Severe injury mechanisms in two paediatric trauma centres: Determination of prevention priorities.

BACKGROUND: To meet community needs, injury prevention programs for children should be targeted to trends in objective data on mechanisms of injury. The aim of the present study was to identify the most important severe injury mechanisms. METHODS: The present study retrospectively reviewed severe paediatric trauma patients in two regional trauma centres. Injury prevention priority scores were computed using different severity measures - injury severity score (ISS), revised trauma score, trauma-related injury severity score, Glasgow Coma Scale (GCS) and mortality - to identify prevention prior

Radiologic, Neurologic and Cardiopulmonary Aspects of Submersion Injury.

Many indices and scores are used in critical care medicine to aid management and predict risk of mortality. We report 2 cases of submersion injury and discuss the usefulness and application of common respiratory and critical care indices. The respiratory indices help better understand the pulmonary pathophysiology and characterize the severity of lung injury and ventilation/perfusion mismatch. Severe lung injury resolved after ventilation support with appropriate positive end-expiratory pressure in both cases. The 6-year-old girl survived the near-fatal submersion injury intact despite grossly

Dissociative experiences as mediators between childhood trauma and auditory hallucinations.

The purpose of this work was to study the relationship between reported traumatic experiences in childhood and positive psychotic symptoms. We hypothesized that dissociative experiences were potential mediators between childhood trauma and hallucinations, but not delusions. The sample comprised 71 patients diagnosed with psychoses. They were assessed with the Dissociative Experiences Scale (DES-II; Carlson & Putnam, 1993), a questionnaire on trauma (TQ; Davidson, Hughes, & Blazer, 1990), and the Positive and Negative Syndrome Scale (PANSS; Kay, Opler, & Lindenmayer, 1988) delusions and halluci

History of high-velocity impact water trauma at Letterman Army Medical Center: a 54-year experience with the Golden Gate Bridge.

Three time frames were studied during the 54-year history of the Golden Gate Bridge from 1937 to 1991. During that period of time, there were 918 documented jumps from this majestic structure to the water 250 feet below. The last 15 years provided us with 297 consecutive patients, all brought to one institution, which were retrospectively reviewed and categorized as to site and type of injury for survivors and fatalities. This is the largest high-velocity water impact trauma series in the world. Certain unique characteristics of the Golden Gate Bridge and the San Francisco Bay lend itself to t

Social support moderates association between posttraumatic growth and trauma-related psychopathologies among victims of the Sewol Ferry Disaster.

Disasters have numerous harmful effects on the mental health status of trauma-exposed people. We investigated the differences in the association between trauma-related psychopathologies and posttraumatic growth according to the perceived social support level among victims of the Sewol Ferry disaster on April 16, 2014, in South Korea. Data from 241 bereaved family members, survivors, and family members of survivors were used. The Duke-UNC Functional Social Support Questionnaire, Posttraumatic Growth Inventory, PTSD Checklist-5, Posttraumatic Embitterment Disorder Self-Rating Scale, Patient Heal

Successful Resuscitation: Novel Partnership between Paramedics and U.S. Coast Guard.

In this article, we present a notable case that illustrates a novel partnership between a United States Coast Guard small boat station and a civilian paramedic response unit. Patients who experience medical emergencies in remote environments are at particularly high risk for morbidity and mortality. For the most serious conditions, delayed contact with Advanced Life Support (ALS) has grave results. Typically, these circumstances involve small groups of individuals and cannot be easily predicted. The waters off the coasts of Maui, Hawaii, however, host millions of residents and visitors annuall

Migrant deaths along the California-Mexico border: an anthropological perspective.

California shares a 150-mile international border with Mexico. Traditionally, this border has seen non-stop illegal migration. In the 1990s, the Border Patrol began a concerted effort to establish and maintain control of the border, beginning in urban San Diego. This heightened law enforcement presence, known as Operation Gatekeeper, changed the westernmost segment of the border from the most permeable to the least permeable. This enforcement pushed migrants into more dangerous crossing areas in eastern San Diego and Imperial Counties, making their trip longer and more physically challenging a

The Capacity Profile: a method to classify additional care needs in children with neurodevelopmental disabilities.

The aim of this study was to determine the interrater reliability and stability over time of the Capacity Profile (CAP). The CAP is a standardized method for classifying additional care needs indicated by current impairments in five domains of body functions: physical health, neuromusculoskeletal and movement-related, sensory, mental, and voice and speech, in children from 3 to 18 years of age. The intensity of care in each domain is defined from 0 (no need for additional care) to 5 (needs help with every activity). The intensity of additional care in each of the five separate domains indicate

Cross-sectional blood vessel area differences in various causes of death in unenhanced forensic post-mortem computed tomography.

BACKGROUND: In forensic autopsies, the terminal circulatory status in the form of acute congestion or findings consistent with severe blood loss can be significant, as it may indicate different causes of death. However, the autopsy-related description of acute congestion or severe blood loss is predominantly subjective. Post-mortem computed tomography (PMCT) offers the possibility of recording metric parameters such as the cross-sectional area of blood vessels, which could be used for the objective description of acute congestion or severe blood loss. The aim of the present study was therefore

23Na MRI quantification of sodium content in porcine eyes after immersion in saltwater and freshwater en route to time in water estimation.

BACKGROUND: Differentiation between saltwater and freshwater immersion as well as estimating the corpse's time in water can be challenging. We aimed to establish and examine the feasibility of a novel approach based on sodium magnetic resonance imaging (23Na MRI) of the eye to facilitate noninvasive sodium quantification. METHODS: Enucleated porcine eyes were immersed in NaCl 0.9%, NaCl 3.0%, NaCl 5.85%, distilled water (DW) or lake water (LW) at different time intervals, followed by 23Na 7-T MRI sodium quantification. RESULTS: After 6 h of immersion, a significant difference in vitreous bod

Blood pressure, pulse rate, and skin temperature during hot-water bathing in real-world settings among community-dwelling older adults: the HEIJO-KYO Study.

BACKGROUND: Home hot-tub bathing substantially increases drowning mortality rates among older adults in Japan. Previous laboratory studies on hemodynamic responses during hot-tub bathing have been inconsistent depending on the thermal conditions. Furthermore, real-world hemodynamic changes that occur during bathing remain poorly understood. This study investigated the association between individual thermal states and hemodynamic parameters during hot-tub bathing among community-dwelling older adults. METHODS: In this cross-sectional study conducted between January 2016 and April 2019, which i

Waterborne Disease Outbreaks Associated with Splash Pads - United States, 1997-2022.

PROBLEM/CONDITION: Splash pads are recreational interactive water venues that spray or jet water on users. Splash pads are intended for children aged <5 years and designed so that water typically does not collect in areas accessible to users, thereby minimizing the risk for drowning. Splash pads were first found to be associated with waterborne disease outbreaks in 1997. PERIOD COVERED: 1997-2022. DESCRIPTION OF SYSTEM: Since 1971, waterborne disease outbreaks have been voluntarily reported to CDC by state, local, and territorial health departments using a standard paper form via the Waterbo

Suicide along the Australian coast: Exploring the epidemiology and risk factors.

Suicide is an increasing global concern with multiple risk factors, yet location-based understanding is limited. In Australia, surf lifesavers (SLS) and lifeguards patrol the coast, performing rescues and assisting injured people, including people who suicide. This study is a descriptive epidemiological analysis of Australian coastal suicide deaths. The results will be used to inform training and support surf lifesaving personnel and suicide prevention organisations. This is a population-based cross-sectional study of suicide deaths at Australian coastal locations (between 1 January 2005 and 3

[Aspiration and pneumonia risk after preclinical invasive resuscitation: Endotracheal intubation and supraglottic airway management with the laryngeal tube S].

BACKGROUND: Laryngeal tubes (LT) have substantially facilitated emergency airway management. However, it remains unclear whether LTs provide comparable protection against aspiration or even higher rates of aspiration and pneumonia compared to endotracheal intubation (ET) as the former gold standard. METHODS: The indices for aspiration and early onset pneumonia in patients after preclinical airway management by either LT or ET were retrospectively analyzed. Furthermore, in-hospital mortality was analyzed. RESULTS: A total of 90 patients with invasive ventilation by either ET (n = 69) or LT (n
