 58 patients were reconstructed with calvarium grafts between 1992 and 2002 with more than 1 year of prosthetic loading. Two hundred twenty-three implants were placed 3 to 11 months after grafting. Bone levels were measured preoperatively after grafting and 1 year

Complications of grafting in the atrophic edentulous or partially edentulous jaw.

Complications of ridge reconstruction can delay healing or even cause total failure of the procedure, making implant placement impossible. Most intraoperative complications, such as insufficient material for a graft or inadequate range of a soft tissue flap, are the result of poor treatment planning or execution. Postoperative infection is usually associated with onlay, veneer, and J grafts and dehiscence. Resorption of the graft can range from minor (exposure of the heads of the fixation screws) to total. Retraction of a soft tissue flap is most likely where the vestibule is shallow or the mu

Utility of bispectral index monitoring during intravenous sedation in the dental office.

PURPOSE: This research sought to compare two different systems to monitor sedated patients undergoing implant surgery in the dental office: the bispectral index (BIS) and the Ramsay scale. This information was used to establish an optimal BIS range for surgery in these patients and to calculate differences in drug consumption in both groups. MATERIALS AND METHODS: Consecutive patients undergoing implant surgery were studied and randomly assigned to two groups. Patients were sedated using intravenous propofol, fentanyl, and midazolam. The sedation level in group A was measured using the Ramsay

Clinical similarities and histological diversity comparing fresh frozen onlay bone blocks allografts and autografts in human maxillary reconstruction.

BACKGROUND: In the absence of autologous bone for harvesting, fresh-frozen bone allografts turned into an alternative for bone reconstruction procedures. PURPOSE: The purpose of this study was to make a histological analysis of fresh-frozen onlay bone allografts (ALs), compared with autografts, in patients who needed maxillary reconstruction prior to dental implants placement. MATERIALS AND METHODS: Twelve patients with bone deficiencies (width inferior to 4 mm) in the sites where the implants were planned were enrolled in the study. From these, six were elected to be treated with autogenous

In vivo comparison of hard tissue regeneration with human mesenchymal stem cells processed with either the FICOLL method or the BMAC method.

OBJECTIVE: To compare new bone formation in maxillary sinus augmentation procedures using biomaterial associated with mesenchymal stem cells (MSCs) separated by two different isolation methods. BACKGROUND: In regenerative medicine open cell concentration systems are only allowed for clinical application under good manufacturing practice conditions. METHODS: Mononuclear cells, including MSCs, were concentrated with either the synthetic polysaccharide (FICOLL) method (classic open system--control group, n = 6 sinus) or the bone marrow aspirate concentrate (BMAC) method (closed system--test gro
