We tested the colour vision of 72 people with Down's syndrome using the Ishihara test and an anomaloscope. We found that 13 of the subjects, 6 males and 7 females, had defective colour vision according to Pickford's classification. In monocular vision 10 eyes were protan (five simple, three extreme and two deviant), one eye was simple deuteranomalous and the remaining eyes were normal: in binocular vision four of the subjects were protan (two simple and two deviant), two subjects were deutan (one simple and one deviant) and the rest were normal. A new method is described for calculating the topography and curvature of an irregularly shaped cornea. Vertical planes of light are projected onto the cornea and points in x, y, z space are calculated from the light images on the corneal surface. A matrix of points is produced on the cornea and a mathematical surface is fitted to them. This study sought to determine whether proximally-induced accommodation (PIA) is capable of sustaining an accommodative response over time. Accordingly, the steady-state PIA response was measured during the course of a continuous 5 min stimulus period for targets located at distances of 0.2 or 6 m. Amplitude of accommodation was measured in 121 Hong Kong Chinese subjects between the ages of 11 and 65 years, using the 'push-up' method and the results compared with those previously obtained for Caucasian subjects. The relative contribution of tonic vergence to the maximum near vergence response as a function of age and the development of presbyopia has received little attention. Tonic vergence position was measured subjectively in a group of 50 subjects between 20 and 68 years of age. While there was no progressive trend with age, tonic vergence did exhibit a small but consistent convergent increase after 40 years of age. Thus, tonic vergence showed a slight change, possibly of a motor adaptive nature, concurrent with increasing age and the development of clinical presbyopia.

It has been widely assumed that VDU users position their paperwork closer than the screen to their eyes by an amount which may exceed their depth of field. The design and prescription of spectacles for the presbyopic VDU user must take these working distances into account. A computer-assisted optic nerve head analysis system was used to measure parameters of the optic nerve head from one eye from each of 15 subjects. Hong Kong Chinese infants were refracted by retinoscopy at regular intervals between the age of approximately 10 and 40 weeks. Cyclopentolate 1% was used to obtain cycloplegia. The spherical equivalent of the refractive error was calculated for each child and analysis of variance carried out according to the refractive status of the parents. Research relating to the use of Irlen tinted lenses and coloured overlays for underachievers is reviewed. Many of the studies were not published in refereed journals and were methodologically poor. The weaknesses of the Irlen argument are discussed, including the absence of evidence to support the claims that these tints need to be uniquely prescribed and manufactured. Distance and near readings of associated heterophoria were taken, using standard Mallett units, on 500 patients passing consecutively through an optometric practice and any symptoms, connected with distance and near vision, were noted. Visual conversion reaction (VCR) is a psychosomatic anomaly which occasionally manifests in children, most commonly in the form of reduced vision and visual field defects. Malingering or conscious simulation for secondary gain would appear to be rare in children but occasionally VCR and simulation coexist. A case history is presented of a 12-year-old child with behavioural and reading difficulties who manifested reduced vision most probably attributable to VCR and severe colour deficiency, which was best explained in terms of simulation or malingering.

Prism adaptation responses in orthophoric, exophoric and esophoric subjects are compared and found to be significantly different. Following a prolonged period of monocular occlusion the prism adaptation responses were compared, after occlusion the differences are no longer significant. This supports the theory that vergence (prism) adaptation is responsible for the existence of heterophoria. This paper shows that changes in heterophoria with monocular occlusion are caused by desaturation of prism adaptation.
