Prenatal diagnosis in fetal right aortic arch using chromosomal microarray analysis and whole exome sequencing: a Chinese single-center retrospective study.

BACKGROUND: Right aortic arch (RAA) is a common congenital aortic arch abnormality. Fetuses with RAA frequently have good outcomes after birth. However, chromosomal abnormalities and genetic syndromes suggest poor prognosis for these patients. So far the underlying genetic etiology is still not identified in most RAA patients based on traditional genetic techniques and a problem is still debated whether fetuses with isolated RAA should be referred for CMA. Our study aims to investigate the genetic etiology of fetuses with right aortic arch (RAA) by chromosomal microarray analysis (CMA) and who

Computed tomography of the superior vena cava.

The superior vena cava (SVC) can be visualized and reliably evaluated by computed tomography (CT). Opacification of this vessel with iodinated contrast material and multiplanar reformations can provide information concerning the patency of the lumen and the relationship of the SVC with adjacent mediastinal and lung structures, both normal and abnormal. SVC obstruction leading to SVC syndrome is the most common condition affecting this vessel. It can be caused by both extrinsic compression and intraluminal thrombosis. Numerous collateral pathways have been described by conventional radiographic

Impact of concomitant complex cardiac anatomy in nonsyndromic patients with complete atrioventricular septal defect.

OBJECTIVE: We studied a cohort of patients with nonsyndromic complete atrioventricular septal defect with and without concomitant complex cardiac anatomy and compared the outcomes after surgical repair. METHODS: Between 1993 and 2018, 62 nonsyndromic patients underwent complete atrioventricular septal defect repair. Sixteen patients (26%) had complex complete atrioventricular septal defect with variables representing concomitant cardiac anatomic complexity: tetralogy of Fallot, double outlet right ventricle, total anomalous pulmonary venous return, concomitant aortic arch reconstruction, mult

Anomalous left brachiocephalic vein: important vascular anomaly concomitant with congenital anomalies and heart diseases.

BACKGROUND: Anomalous left brachiocephalic vein (ALBCV) is a rare and less known systemic venous anomaly. Infrequently, this vein takes an abnormal course and passes to the right behind or beneath the aortic arch to create the superior vena cava (SVC). Its incidence was reported much higher in patients with congenital heart disease especially in conotruncal and aortic arch anomalies. It could be misdiagnosed with normal or abnormal mediastinal structures. It also could make complication during surgeries or invasive strategies. Previously, this anatomical finding has been reported in case repor

Bilateral renal agenesis/hypoplasia/dysplasia (BRAHD): postmortem analysis of 45 cases with breakpoint mapping of two de novo translocations.

BACKGROUND: Bilateral renal agenesis/hypoplasia/dysplasia (BRAHD) is a relatively common, lethal malformation in humans. Established clinical risk factors include maternal insulin dependent diabetes mellitus and male sex of the fetus. In the majority of cases, no specific etiology can be established, although teratogenic, syndromal and single gene causes can be assigned to some cases. METHODOLOGY/PRINCIPAL FINDINGS: 45 unrelated fetuses, stillbirths or infants with lethal BRAHD were ascertained through a single regional paediatric pathology service (male:female 34:11 or 3.1:1). The previously

Spectrum of potentially lethal cardiac conditions presenting with fetal sinus bradycardia: a report on 34 cases.

OBJECTIVE: Fetal sinus bradycardia (FSB) is usually a benign antenatal finding but it can be a marker of an underlying serious condition that manifests in later life. The aim of this study was to report the antenatal course and postnatal outcome of FSB. METHOD: This study was a retrospective review of all cases of persistent FSB, defined as a fetal heart rate (FHR) < 3rd percentile for gestational age (GA) with 1:1 atrioventricular conduction on at least two consecutive fetal echocardiograms, diagnosed between January 2014 and December 2024 at the Brompton Centre for Fetal Cardiology, London,

Ectopic mediastinal thyroid removed by U-VATS approach. A case report.

INTRODUCTION: Ectopic thyroid tissue is a rare entity, and accounts for approximately 1% of all mediastinal tumours. It is a differential diagnosis of the mediastinum tumors or metastatic deposits from an orthotopic gland, as well as other benign or malignant masses. Although most cases are asymptomatic and discovered incidently by imaging, symptoms related to tumor size and its compression of adjacent structures may also appear which necessites explorations and lead to diagnosis. CASE PRESENTATION: This is a 59-year-old women, followed for glaucoma and operated for bilateral congenital catar

The Bilateral Bidirectional Glenn Operation as a Risk Factor Prior to Fontan Completion in Complex Congenital Heart Disease Patients.

INTRODUCTION: The Glenn procedure is generally performed as a second-stage palliative procedure toward the completion of Fontan circulation in patients with a functionally univentricular heart. Safe Fontan completion requires normal ventricular function, competent atrioventricular valves, normal pulmonary arteries anatomy, and low pulmonary vascular resistances (PVRs), which is merely an estimation that considers both lungs as a single unit. Clinical observations revealed that patients who previously underwent bilateral bidirectional cavopulmonary anastomosis (b-BCPA) had a stormier postoperat

Investigation of cardiac status and bone mineral density in Turner syndrome.

This review highlights recent developments in the detection and management of congenital heart disease and osteoporosis in patients with monosomy X, or Turner syndrome (TS). Magnetic resonance angiography (MRA) using gadolinium as a contrast agent demonstrates a higher prevalence and greater diversity of congenital cardiovascular defects than previously recognized in TS. Almost 50% of girls and women with TS have marked tortuosity or ectasia of the aortic arch, suggesting that these individuals may be at greater risk for aneurysm formation or dissection and therefore require closer monitoring.

Robotic Right Hepatectomy with Portal Vein Thrombectomy for Colorectal Liver Metastasis (with Video).

BACKGROUND: Hepatectomy is the standard treatment for colorectal liver metastases. However, the high recurrence rate is a persistent problem that occurs in up to 65% of patients. Repeat hepatectomy is a feasible treatment and may offer favorable surviva but is technically demanding so minimally invasive repeat hepatectomy has been used in a few patients. Colorectal liver metastases are different from hepatocellular carcinoma and rarely present with macroscopic portal vein tumoral thrombus. To the best of our knowledge, minimally invasive approaches for this rare condition have not yet been rep

Concurrent Chylothorax, Chylopericardium, and Superior Vena Cava Syndrome in a Young Male With Primary Mediastinal Large B-cell Lymphoma: A Report of a Rare Case.

Primary mediastinal large B-cell lymphoma (PMBCL) is an uncommon and aggressive subtype of non-Hodgkin lymphoma (NHL) that predominantly affects young adults and typically presents with a bulky anterior mediastinal mass. We report the case of a 23-year-old male who presented with progressive dyspnea, orthopnea, and facial swelling, subsequently found to have superior vena cava (SVC) syndrome, chylothorax, and chylopericardium, an exceedingly rare triad of complications. Diagnostic evaluation required integration of advanced imaging, fluid analysis, and histopathological confirmation, ultimatel

When Cancer Clots: An Extensive Radiologic Analysis of Cancer-Associated Thromboembolism.

Cancer-associated thrombosis (CAT) is a leading cause of morbidity and mortality in patients with malignancy, yet its imaging manifestations extend far beyond the conventional diagnosis of deep vein thrombosis and pulmonary embolism. This comprehensive review examines the full spectrum of CAT as encountered by radiologists, from routine venous thromboembolism to unusual-site thromboses, arterial thromboembolic events, catheter-related complications, and endovascular management strategies. Patients with cancer face a four- to seven-fold increased risk of venous thromboembolism compared with the

Anticoagulation for Cancer Patients in Special Situations: A Narrative Review of Guidelines and Literature.

Cancer-associated thrombosis (CAT) is a major cause of morbidity and mortality in patients with cancer. The management of special situations-including recurrent venous thromboembolism (VTE), thrombosis at unusual sites, and central venous catheter-associated thrombosis (CVC-AT)-remains particularly challenging because of the limited availability of high-quality evidence. This narrative review synthesizes recommendations from major international and Spanish clinical practice guidelines and expert consensus documents, including those from SEOM, ESMO, ASCO, NCCN, ITAC and SEMI, to provide a struc
