Multicentre controlled open randomised clinical trial to assess efficacy and safety of an anti-tuberculosis drug combination based on optimised-dose rifampicin, optimised-dose moxifloxacin and optimised-dose linezolid for TB: the RML-TB trial protocol.

INTRODUCTION: International organisations, scientists and the tuberculosis (TB) community have been advocating for a shorter, safer treatment for drug-susceptible (DS)-TB with equal or better efficacy than current regimens. A promising approach to achieve this is the combination of dose-optimised repurposed drugs. METHODS AND ANALYSIS: The RML-TB trial is a phase IIb, randomised, non-inferiority, controlled, open-label, multicentre clinical trial. It compares an experimental regimen (optimised-dose rifampicin (R) at 30mg/kg/day, moxifloxacin 600mg/day and linezolid (L) 600 mg/day) with the st

Four-month moxifloxacin-based regimens for drug-sensitive tuberculosis.

BACKGROUND: Early-phase and preclinical studies suggest that moxifloxacin-containing regimens could allow for effective 4-month treatment of uncomplicated, smear-positive pulmonary tuberculosis. METHODS: We conducted a randomized, double-blind, placebo-controlled, phase 3 trial to test the noninferiority of two moxifloxacin-containing regimens as compared with a control regimen. One group of patients received isoniazid, rifampin, pyrazinamide, and ethambutol for 8 weeks, followed by 18 weeks of isoniazid and rifampin (control group). In the second group, we replaced ethambutol with moxifloxac

Clinical manifestation and outcome of tuberculosis in children with acquired immunodeficiency syndrome.

BACKGROUND: Atypical clinical manifestations and rapid progression of tuberculous disease (TB) are well-recognized in adults with the acquired immunodeficiency syndrome (AIDS). There are few reports of children with AIDS and TB. We report the manifestations, clinical course and outcome of 12 pediatric patients with AIDS and TB. METHODS: The charts of all children admitted to our institution, from 1989 through 1994, with the diagnoses of AIDS and culture-proved TB were reviewed. RESULTS: Twelve children between the ages of 2 months and 13 years fit the criteria. The mean time between the diag

Tuberculous spondylodiscitis following intravesical Bacille Calmette-Guérin therapy: A case report and systematic literature review.

BACKGROUND: Intravesical Bacille Calmette-Guérin (BCG) therapy is the standard treatment for non-muscle-invasive bladder cancer. Disseminated BCG infection occurs in fewer than 5% of patients, with skeletal involvement (i.e., tuberculous spondylitis) being reported in only 36 cases worldwide. Here, a 77-year-old male with a history of BCG therapy for non-muscle-invasive bladder cancer developed L1/L2 osteomyelitis treated with percutaneous endoscopic transforaminal discectomy, debridement, and anti-tuberculous therapy. Twelve months later, however, the patient required, posterior instrumentati

Genetic and immunologic determinants of BCG disease: from mechanism to prevention.

BACKGROUND: Bacillus Calmette-Guérin (BCG) infection, a disseminated infection triggered in immunocompromised hosts by the administration of the attenuated BCG vaccine, is characterized by a pathogenic mechanism that involves the synergistic interaction between host immune deficiencies and strain-specific genetic mutations. METHODS: This review synthesizes published multi-omics and proteomics findings from the past decade, along with reported results from in vitro drug susceptibility assays and molecular diagnostics, to explore key pathogenic genes in BCG strains. We compare differences in pa

When Otorrhea Hides Something Unexpected: A Challenging Case Report.

Tuberculosis remains a major global health concern, predominantly in low and middle-income countries. Tuberculous otitis media (TOM) is a rare form of extrapulmonary tuberculosis, representing a small fraction of tuberculosis and chronic suppurative otitis media cases. The nonspecific and indolent presentation of TOM, besides its rarity, often results in delayed diagnosis, mainly in regions with a low tuberculosis prevalence, with increased risk of irreversible complications. We report a case of TOM in a 36-year-old Portuguese woman with a four-month history of right ear pain, otorrhea, obstru

Miliary Tuberculosis: A Comprehensive Review of Epidemiology, Clinical Manifestations, Diagnosis, Treatment, and Complications.

Miliary tuberculosis (TB) represents a potentially fatal form of disseminated TB resulting from the lymphohematogenous dissemination of Mycobacterium tuberculosis. Although accounting for approximately 1-2% of all TB cases in immunocompetent adults, its mortality rate exceeds 30%. This review screened peer-reviewed, original research studies via four databases (PubMed, EMBASE, MEDLINE, and CENTRAL) to synthesize up-to-date evidence. Key risk factors include human immunodeficiency virus (HIV) co-infection, anti-tumor necrosis factor-α (TNF-α) agents and chronic malnutrition. The clinical presen

Design, synthesis, and biological evaluation of (E)-3-amino-N'-substituted benzylidene-6-chloropyrazine-2-carbohydrazide derivatives as anti-mycobacterial agents.

Pyrazinamide is a powerful sterilizing agent that reduces the treatment duration required to cure tuberculosis and works synergistically with both new and existing anti-tuberculosis drugs. Thirty-one derivatives of (E)-3-amino-N'-substituted benzylidene-6-chloropyrazine-2-carbohydrazide (20a-20ae) were designed and synthesized. The structures of these compounds were confirmed through various analytical methods, such as 1H NMR, 13C NMR, and mass spectrometry. To better understand the arrangement of atoms and confirm the structures, single crystals of 20 m and 20aa were grown and analyzed. The f

Efficacy of Vitamin D Supplementation in Accelerating Sputum Conversion of Bacteriologically Diagnosed Tuberculosis Patients: A Meta-Analysis.

Tuberculosis (TB) is a global pandemic disease whose first-line treatment is with a combination of four antimicrobials commonly abbreviated as HRZE (isoniazid, rifampin, pyrazinamide, and ethambutol). However, some consider the regimen inadequate, thus conducting numerous trials to search for adjunctive therapies, including vitamin D supplementation. Trials so far have led to mixed results. The present study aims to consolidate data on the effects of vitamin D supplementation on clinical outcomes, specifically rates of sputum culture conversion. PubMed databases were searched for peer-reviewed

Surveillance and analysis of drug resistance and drug resistance levels in multidrug resistant tuberculosis on the tropical islands of China.

OBJECTIVE: Multidrug-resistant tuberculosis (MDR-TB) remains a major public health challenge in China. Hainan, China's largest tropical island, possesses distinct socio-geographical features. However, the drug resistance patterns and molecular epidemiology of MDR-TB in this region have not been fully elucidated. This study aimed to assess the correlation between drug resistance genotypes and phenotypic resistance levels in multidrug-resistant Mycobacterium tuberculosis (MDR-MTB) strains collected from Hainan Island, using whole-genome sequencing (WGS) and phenotypic drug susceptibility testing

Whole Genome Sequencing of Mycobacterium tuberculosis under routine conditions in a high-burden area of multidrug-resistant tuberculosis in Peru.

Whole Genome Sequencing (WGS) is a promising tool in the global fight against tuberculosis (TB). The aim of this study was to evaluate the use of WGS in routine conditions for detection of drug resistance markers and transmission clusters in a multidrug-resistant TB hot-spot area in Peru. For this, 140 drug-resistant Mycobacterium tuberculosis strains from Lima and Callao were prospectively selected and processed through routine (GenoType MTBDRsl and BACTEC MGIT) and WGS workflows, simultaneously. Resistance was determined in accordance with the World Health Organization mutation catalogue. Ag

Pharmacokinetics of drugs used to treat drug sensitive-tuberculosis in breastfeeding mother-infant pairs: An observational pharmacokinetic study.

BACKGROUND: Globally, more than half of women take medicines whilst breastfeeding. Data concerning the exposure of the breastfed infant to drugs and any related risks are sparce. Lactation studies are only rarely performed close to licensing for medicines anticipated to be widely used in women of childbearing age. Medicines taken by breastfeeding mothers on tuberculosis (TB) treatment can be transferred to the breastfed infant. Potential effects of anti-tuberculosis medicines on nursing infants are not well understood. Similarly, women face mental health challenges while taking medications, in

Pulmonary tuberculosis in two preterm infants conceived by in vitro fertilization.

The early diagnosis of tuberculosis (TB) in infants is challenging owing to the non-specific clinical manifestations in infancy, particularly preterm infants. Two cases in preterm infants are reported. Case 1, conceived by in vitro fertilization (IVF), was born at 27 + 1 weeks gestational age weighing 880 g. He presented on Day 85 with intermittent fever. Following a course of systemic broadspectrum antibiotics, he deteriorated, developing acute respiratory distress syndrome (ARDS). TB Xpert polymerase chain reaction (PCR) of the sputum obtained by laryngeal aspiration confirmed Mycobacterium

Genotypic and phenotypic comparison of drug resistance profiles of clinical multidrug-resistant Mycobacterium tuberculosis isolates using whole genome sequencing in Latvia.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) remains a major public health problem in many high tuberculosis (TB) burden countries. Phenotypic drug susceptibility testing (DST) take several weeks or months to result, but line probe assays and Xpert/Rif Ultra assay detect a limited number of resistance conferring gene mutations. Whole genome sequencing (WGS) is an advanced molecular testing method which theoretically can predict the resistance of M. tuberculosis (Mtb) isolates to all anti-TB agents through a single analysis. METHODS: Here, we aimed to identify the level of concordance
