Base-of-skull/infratemporal fossa shell fragment retrieval.

PURPOSE: Shell fragments and bullets are frequently observed to have settled in the skull base and infratemporal fossa (ITF) with the impact resulting in a new surgically complicated trauma. This compact region is one of the most difficult and dangerous locations for lodged objects. When called upon to retrieve these objects, the surgeon faces a complicated challenge. The complexity of the task depends on many factors, such as size, deformation, and surface hooks, as well as identification of the precise anatomic location. MATERIALS AND METHODS: New surgical approaches and successful methods

Placement of tapered implants using an osteotome sinus floor elevation technique without bone grafting: 1-year results.

PURPOSE: Achieving implant primary stability in poor-density bone is difficult when the available bone height is less than 6 mm. This study assesses the 1-year clinical performance of tapered implants in sites of reduced height in combination with osteotome sinus floor elevation without bone grafting material. MATERIALS AND METHODS: An osteotome sinus floor elevation procedure without grafting material was performed in the atrophic posterior maxilla. Tapered implants were placed in maxillary sites with residual bone height of 1 to 6 mm. Implant primary stability was assessed by finger pressur

Simplified osteotome sinus augmentation technique with simultaneous implant placement: a clinical study.

The aim of this study was to describe and evaluate a modified maxillary sinus floor augmentation technique: the simplified osteotome technique. The study consisted of 26 patients treated with 39 Brånemark implants (Nobel Biocare) placed using the simplified osteotome technique between September 1997 and November 2004 (87 months). Implant length ranged from 10 to 15 mm, while the loading time ranged from 5 to 74 months (mean: 35.2 months). The success rate was 97.4% according to Albrektsson's criteria. These preliminary data indicate that the simplified osteotome technique is effective and safe

Maxillary sinus augmentation using sinus membrane elevation and peripheral venous blood for implant-supported rehabilitation of the atrophic posterior maxilla: case series.

BACKGROUND: Dental implants need appropriate bone volume for adequate stability in the rehabilitation after tooth loss. In the severely atrophic posterior maxilla, the clinical success of implant treatment sometimes requires a vertical ridge augmentation in the maxillary sinus floor. PURPOSE: The purpose of this investigation was to evaluate a maxillary sinus floor augmentation technique using a replaceable bone window, elevation of the membrane, placement of implants, and injection of the patient's own venous blood to fill the voids. MATERIALS AND METHODS: Six patients with need of maxillar

Surgical strategy for medication-related osteonecrosis of the jaw (MRONJ) on maxilla: A multicenter retrospective study.

BACKGROUND/PURPOSE: Because the anatomy and the nature of the bone tissue between the mandible and maxilla are largely different, more site-specific studies are required to improve the healing rate on medication-related osteonecrosis of the jaw (MRONJ). The present study assessed maxillary MRONJ that was treated by surgery to understand its clinical characteristics, and to identify critical factors that influenced outcomes. MATERIALS AND METHODS: The medical records of 54 patients with maxillary MRONJ who underwent surgery were retrospectively reviewed. Variables related to the prognosis of M

Anatomic and functional masseter muscle adaptation following orthognathic surgery-MRI analysis in 3 years of follow-up.

BACKGROUND: Orthodontic and surgical technical advances in recent years have resulted in treatment opportunities for a whole range of craniofacial skeletal disorders either in the adolescent or adult patient. In the growing child, these can include myofunctional orthodontic appliance therapy or distraction osteogenesis procedures, while in the adult, the mainstay approach revolves around orthognathic surgery. The literature agrees that for a change in craniofacial morphology to remain stable, the muscles acting upon the facial skeleton must be capable of adaptation in their structure and, ther

Reclassification and treatment of odontogenic keratocysts: A cohort study.

The odontogenic keratocyst (OKC) is a recurrent cyst that has been recently reclassified from an odontogenic tumor to an odontogenic cyst. The aim of the present study was to investigate its treatment and address issues related to its association with nevoid basal cell carcinoma syndrome (NBCCS). Lesions from the cohort of patients included in the present study consisted of 40 OKCs, of which 27 lesions were treated by enucleation (GE) and 13 underwent decompression (GD). Complementary treatment occurred in 38 (95%) lesions, of which 10 underwent isolated peripheral ostectomy (GO) and 28 underw

Comparison of the long-term skeletal stability between a biodegradable and a titanium fixation system following BSSO advancement - a cohort study based on a multicenter randomised controlled trial.

Biodegradable fixation systems could reduce or eliminate the problems associated with removal of titanium plates. A multicenter randomised controlled trial (RCT) was performed in the Netherlands from December 2006-July 2009, and originally 230 injured and orthognathic patients were included. The patients were randomly assigned to either a titanium control group (KLS Martin) or to a biodegradable test group (Inion CPS). The aim of the present study was to compare the long-term skeletal stability of advancement bilateral sagittal split osteotomies (BSSO) of a biodegradable system and a titanium

Maxillary sinus floor augmentation in patients with maxillary sinus pseudocyst: case report.

The maxillary sinus floor elevation procedure has gained popularity with predictable results, and is a safe, acceptable technique for bone augmentation, providing a base for dental implant treatment. Faint radiopaque lesions at the base of the maxillary sinus are frequent diagnoses on radiographs and must be identified during dental implant planning. Pseudocysts classically appear hemispheric, homogeneously opaque, and well delineated in panoramic and periapical radiographs. The great majority of these lesions are asymptomatic and do not require surgical treatment. In this case report, we pres

Osteotome sinus floor elevation and simultaneous implant placement in grafted biomaterial sockets: 3 years of follow-up.

BACKGROUND: Immediate bone grafting procedures were proposed to preserve bone volume in residual damaged alveolar walls and to prevent the expansion of the sinus floor in the maxillary molar region. The use of an osteotome allows vertical bone augmentation and localized sinus elevation with minimal surgical trauma. The aim of this study is to evaluate the clinical outcome of implants placed in previously grafted alveoli that were expanded at a second-stage surgery by an osteotome technique. METHODS: Twenty patients requiring extraction of one or two upper molar teeth and/or a second premolar

Fully vs. partially rough implants in maxillary sinus floor augmentation: a randomized-controlled clinical trial.

OBJECTIVES: To compare implants with a rough surface in their whole length (FR) with implants having a 2 mm coronal machined portion (PR) when used in association with a sinus-lift procedure. MATERIAL AND METHODS: Twenty-six patients with 2 mm< or =x< or =9 mm residual alveolar crest were prosthetically restored with implants after a staged sinus-lift procedure using osteotomes. In 13 randomly chosen patients, no more than one FR implant was placed (test group), while the rest were PR implants. The other 13 patients received only PR implants (control group). For comparisons, only one implant

Management of sinusitis in adult cystic fibrosis.

Until recently, cystic fibrosis was frequently fatal during childhood. However, with current medical management, many patients are living into adulthood. This has created a new population of patients with chronic sinusitis and severe medical problems. In this report, experience with 22 patients, eight of whom have undergone sinus surgery, is presented, and recommendations for management are proposed. Presenting symptoms are typical of sinusitis, but in a few patients, severe debilitating headaches predominate. Oral antibiotics are often of little use due to the numerous courses of high dose in

Implant placement in fresh extraction sockets and simultaneous osteotome sinus floor elevation: a case series.

The purpose of this study was to evaluate the clinical success of implants placed in fresh extraction sockets with simultaneous maxillary sinus floor elevation using the osteotome technique. Twelve patients were included. All the patients required the extraction of a maxillary premolar--close to the maxillary sinus--and were scheduled for immediate implant placement. One experimental implant was placed per patient, with an 18-month follow-up period. The graft materials used in both sinus augmentation and peri-implant bone defects were a mixture of collagen gel and corticocancellous porcine bon

Bisphosphonate-related osteonecrosis of the jaws: a report of three cases demonstrating variability in outcomes and morbidity.

BACKGROUND: Bisphosphonates are used widely to manage skeletal disorders resulting from malignancies that destroy bone and from some metabolic bone diseases. A strong association between bisphosphonate treatment and the appearance of painful exposed nonvital bone in the mandible and maxilla after oral surgery has been reported in the last decade. Extensive reviews have appeared in the dental literature regarding bisphosphonate-related osteonecrosis of the jaws (BRONJ), including protocols for diagnosis, management and diagnostic imaging for early detection; feature definition; and determinatio

Immediate labial contour restoration for improved esthetics: a radiographic study on bone splitting in anterior single-tooth replacement.

A bone-splitting technique used for anterior single-tooth replacement was evaluated in 54 patients and 68 sites. The cumulative rate of implant survival was 93.7% (SE 4.6%) after more than 4 years. The decrease in marginal bone height ranged from 0.8 to 1.3 mm. Some reaction of the bone levels around the adjacent teeth should be anticipated (0.3 to 0.5 mm). It was concluded that the bone-splitting procedure is a safe and predictable technique when performed carefully on selected patients and with the proper instrumentation. The procedure seeks to reconstruct the labial contour of the alveolar
