Using π-Shaped Auricular Cartilage With a Local Flap for Lower Eyelid Support.

INTRODUCTION: Auricular cartilage has been largely reported as an available substitute of defected tarsus to restore lower eyelid stability. However, undesirable results such as displacement, detachment, warping, down-gaze disturbance, and surface irregularity of the cartilage graft have been reported in previous reports, some of which require reoperations. In this study, the authors present a new technique to use strips of auricular cartilage with a local flap to restore the lower eyelid stability for a reliable eyelid support. METHODS: From 2013 to 2018, the technique was carried out on 13

Full-thickness lower eyelid reconstruction with a conchal chondro-perichondral graft and local coverage with Mio-cutaneous flaps--our divisional experience.

BACKGROUND: Eyelid reconstruction represents a challenge because of the complexity of this structure. Full-thickness eyelid defects demand the reconstruction of 2 fundamental elements: anterior and posterior lamellae. The available reconstructive techniques are all associated with the attention to restore the support and responsibility of eyelid stability, represented physiologically by the tarsus. In 1987, Matsuo proposed a reconstructive technique that involved the use of a chondro-perichondral graft harvested from the auricular concha to reconstruct the posterior lamella, and an adjacent sk

Concentric Malar Lift in the Management of Lower Eyelid Rejuvenation or Retraction: A Clinical Retrospective Study on 342 Cases, 13 Years After the First Publication.

BACKGROUND: Lower eyelid rejuvenation can, unfortunately, induce scleral show even if the lower eyelid procedure is limited. This study was designed to assess the effectiveness and reliability of the concentric malar lift technique in two scenarios: the first, in rejuvenation of the mid-face and, the second, in reconstructive surgery for correction of congenital or acquired eyelids malposition. METHODS: The concentric malar lift technique was first published by Le Louarn (Aesthet Plast Surg 28(6):359-372, 2004). This retrospective study was carried out by analyzing data on patients operated o

Surgical desensitisation of the mechanoreceptors in Müller's muscle relieves chronic tension-type headache caused by tonic reflexive contraction of the occipitofrontalis muscle in patients with aponeurotic blepharoptosis.

Proprioceptively innervated intramuscular connective tissues in Müller's muscle function as exterior mechanoreceptors to induce reflex contraction of the levator and occipitofrontalis muscles. In aponeurotic blepharoptosis, since the levator aponeurosis is disinserted from the tarsus, stretching of the mechanoreceptors in Müller's muscle is increased even on primary gaze to induce phasic and tonic reflexive contraction of the occipitofrontalis muscle. It was hypothesised that in certain patients with aponeurotic blepharoptosis, the presence of tonic reflexive contraction of the occipitofrontal

Staged reconstruction of the lower eyelid following tri-lamellar injury: a case series and anatomic study.

PURPOSE: Lower eyelid scaring and malposition following violation of all three lamellae pose a significant ophthalmologic reconstructive challenge. The purpose of our study was to document a staged approach for this problem using: 1) transconjunctival scar release followed by palatal graft below the tarsal plate and subciliary scar release followed by full-thickness skin graft superficial to the tarsal plate and 2) subsequent autologous fat grafting to the lower eyelid. METHODS: Cadaveric anatomic dissections were performed. Post-traumatic and post-surgical lower eyelid deformities requiring

Palpebral spring in the management of lagophthalmos and exposure keratopathy secondary to facial nerve palsy.

PURPOSE: To evaluate the use of a palpebral spring, a dynamic facial reanimation technique, in the management of lagophthalmos and exposure keratopathy secondary to facial nerve palsy. METHODS: A palpebral spring was placed in 29 eyelids of 28 patients with symptomatic facial nerve palsy. Preoperative and postoperative symptoms, upper eyelid margin to midpupil distance, lagophthalmos, and exposure keratopathy were evaluated. RESULTS: At an average of 83 months follow-up, preoperative symptoms improved or resolved in 26 (90%) eyes. The upper eyelid margin to midpupil distance decreased and la

[Orbital hernias: new views of the pathogenesis, possibilities of correction].

The paper deals with the basic pathogenetic aspects of development of orbital hernias--the factors of a tarsoorbital fascial change in the presence of the hereditary syndrome of connective tissue hyperplasticity and elevated intraorbital pressure, which affects the volume of orbital fat. The possibilities of a differential diagnosis of orbital hernias and eyelid edemas are considered. There is a biomechanical association of orbital hernias with acquired age-related enophthalmos. The examples of impairments in the tolerance of the optic nerve and in the development of optic neuropathy in enopht

Claims prevention for the aesthetic surgeon: preparing for the less-than-perfect outcome.

Many surgeons will find themselves faced with a claim of malpractice. At times this may be due to a less-than-perfect outcome. Well over half of such claims are preventable, as most are based on failures of communication and/or patient selection criteria. Attention to legal principles such as standard of care, warranty and disclosure, knowledge of patient factors, and the ability to communicate clearly and effectively will hopefully maximize the possibility of remaining a claims-free surgeon.

Three-Dimensional Analysis of Eyelid Morphological Outcomes After Hughes Flap Reconstruction for Full-Thickness Lower Eyelid Defects.

BACKGROUND: The standard technique used for full-thickness reconstruction of the lower eyelid is the Hughes tarsoconjunctival flap. Despite its widespread use, objective postoperative assessments of eyelid morphology and tension remain rare. METHODS: This cross-sectional clinical study included patients who underwent the Hughes procedure at the Department of Ophthalmology, University of Cologne. Eyelid morphology was evaluated using the VECTRA M3 stereophotogrammetry system in neutral position, manual downward traction, and standardized hook distraction with a 15.9 g eyelid hook. Seventeen an

Outcomes after surgical resection of lower eyelid tumors and reconstruction using a nasal chondromucosal graft and an upper eyelid myocutaneous flap.

INTRODUCTION: Surgical excision of large malignant lower eyelid tumors may cause important full-thickness eyelid defects. The reconstruction of such defects must restore the physiologic function of the eyelid and also re-establish an acceptable aesthetic result. MATERIALS AND METHODS: We report the outcomes of full-thickness excision of tumors extending over half of the horizontal lid length, followed by reconstruction using a nasal chondromucosal graft (coming from the ipsilateral ala of the nose) and an upper eyelid myocutaneous flap. Histological analysis of the specimen identified the tum

Zygoma reconstruction.

Ideal reconstruction of the zygoma position is essential in restoring facial width, projection, and symmetry. Reconstruction should be focused on the zygoma's 4 articulations and restoring the vertical and horizontal pillars of the facial skeleton. This article describes the applied surgical anatomy as it relates to zygomatic deformities, surgical approaches, and reconstruction. The basis for diagnosing and classifying zygoma deformities as they relate to severity of injury and associated displacement, comminution, and comorbidities is also discussed. Traditional and contemporary concepts in p

Correction of superior sulcus deformity with orbital fat anatomic repositioning and fat graft applied to retro-orbicularis oculi fat for Asian eyelids.

BACKGROUND: "Sunken eyelid" is a term used to describe a deeply sunken area between the upper eyelid and upper edge of the orbital bone. The condition frequently is accompanied by blepharoptosis. Correction of a "sunken eyelid" generally is performed with a fat graft. However, surgical limitations such as a movement dysfunction of the upper eyelid or irregular skin surface often are encountered using the grafting method previously reported. An open technique is believed to be more precise than a closed procedure, and satisfactory results may be achieved by repositioning the upper eyelid fat an

Minimally invasive periorbital rejuvenation.

Minimally invasive procedures have become increasingly popular over the last decade. In many cases, the use of neuromodulators and fillers has replaced surgical procedures. This article reviews the analysis and evaluation of the aesthetic patient presenting for periorbital rejuvenation. A layered approach is used, evaluating the skin, fat, muscle, and bone to determine which procedure is best suited for each patient. Volume enhancement with the use of fat and fillers and muscle manipulation with the use of neuromodulators are discussed. A brief summary of currently available skin-resurfacing t

Renaissance of Traditional Mineral Drugs in Cancer: Advanced Delivery Strategies and Bioengineering Approaches.

Traditional mineral drugs represent an underexploited reservoir of natural antitumor agents; however, their clinical translation has historically been hindered by poor bioavailability, non-specific biodistribution, and dose-limiting toxicity. This review comprehensively examines the pharmacological mechanisms and modern formulation strategies driving the renaissance of mineral-based oncology therapeutics. We highlight how mineral drugs exert potent anticancer effects through interconnected pathways, including regulated cell death (e.g., apoptosis, ferroptosis), cell-cycle arrest, and immunomod

Immunosenescence: signaling pathways, diseases and therapeutic targets.

Immunosenescence refers to the abnormal activation or dysfunction of the immune system as people age. Inflammaging is a typical pathological inflammatory state associated with immunosenescence and is characterized by excessive expression of proinflammatory cytokines in aged immune cells. Chronic inflammation contributes to a variety of age-related diseases, such as neurodegenerative disease, cancer, infectious disease, and autoimmune diseases. Although not fully understood, recent studies contribute greatly to uncovering the underlying mechanisms of immunosenescence at the molecular and cellul
